La pubblicazione nell’ADI Design Index costituisce preselezione per il Premio Compasso d’Oro.
Il volume dell’ADI design Index 2009 è stato presentato nel giugno 2010 presso la Triennale di Milano, Salone d’Onore.

Onychomycosis caused by nondermatophytic molds: Clinical features and response to treatment of 59 cases
Background: Nail invasion by nondermatophytic molds (NDM) is considered uncommon with prevalence rates ranging from 1.45% to 17.6%. Objective: We report the clinical features and response to treatment of onychomycosis caused by these molds. Methods: From 1995 through 1998 we performed a mycologic study on 1548 patients affected by nail disorders, and we diagnosed 431 cases of onychomycosis including 59 cases of onychomycosis caused by molds. These include 17 patients with onychomycosis caused by Scopulariopsis brevicaulis, 26 patients with onychomycosis caused by Fusarium sp, 9 patients with onychomycosis caused by Acremonium sp, and 7 patients with onychomycosis caused by Aspergillus sp. Results: Onychomycosis caused by S brevicaulis, Fusarium sp, and Aspergillus sp may often be suspected by clinical examination. In fact 38 of 50 patients with onychomycosis resulting from these molds were affected by proximal subungual onychomycosis associated with inflammation of the proximal nailfold. In our experience mold onychomycosis is not significantly associated with systemic diseases or immunodepression. NDM are difficult to eradicate; by using and combining different treatments (systemic itraconazole, systemic terbinafine, topical terbinafine after nail plate avulsion, and ciclopirox nail lacquer) we were able to cure only 69.2% of patients with S brevicaulis onychomycosis, 71.4% of patients with Acremonium onychomycosis, and 40% of patients with Fusarium onychomycosis. Aspergillus onychomycosis, on the other hand, responded very well to therapy and all our patients were cured after systemic or topical treatment. Eradication of the mold produced a complete cure of the nail abnormalities in all the patients who responded to treatment. Conclusion: Clinical examination usually suggests diagnosis of onychomycosis resulting from NDM. Topical treatment can be more successful than systemic therapy to cure onychomycosis caused by S brevicaulis, Fusarium sp, and Acremonium sp

Ha esposto nelle principali città italiane. Il percorso espositivo, definito e caratterizzato da elementi di allestimento modulari e smontabili, assume differenti configurazioni in rapporto ai diversi spazi espositivi. Onychomycosis due to Scopulariopsis brevicaulis: Clinical features and response to systemic antifungals
Six cases of Scopulariopsis onychomycosis, including four patients with onychomycosis exclusively caused by Scopulariopsis brevicaulis and two patients with a mixed nail infection (S. brevicaulis + Tricophyton rubrum and S. brevicaulis + T. interdigitale), are reported. Four patients presented with a typical distal subungual onychomycosis characterized by subungual hyperkeratosis and onychomycosis of the distal nail plate. In two patients, Scopulariopsis infection produced a total dystrophic onychomycosis associated with painful periungual inflammation. Three patients were treated with four pulses of itraconazole, 400 mg daily for 1 week a month, and three patients with terbinafine, 250 mg daily for 4 months. The mycological examination 8 months after discontinuation of treatment showed that one patient was mycologically cured whereas the remaining five patients still carried S. brevicaulis in their nails. The clinical examination at the end of the follow-up period showed a complete cure of the nail abnormalities in only one patient
Nail Disorders in Children
Nail diseases in children do not account for a significant proportion of pediatric consultations, and most of the time the nails are not observed by the clinician, overlooking their importance. Specific examination of the nails is neglected, while localization to the nails could be an initial sign of a syndrome or a systemic disorder. Nail diseases in the pediatric population differ from those in adults in terms of diagnostic approach and management; some of them even are manifested mainly or exclusively in children. Pediatric patients with underlying systemic disorders are more likely to manifest acquired disorders of the nails. Although rare, nail diseases in children are a source of anxiety for the parents. Examination of the nails is an essential part of pediatric physical examination. A correct clinical history and careful examination help the clinician to distinguish the different conditions and to decide on the correct management of nail diseases in young patients. A classification of nail dystrophies according to age is somewhat arbitrary and a unique classification does not exist. Nail diseases in the pediatric population can be divided according to age groups where a predilection appears in most of the cases. Moreover, certain abnormalities may be lifelong once acquired, but their presentation may be modified by age, worsening or improving during life. This review describes many of the nail conditions that are seen in the pediatric population aging from newborn to toddler, starting with physiological aspects to better recognize the pathological conditions
Stressing the passive behavior of a Passivhaus: An evidence-based scenario analysis for a Mediterranean case study
This paper first reports the outcomes of a one-year measurement campaign of a passive house built in the Mediterranean climate of Cesena (Italy) in terms of thermal comfort parameters temperature and relative humidity and Indoor Environmental Quality (IEQ) parameter CO2 concentrations. The design carried out with the help of the steady state Passive House Planning Package (PHPP) was able to guarantee good comfort conditions during the heating period, but on the other hand, overheating occurrences during the cooling season have been recorded for almost 50% time according to EN 15251 Standard. Further analyses conducted with the help of dynamic simulations in EnergyPlus allowed identifying the insulation levels and ventilation mode as the key design factors to change in order to reduce overheating to less than 20% of time while keeping a comfortable indoor environment in winter. The simplifications that can be made by reducing the insulation material thickness (up to a third of the original value) on the roof and on the walls, replacing triple-glazed windows with double-glazed windows and implementing a hybrid ventilation strategy instead of using Mechanical Ventilation with Heat Recovery (MVHR) alone could also lead to economic savings. These savings, due to both lower construction costs and operational energy savings, amount to 8755 euros in terms of Net Present Value (NPV) over 30 years' time. The Passivhaus Standard can still be regarded as a good reference for designing low-energy and comfortable houses in a Mediterranean climate if some simplifications are made according to detailed building performance simulations
Frontal Alopecia after Repeated Botulinum Toxin Type A Injections for Forehead Wrinkles: An Underestimated Entity?
BACKGROUND:
Injections of botulinum toxin type A in the forehead have never been reported to cause hair side effects.
OBJECTIVE:
The aim of this paper is to report a new type of alopecia, which we have seen in women undergoing periodic injections of botulinum toxin type A for forehead wrinkles, and to differentiate it from other types of hair loss.
METHODS:
We conducted an observational study on 5 females recruited from a private and an institutional practice who complained of progressive recession of the hairline after periodic injections of botulinum toxin type A in the forehead.
RESULTS:
Alopecia of the frontal hairline was evident in all 5 patients, with absence of skin atrophy or scarring and progressive hair miniaturization at trichoscopy.
CONCLUSION:
Dermatologists should be aware of the possible occurrence of frontal alopecia after repeated injections of botulinum toxin type A for forehead wrinkles.

Tinea Incognita following the Use of an Antipsoriatic Gel
Tinea incognita is a dermatophyte infection of the skin whose clinical presentation has been modified by the misuse of steroids or, as has been described recently, calcineurin inhibitors. We report a case of pustular psoriasis treated with an antipsoriatic cream, composed of topical steroids and a vitamin D3 derivative, which gave rise to a tinea incognita. Our case underlines that clinical features of fungal infection can be modified by the increasing use of self-prescribed topical therapies, usually applied to treat incorrectly 'self-diagnosed' diseases. Moreover, we suggest that a mycological examination should be performed in every atypical presentation of skin lesion, and to rule out tinea pedis, in any disease of the feet that requires topical application of steroids
