    Dickens’s consumptive urbanity: science and sentiment in nineteenth-century healthcare

    The representations in the oeuvre of Charles Dickens of pulmonary tuberculosis (TB) characterize him as a realist and, at the same time, as a sentimentalist who saw TB as the result of hard-heartedness and the disintegration of communal ties based on affection and mutual support. Using the critical approach of the medical humanities, this article relates Dickens’s understanding about disease and therapy to problems in contemporary healthcare like the unexpected rise of TB in the 1980s, the challenge posed by drug-resistant bacteria, and the discontents with the assembly-line modes of professionalized medical services. It argues that modern health policy and practice can profit from the records found in great works of literature of how communities in previous eras fought disease

    Acquisition of medical register through the communicative act of speaking

    The study explores the role of speaking skills in the acquisition of medical register by international students learning Bulgarian as a foreign language in their preparatory year at the Medical University – varna. Speaking as a productive language skill is particularly effective for the acquisition of the medical terminology that is most common, appears in speech with high frequency, and is typical mainly for the low and middle register. although the study examines some of the challenges that the students experience specifically with Bulgarian oral speech (intonation, stress, pausing, clarity of articulation), it bears implications for the theory and practice of teaching the host country’s tongue to international students of medicine or medical interns. it concludes that the objectives of language courses for international students cannot be limited to the students’ acquisition of medical register for academic purposes, but inevitably ought to address their needs as future interns who will interact with patients and medical staff with different language and culture

    Thermopower and thermoelectric power factor of Zk parafermion quantum dots

    AbstractUsing the conformal field theory approach to the thermoelectric characteristics of fractional quantum Hall states, previously developed in Georgiev (2015) [10], we show that the thermoelectric power factor of Coulomb-blockaded islands, realized by point contacts in Fabry–Pérot interferometers in the Zk parafermion Hall states, could give reliable signatures for distinguishing the topological orders of different quantum Hall states having identical electric properties. For example, while the conductance peak patterns in the Coulomb blockade regime for such states are practically indistinguishable for vn≪vc even at finite temperature, where vn and vc are the Fermi velocities of the neutral and charged modes respectively, the power factors PT of the corresponding states are much more sensitive to the neutral modes. In particular, the smaller r=vn/vc the bigger the asymmetries in the power factor which combined with the thermal broadening of the conductance peaks due to the neutral modes' multiplicities could give us the ultimate tool to figure out which of the competing quantum Hall universality classes are indeed realized in the experiments. We give a complete description of the power factor profiles in the Z3 and Z4 parafermion states with arbitrary number of quasiparticles localized in the bulk which could be useful for comparison with the experiments

    Applying medical discourse in ergonomics courses for students of technical sciences

    The present paper explores the role of medical terminology in the teaching of ergonomics to students of technical sciences. it aims to describe and encourage certain positive practices in teaching "ergonomics and ergonomic design" (eed), a course for students of technical sciences in Bulgarian universities, the technical University of Varna in particular. The observations are based on the texbook and study aids for eed used by industrial design students who, in their second year at university, are enrolled in the course "ergonomics and ergonomic design.

    The article explores an aspect of American writer Lois Lowry`s award-winning adolescent novel The Giver (1993) that has not been sufficiently examined in criticism, namely, the author`s stance toward the general enthusiasm among scientists and politicians about the possibilities opened by the latest advances in science and technology to perfect human societies and improve the individual subject`s experience of life. Presenting the failure of a utopian project based on all-pervading social control and, on the other hand, biotechnology, the novel offers a cautionary tale against the dangers stemming from instrumental, soulless biopolitics. Lowry believes that public order and epidemiological control promoted by technocrats can give us physical safety, but can take a toll on our emotional health and intellectual development. As an alternative to the technocratic utopia Lois Lowry`s novel proposes social spaces organized on the basis of sentiment as the most natural political arrangement that guarantees human happiness and the fulfilment of human potential

    In the past few years, blended learning has become more and more rapidly spread in the field of education worldwide. The idea of blended learning seems attractive because it allows for the preservation of the traditional forms of education resulting from accumulated pedagogical experience for centuries and we could altogether use the various educational functions of new technologies. The methodology developed by the author introduces a key subsystem from the integrated management information system of Veliko Tarnovo University, namely the ‘E-University’ subsystem. On the base of own experience and the analyzed research, there is an opportunity to use different media, individualize the learning process, opportunity to use different styles of learning, organizing joint learning activities. The successful application of this approach together with the organization of education and the quality of the virtual environment require the presence of the human factor which is able to combine different types of learning environments in order to provoke  students to perform various types of activities and develop their potential abilities and talents in the future.U poslednjih nekoliko godina, mešovito učenje se sve brže širi na polju obrazovanja širom sveta. Ideja kombinovanog učenja deluje privlačno jer omogućava očuvanje tradicionalnih oblika obrazovanja koji proističu iz vekovima akumuliranog pedagoškog iskustva kroz korišćenje različitih obrazovnih funkcija novih tehnologija. U radu je predstavljena metodologija razvijena od strane autora koja predstavlja ključni podsistem integrisanog informacionog sistema upravljanja Univerziteta u Velikom Trnovu, pod nazivom “E – Univerzitet” podsistem. Na osnovu analize dosadašnjih istraživanja i sopstvenog iskustva, predstavljene su mogućnosti korišćenja različitih medija u cilju individualizacije procesa učenja, korišćenja različitih stilova učenja, organizovanja aktivnosti grupnog učenja. Uspešna primena ovog pristupa, zajedno sa organizacijom obrazovanja i kvalitetnim virtuelnim okruženjem zahteva prisustvo ljudskog faktora koji je sposoban da kombinuje različite tipove okruženja za učenje kako bi provocirao studente da obavljaju različite tipove aktivnosti i razvijaju potencijalne sposobnosti i talente u budućnosti

    Background: The influence of periprocedural ischaemia on coronary artery bifurcation stenting (percutaneous coronary intervention [PCI]) remains uncertain. Aim: To determine the differences in rates of end procedural ischaemia after bifurcation lesion PCI detected with intracoronary electrocardiography (icECG). Methods: Unipolar icECGs were recorded before, during, and after stent placement and at the end of procedure in side branch (SB) and main branch (MB). Coronary wire was placed in all distal vessels with diameter > 1.5 mm to “map” the distal zones of ischaemia. The patient population consisted of patients with stable/unstable angina with troponin I evaluated before and after PCI. Results: We studied 147 patients (68% males) with mean age of 64 ± 9 years. One hundred and forty-two patients had icECG recordings at the end of PCI from all locations of the treated region; 36% of patients had MB ST segment elevation (STE) and 31% had icECG STE in the SB region (p = 0.378). The icECG had sensitivity of 82% and specificity of 81% to detect troponin I elevation, with positive predictive value of 81% and negative predictive value of 83%. The independent predictors of troponin increase (> 5 × N) were: sex (for female gender, OR = 0.130, CI 0.017–0.995, p = 0.049), previous myocardial infarction (OR = 33.23, CI 2.802–394.1, p = 0.005), and icECG STE in MB or SB or occlusion of secondary SB (OR = 7.877, CI 2.474–25.07, p < 0.001) and for any troponin elevation were double product — SBPxHR (OR = 0.999, CI 0.999–1.00, p = 0.022) and icECG STE in MB or SB or occlusion of secondary SB (OR = 9.762, CI 3.273–29.12, p < 0.001). Conclusions: Intracoronary electrocardiography is a highly sensitive and specific method for determination of ischaemic regions and prediction of elevated troponin I.Wstęp: Nie określono dotychczas wpływu niedokrwienia okołozabiegowego na wynik wszczepienia stentu w obrębie rozwidlenia tętnic wieńcowych (przezskórna interwencja wieńcowa [PCI]). Cel: Badanie przeprowadzono w celu ustalenia różnic w częstości występowania niedokrwienia wykrytego za pomocą elektrokardiografii wewnątrzwieńcowej (icECG) po zakończeniu PCI w obrębie rozwidlenia tętnic wieńcowych. Metody: U chorych wykonano jednobiegunowy zapis icECG przed, w trakcie i po zabiegu wszczepienia stentu, a także po zakończeniu procedury w gałęzi bocznej (SB) i głównej (MB). Cewnik wieńcowy umieszczono we wszystkich dystalnych naczyniach o średnicy > 1,5 mm w celu mapowania dystalnych obszarów niedokrwienia. Badana populacja obejmowała chorych ze stabilną/niestabilną dławicą piersiową, u których stężenie troponiny I było podwyższone przed i po PCI. Wyniki: Do badania włączono 147 chorych (68% mężczyzn, średnia wieku 64 ± 9 lat). U 142 pacjentów po zakończeniu PCI wykonano icECG z wszystkich leczonych okolic; u 36% chorych stwierdzono uniesienie odcinka ST (STE) w zapisie z MB, a u 31% stwierdzono STE w zapisie icECG z okolicy SB (p = 0,378). Badanie icECG cechowało się 82-procentową czułością i 81-procentową swoistością w wykrywaniu zwiększonego stężenia troponiny, a wartość predykcyjna dodatnia i ujemna wynosiły odpowiednio 81% i 83%. Niezależnymi czynnikami predykcyjnymi wzrostu stężenia troponiny wynoszącego > 5 × N (norma) były: płeć (w przypadku kobiet OR = 0,130; CI 0,017–0,995; p = 0,049), przebyty wcześniej zawał serca (OR = 33,23; CI 2,802–394,1; p = 0,005) oraz STE w zapisie icECG w MB lub SB, lub okluzja drugorzędowej SB (OR = 7,877; CI 2,474–25,07; p < 0,001), a niezależnymi czynnikami jakiegokolwiek wzrostu stężenia troponiny były: produkt podwójny SBPxHR (OR = 0,999; CI 0,999–1,00; p = 0,022) oraz STE w zapisie icECG w MB lub SB, lub okluzja drugorzędowej SB (OR = 9,762; CI 3,273–29,12; p < 0,001). Wnioski: Badanie icECG jest wysoce czułą i swoistą metodą określania obszarów niedokrwienia i prognozowania zwiększonego stężenia troponiny I.
