DOHaD - the challenge of translating science into policy
The DOHaD Society has passed its 10th birthday, so it seems an appropriate time to reflect on what has been achieved and the Society's aspirations. At the 10th International Congress in Rotterdam in November 2017, Peter Gluckman (the Society's first President) delivered a plenary lecture entitled 'DOHaD - addressing the science-policy nexus: a reality check'; in opening the Congress, Mark Hanson (second, and outgoing President) not only highlighted the success of the Society but also the challenges it now faces in achieving impact for its work in the global health arena, that is beyond the research agenda; and in assuming the role of third President, Lucilla Poston highlighted the need for the Society to grasp opportunities to change healthcare policy, while persevering with basic research and well-planned intervention studies. In this review we summarize the points made in these three presentations and issue a call to action to the membership to take up the challenge of taking the Society's work to the next level of translating science to policy
The Urban Hierarchies of China and the United States
In the year 2012, China had 177 urban agglomerations (referred to as high-density built-up areas, or “shiqu” in Chinese) with populations of over 750 thousand inhabitants. These 177 areas are the commonly known major “cities” of China. Of these cities, Chongqing had the largest population numbering almost 17.7 million inhabitants. Shanghai followed at almost 13.5 million, and then Beijing at almost 12.2 million. Of special interest in this chapter is the fact that even though Chongqing has more inhabitants than Shanghai which has more inhabitants than Beijing, we will show that Beijing is the most “dominant” of the cities in China and occupies the top place in the urban system. By comparison, the United States in the year of 2012 had 69 urban agglomerations (referred to in the U.S. as metropolitan statistical areas) with populations of 750 thousand or more. The New York-Northeastern New Jersey-Long Island area has the largest population at over 18.9 million, followed by Los Angeles-Long Beach-Santa Ana at over 12.8 million, and Chicago-Joliet-Naperville at almost 9.5 million. It turns out that these three largest metro areas in the U.S. are also the three most “dominant” of all the U.S. metro areas in the same order as the size of their populations. Our analysis will demonstrate that these giant cities usually rank at the top layer of the country’s urban system
Mechanisms underlying the programming of small artery dysfunction: review of the model using low protein diet in pregnancy in the rat
Human and animal studies have shown that unbalanced maternal nutrition is associated with the development of cardiovascular and metabolic disease in adulthood. In the Southampton maternal low protein model (SMLP), protein deprivation (50%) throughout pregnancy in rats leads to elevated blood pressure in adult offspring. Impaired peripheral arterial function may contribute to the cardiovascular dysfunction observed in these offspring. This review discusses the impact of such a dietary insult on the vascular function of resistance arteries from pregnant rats (pF 0 ), their offspring (F 1 ), the pregnant offspring (pF 1 ) and the second generation (F 2 ). At each stage, disturbances in endothelium-dependent relaxation were observed, implicating changes in endothelial nitric oxide (NO)-guanylate cyclase (GC) signalling pathway in the vascular adaptations to pregnancy and the programmed effects on offspring
Dietary restriction in pregnant rats causes gender-related hypertension and vascular dysfunction in offspring
We have investigated the effects of moderate global undernutrition during gestation in the rat on the blood pressure of male and female offspring, and on the development of systemic vascular function. Pregnant Wistar rats were nutritionally restricted (R) by feeding with 70% of the normal gestation-matched dietary intake from 0 to 18 days gestation. R offspring were growth retarded at birth but of similar weight to controls (C) at 20 days. Systolic and/or diastolic and mean arterial blood pressures, measured directly by femoral artery catheter, were elevated from 60 days onward in male R offspring (mean arterial pressure: day 60, P < 0.01; day 100, P < 0.05; day 200, P < 0.005, R vs. C), and from 100 days onward in female R offspring (mean arterial pressure day 100 and day 200, P < 0.05; R vs. C). Maximal constriction to phenylephrine (PE) (P < 0.05) and to noradrenaline (NA) (P < 0.05) was reduced in isolated femoral arteries of day 20 R pups. These differences did not persist into adulthood. In male adult R offspring (200 days), maximal vasoconstriction to the thromboxane A2 mimetic, U46619 (P < 0.05) and sensitivity to potassium (P < 0.01) were enhanced. Moderate maternal undernutrition in rat gestation adversely affects cardiovascular function in the offspring. These abnormalities increase with age and are more pronounced in males
Fertility in China in 2000: a county level analysis
In order to maintain itself into the future, the People��s Republic of China undertook in the 1970s a legendary demographic endeavor dealing with the artificial constraint of population growth. The ��later, longer, fewer�� policy and the more rigid one-child policy were efforts to expedite the demographic transition in the country. The ultimate goal was the stabilization and eventual decline of the population, via fertility at below-replacement levels for an extended period of time. According to the 2000 census, the total fertility rate (TFR) for China was 1.22��well below 2.1, the replacement level of fertility. However, the country��s TFR fluctuated spatially with rates of .86, 1.08, and 1.43, for cities, towns, and rural areas, respectively. Undoubtedly, China��s family planning policy is largely responsible for the nation��s current low fertility, as well as the geographical variation in fertility just mentioned. Research has shown, however, that other factors have played a part in this fertility transition and the subsequent variation at the regional, provincial, and county levels. In keeping with the expectations of demographic transition theory (DTT), quantitative studies conducted over the last twenty years have linked an assortment of socioeconomic factors with China��s fertility decline and nationwide inconsistencies (Birdsall and Jamison 1983; Tien 1984; Poston and Gu 1987; Freedman et al. 1988; Peng 1989; Poston and Jia 1990; Poston 2000). My thesis built on and extended the above work, using the newly available demographic data provided by Census 2000. I tested the efficiency of demographic transition variables in explaining the variation in the TFR among the counties of China by estimating twelve Ordinary Least Squares (OLS) regression equations. Specifically, I examined the ways in which variables such as ethnicity, agricultural detachment, urbanization, economic conditions, cultural norms and gender differences were related to Chinese fertility in a nationwide analysis and in two region-specific analyses. My results showed rather definitively that demographic transition theory is applicable for predicting and understanding fertility among the counties of China. Irrespective of the nation��s extensive family planning policy, it is apparent that other factors contribute to the varying fertility rates across the country
