Holocene coastline evolution of Procida Island, Bay of Naples, Italy
We studied the sea-level change and the volcano-tectonic behaviour of the pericoastal belt of Procida and Vivara islands, which are part of the insular Phlegraean Fields in the Naples Bay, southern Italy, on the basis of geological, geomorphological, and archaeological markers. The recognition of two outcrops of submerged beach rocks and the use of many different archaeological remains allowed us to better constrain the morpho-evolutionary history of the study area in term of both temporal and palaeoenvironmental reconstructions. The archaeological heritage of the Phlegraean area is very rich, spanning from the Bronze Age to the Middle Age and presenting a high density of evidences, also in the submarine environment. On the other hand, the submarine geomorphological survey furnished a lot of new data for the construction of reliable eustatic and subsidence curves. Three major episodes of subsidence can be recognized in the Phlegraean area, set at the Bronze Age, Roman period, and Middle Age. The events related to the oldest period have been here strictly linked to the history of the Vivara-Procida geoarchaeological system, whereas the subsequent episodes recorded in the study area have been correlated to the Roman and Medieval episodes of subsidence recorded in part of the continental Phlegraean area. The curve relative to the sea-level changes reveals some differences with regard the other similar curves from the continental area, both in the amount of subsidence in the single time intervals and for the absence of positive motion during the post-Roman period. The curve relative to the subsidence rates is characterized by a cyclic alternation of periods with different rates, with a frequency of about 2 ka, due to the inversion of the volcano-tectonic vertical mobility

Doctors commitment and long-term effectiveness for cost containment policies: lesson learned from biosimilar drugs

In a context of scarce resources, doctors are required to take opportunity cost into account when prescribing treatments, while cost containment policies cannot overlook their active role in determining health care resource allocation. This paper addresses this issue, investigating the effects of cost containment measures in the market of biosimilar drugs that represent a viable and cost-saving strategy for the reduction of health care expenditure. The analysis focuses on a particular region in Italy, where several timely policies to incentivize biosimilar prescribing were launched. Methods: Drugs were identified by the anatomical therapeutic chemical classification system. Information about biosimilar drugs and their originator biological products was extracted from the IMS Health regional database. Drug consumption was expressed in terms of counting units, while expenditure was evaluated in Euro (€).The market penetration of biosimilars was analyzed by year and quarterly. Results: In the Campania region of Italy, the effects of cost containment policies, launched between 2009 and 2013, showed the prescription of biosimilars strongly increasing in 2010 until prescribing levels reached and exceeded the market share of the reference biological products in 2012. After a slight reduction, a plateau was observed at the beginning of 2013. At the same time, the use of the originator products had been decreasing until the first quarter of 2011. However, after a 1-year plateau, this trend was reversed, with a new increase in the consumption of the originators observed. Conclusion: Results show that the cost containment policies, applied to cut health expenditure "to cure and not to care", did not produce the cultural change necessary to make these policies effective in the long run. Therefore, top-down policies for cost containment are not successful; rather, a bottom-up approach based on consensus among professionals should become the preferred option. On the Role of Roughness in the Indentation of Viscoelastic Solids
A numerical boundary element methodology is employed to understand how fractality intervenes when a 1D rigid rough profile indents a linear viscoelastic half-plane. The focus is, in particular, on the viscoelastic dissipation and how this is influenced by the profile statistical parameters, namely, the mean square roughness h(rms) of the profile, the mean square slope m(2) and the Hurst exponent H. Our numerical investigation, properly supported by a dimensional analysis, reveals that, in the one-dimensional case under investigation, the leading role is played by h(rms) and, thus, mainly by the large scales of the rough spectrum. Clearly, on an experimental level, this implies that a simple measure of the roughness parameter h(rms) is sufficient to determine the viscoelastic dissipation
The peculiar case of Myxicola infundibulum (Polychaeta: Sabellidae): echo from a science 200 years old and description of four new taxa in the Mediterranean Sea
AbstractMyxicola infundibulum Montagu, 1808 is the most reported species of its genus, showing an unusually wide distribution from the Mediterranean area to Australia, North Europe, and North America, a situation deriving from a wide synonymizing of numerous species with M. infundibulum. Recently, genetic analysis confirmed that the Australian form of this species is an introduced taxon from the Mediterranean area, while the examined North American specimens were genetically and morphologically different. In the present paper we travel through the history of M. infundibulum from the first descriptions, trying to trace both the origin of this taxon and the origin of its wide distribution, through an analysis of the descriptions of all valid and invalid taxa to date. We also examined material present in the collection of one of the authors previously identified as M. infundibulum, comparing Mediterranean material to some from the English Channel, and material from North America. This led to the erection of four taxa new to science from material recently collected along the Italian coasts, and the restoration of Myxicola pacifica Johnson, 1901. Delimitation of taxa is based only on morphology, and we propose new morphological features to be considered; however, a molecular examination is planned in the near future.http://zoobank.org/urn:lsid:zoobank.org:pub:7CF64CB2-8B9C-4001-BE57-199B5F95213
