Carotid endarterectomy or carotid artery stenting? Matching the patient to the intervention

    No full text
Extracranial carotid disease accounts for approximately 25% of ischemic strokes. Although carotid endarterectomy (CEA) is the established gold standard for carotid revascularization, carotid artery angioplasty and stenting (CAS) is continually developing into a safer and more efficacious method of stroke prevention and has gained popularity as an alternative to CEA. Recent trials have reported clinical equipoise between CEA and CAS. There are certain patient characteristics that can increase the risk of adverse outcomes for both CEA and CAS. Proper patient selection is the key to successful outcomes when deciding the optimal treatment for carotid stenosis. Patients must be individualized, and a specific risk-benefit ratio must be formulated for CEA, CAS, and best-medical therapy (BMT). Ultimately, optimizing medical therapy and using CEA and CAS as complementary therapies rather than competing ones will likely achieve the best patient outcomes. © The Author(s) 2010

    Unifying logics via context-sensitiveness

    No full text
The goal of this article is to design a uniform proof-theoretical framework encompassing classical, non-monotonic and paraconsistent logic. This framework is obtained by the control sets logical device, a syntactical apparatus for controlling derivations. A basic feature of control sets is that of leaving the underlying syntax of a proof system unchanged, while affecting the very combinatorial structure of sequents and proofs. We prove the cut-elimination theorem for a version of controlled propositional classical logic, i.e. the sequent calculus for classical propositional logic to which a suitable system of control sets is applied. Finally, we outline the skeleton of a new (positive) account of non-monotonicity and paraconsistency in terms of concurrent processes

In 1941, namely the year of Henri Bergson’s death, Raymond Bayer publishes in Revue philosophique an inflammatory article on Bergson’s philosophy, wondering whether the French philosopher had ever delivered an actual aesthetic theory. Although it is well known that Bergson recurred, along his entire corpus, to several examples borrowed from art, and based his metaphysical assumptions on the psychological act of intuition, he never wrote a treatise on aesthetics, nor he dealt minutely with the problem of beauty. As shown by Bayer, this characteristic of Bergson’s philosophy raises several doubts: being his metaphysics essentially anchored on the concept of pure perception, and being this very perception a form of knowledge that primarily belongs to the artist, Bergson should have elaborated an aesthetic theory. Now the problem is to understand why he did not do that

    Fractional semantics for classical logic

    No full text
This article presents a new (multivalued) semantics for classical propositional logic. We begin by maximally extending the space of sequent proofs so as to admit proofs for any logical formula; then, we extract the new semantics by focusing on the axiomatic structure of proofs. In particular, the interpretation of a formula is given by the ratio between the number of identity axioms out of the total number of axioms occurring in any of its proofs. The outcome is an informational refinement of traditional Boolean semantics, obtained by breaking the symmetry between tautologies and contradictions
