End-stage respiratory diseases and respiratory support
This chapter will focus on reviewing the current literature on care of dying patients with end-stage respiratory disorders in the last days of their life. First, the focus will be on recognising, assessing and relieving the most typical late-stage symptom: breathlessness (dyspnoea). Secondly, specific challenges to face when looking after these patients will be discussed: assisted breathing, oxygen use, mechanical ventilation withdrawal, medications and the role of palliative care. Lastly, ancillary topics will be covered, such as ethics, spiritual needs, and patients’ and their families’ perspectives
A finite state Markov quantizer for speech coding
A low-bit-rate codec based on an ergodic hidden Markov model (EHMM) is presented. A 256-state autoregressive Gaussian EHMM is trained on speech uttered by eight different speakers by means of the Baum-Welch algorithm. Initial estimates are obtained from vector quantization. The resulting EHMM is then utilized for a Viterbi decoding of incoming speech data. The state sequence obtained is frame synchronously encoded. The bit rate is gradually lowered by cutting off low-probability transitions and thus reducing the destination state encoding bit allocation requirements. The encoded spectra sequence is used, on the receiver side of the codec, for linear predictive coding synthesis. Global entropy and distortion measures for different bit rates are reported and compared to vector quantization results. Il volume documenta lo svolgimento di alcuni cicli di trattamento shiatsu visti con gli occhi della persona “ricevente”, utilizzando lo strumento dei “Diari di salute e di cura” (DSC) che portano alla luce i processi salutogenici – ossia rigenerativi, energetici, di resilienza e miglioramento della qualità di vita – che la tecnica shiatsu riesce a generare, ma anche le pratiche di cura di sé che il soggetto attiva nel periodo esaminato. L’indagine conferma la capacità che ha lo Shiatsu di stabilire un contatto corporeo, emozionale, personalizzato col soggetto passando per i canali somatici. Per la sua specificità, questa disciplina – condotta in modo professionale – costituisce una risorsa che dovrebbe essere meglio conosciuta da parte del pubblico e degli altri professionisti della cura al fine di contribuire al benessere soggettivo e sociale
Chemical composition of the oil and supercritical CO2 extract of Schinus molle L.
Supercritical carbon dioxide extraction allowed Schinus molle L. volatile oil to be obtained. Extraction conditions were as follows: pressure, 90 bar; temperature, 50 °C, carbon dioxide flow, Φ = 1.0 kg/h. Waxes were entrapped in the first separator set at 90 bar and -10 °C. The oil was recovered in the second separator, working at 15 bar and 10 °C. The main components were α-phellandrene (26.5%), limonene + β-phellandrene (21.0%), elemol (10.8%) and α-eudesmol (6.1%). Comparison with the hydrodistilled oil (HD) did not reveal any large difference, whereas the significant difference between the SFE and SD oils was the content of sesquiterpenes, which is higher in the SFE products. Collection of samples at different extraction times during supercritical extraction allowed changes of the oil composition to be monitored. Lighter compounds, as hydrocarbon monoterpenes, were extracted in shorter times than the heavier hydrocarbon and oxygenated sesquiterpenes
Seasonal spatial ecology of the wild boar in a peri-urban area
The parallel expansion of anthropogenic environments and wild boar Sus scrofa populations has favoured the intrusion of the species into many European metropolitan areas, including Rome. In this study, we used Maxent to analyse the seasonal spatial ecology of the wild boar in Marcigliana natural reserve, a rural area located in the northeastern suburbs of Rome. The wild boar avoided urban settlements year-round. In the growing season, it had a higher probability of occurring with an increasing presence of turkey oak Quercus cerrioides woods, but seemed to be partially adapted to the presence of arable lands, which had a marked negative effect only when very abundant. These results lead us to hypothesise that the species in this part of the year adopts a spatial strategy which optimises the trade-off among the need for thermal cover and food resources. In autumn and winter, the species avoided meadows and pastures. The analysis of wild boar spatial ecology in metropolitan areas is essential to provide important information contributing to the development of effective plans for managing peri-urban populations and mitigating conflicts with humans
