    Dacryocystorhinostomy an endonasal approach is a surgical procedure which permits removal of symptoms related to chronic stenosis of the naso-lacrimal duct without leaving external scars and without presenting risk for the orbital structures. The Author describes the surgical technique employed in 12 surgical cases. Use of the frontal loupe and of the operating microscope allows the surgeon's hands to remain free while all the advantages of binocular vision are maintained. Pre-operative investigation must always include a dacryocystography, a CT of the paranasal sinuses and an endoscopic examination of the nasal cavities

    Results of endoscopic middle ear surgery for cholesteatoma treatment: a systematic review

    Traditional surgery for cholesteatoma of the middle ear is performed by microscopic approaches. However, in recent years endoscopic instrumentation, techniques and knowledge have greatly improved, and in our opinion endoscopic surgical techniques will gain increasing importance in otologic surgery in the future. The aim of this study was to focus on outcomes obtained using endoscopic surgery for the treatment of middle ear cholesteatoma. A systematic review of the literature was performed. A total of 7 articles comprising 515 patients treated exclusively with endoscope or with a combined technique were found. During post-surgical follow-up, 48 (9.3%) patients showed a residual or recurrent pathology. Despite the small number of patients analyzed in our review, the outcomes of this technique appear to be promising. In particular, concerning the rates of recurrences and residual disease, endoscopic middle ear surgery appears to guarantee similar results in comparison to classic microscopic approaches with the advantage of performing minimally invasive surgery

    This course originates from the close collaboration experienced and established over many years between the three professionals most involved in the surgical treatment of lesions of the anterior skull base. We believe, and the results have proved us right, that the achievement of the best treatment in such complex anatomical regions can be born only from an accurate preoperative study, the realization of several surgical procedures and the care of every detail during the post-operative period carried out in a close collaboration among Otolaryngologists, Maxilo-Facial surgeons and Neurosurgeons. The relatively recent introduction of endoscopic surgery and the huge expansion of the latter has further expanded the possibilities of collaboration. On the other hand those who want to approach to this type of pathologies necessarily have to know not only the techniques and possibilities of endoscopy but also the open techniques which are essential to face certain situations or clinical manifestations. The experience of many years of shared work between two surgical university schools with great traditions such as Modena and Verona besides a personal friendship established over time has enabled us both to joint the three specialties and to create a group that is inspired by the same principles

    The anatomy of the middle ear is very complex. The otologist must have a good knowledge of the structures comprising the tympanic cavity; a clear view of all the subsites of this region is essential to ensure the complete removal of all pathology. The development of endoscopic techniques to access the middle ear has offered the possibility to “look around corners” and explore hidden recesses the microscope cannot reach. Moreover, by leading to a better understanding of the different ventilation pathways of the tympanic cavity, alterations of which may cause pathologies such as cholesteatoma, endoscopy represents a surgical approach aimed toward restoring normal physiology as well as eradicating disease.
