Vitamin D levels in children and adolescents with attention-deficit hyperactivity disorder (ADHD): a meta-analysis
The aim of this article was to assess the differences in serum 25(OH)D levels between children and adolescents with attention-deficit/hyperactivity disorder (ADHD) and healthy controls. We used the PubMed (1966–2017), Scopus (2004–2017), ClinicalTrials.gov (2008–2017), Cochrane Central Register of Controlled Trials CENTRAL (2000–2017), and Google Scholar (2004–2017) databases. Statistical meta-analysis was performed with RevMan 5.3. Εight studies were finally included in the present meta-analysis with a total number of 11,324 children. Among them, 2655 were diagnosed with ADHD, while the remaining 8669 were recruited as healthy controls. All eight trials reported significantly lower serum concentrations of 25(OH)D in patients diagnosed with ADHD compared to healthy controls. The pooled data showed that there was a significant difference between the ADHD group and the control group (SMD = − 0.73, 95% CI [− 1.00, − 0.46]). The systematic review and meta-analysis of observational studies demonstrated an inverse association between serum 25(OH)D and young patients with ADHD. Large cohort studies are required to investigate whether vitamin D-deficient infants are more likely to develop ADHD in the future. Also, whether children with ADHD should be supplemented with higher doses of vitamin D3 remains to be confirmed through long-term controlled clinical trials. © 2018, Springer-Verlag GmbH Austria, part of Springer Nature
Kidney biopsy findings in vancomycin-induced acute kidney injury: a pooled analysis
Background: Acute kidney injury represents a major adverse effect of vancomycin administration. The aim of the present study is to accumulate all biopsy-proven cases of vancomycin nephrotoxicity and assess the association of histopathological features with renal prognosis. Methods: Medline, Scopus, CENTRAL, Web of Science, and Clinicaltrials.gov were systematically searched from inception to 29 September 2020. All case reports/series providing individual data of patients with biopsy-proven vancomycin nephrotoxicity were held eligible. A time-to-event analysis was performed evaluating the effects of histological diagnosis on renal recovery. Results: Overall, 18 studies were included, comprising 21 patients. Acute tubulointerstitial nephritis was the predominant pattern in 9 patients and was associated with a significantly higher risk of permanent renal dysfunction (HR: 5.08, 95% CI: [1.05–24.50)] compared to acute tubular necrosis. Tubulitis and eosinophilic infiltration were the most common histopathological findings, while interstitial fibrosis was linked to significantly worse renal prognosis (HR: 5.55, 95% CI: 1.13–27.27). Immunofluorescence and electron microscopy features were non-specific. Obstruction by tubular casts composed of vancomycin aggregates and uromodulin has been identified as a new mechanism of nephrotoxicity. Conclusions: Acute tubular necrosis and tubulointerstitial nephritis represent the main histological patterns of vancomycin-induced acute kidney injury. The presence of fibrosis in the context of interstitial inflammation may be linked to lower recovery rates and worse long-term renal outcomes. A novel cast nephropathy obstructive mechanism has been suggested, necessitating further confirmation. Large-scale studies should define the exact indications of kidney biopsy in cases with suspected vancomycin nephrotoxicity. © 2021, The Author(s), under exclusive licence to Springer Nature B.V

Maslyuzhenko D.N. Turko-Mongol Traditions in the “State of Nomadic Uzbeks” of Abu al-Khair Khan
The author of this article identifies the daily and political traditions in the Khanate of Abu al-Khair, which researchers traditionally call the “State of nomadic Uzbeks”. This State was created in the south of Western Siberia in the 1430’s and annexed by military means to the land of Shiban’s (the younger brother of Batu) descendants. The overwhelming majority of researchers consider it as a Muslim State since its very foundation, substantiating their allegations by indication both on the reading of khutba in honor of Khan after he took the “throne of Sain Khan” and on the presence of the sheikhs and Sufis in his entourage.
At the same time, the authority of Abu al-Khair had the strong support of nomadic elite of more than twenty tribes. Precisely this elite, its inquiries and interests, which were advocated on the nobility councils, formed the existing political and daily traditions. These traditions reflect the preservation of Turko-Mongol customs among nomadic populations. We can find analogies of these practices in the earlier customs among the precursors of post-Golden Horde State, i.e., in the Mongol Empire and the ulus of Jochi. Traditions of the Abu al-Khair’s Khanate represented a phenomenon of cultural inertia, which were restored in the conditions of Islamization and could become the adats. This effect is particularly significant in view of the specific attitude of Sufi tariqats to the phenomena of social life.
In political terms, of particular importance among these was the restoration of the Toy and the related kurultay that could take place in accordance with the existing steppe (animals) calendar. The khans restored the ritual of robes and belts’ distribution as the main mechanism for redeployment of posts and responsibilities among the steppe aristocracy. Military tradition was also focused on samples of the previous time, which was particularly manifested in the selection of wings and avant-garde. In the course of military operations, there was the prevalent practice of magical rites known since the early Middle Ages and aimed at influencing the weather by means of the magic stone “yada” and Yadachi-sorcerers
Cross-Cultural Meta-Analyses
In the enormous collection of cross-cultural data that have been published during the last few decades it is difficult to perceive patterns. There is a clear need for systematizing the vast amount of cross-cultural studies and for developing models that explain cross-cultural differences in psychology. Two methods of cross-cultural meta-analysis can be distinguished. First, the instrument-based method of comparing data for one instrument across countries is suitable for instruments which have been administered in many countries. Second, a domain-based meta-analysis used a thematic domain from which culture-comparative studies are sampled instead of one specific instrument or method
The Role of Tolvaptan Administration After Cardiac Surgery: A Meta-Analysis
Objective: To evaluate the efficacy and safety of tolvaptan in fluid management after cardiac surgery compared with conventional diuretic treatment. Design: Systematic review of the literature with meta-analyses. Setting: The Medline, Scopus, Cochrane Central Register of Controlled Trials, Clinicaltrials.gov, and Google Scholar databases were searched from inception to July 30, 2018. Participants: The study comprised 759 patients undergoing cardiac surgery. Interventions: Tolvaptan administration (n = 397) or standard diuretic therapy (n = 398). Measurements and Main Results: Ten studies were included in the present meta-analysis. Tolvaptan administration was associated with a significantly faster return to preoperative body weight (mean difference [MD)] –1.48 d, 95% confidence interval [CI] –1.92 to 1.03), shorter duration of hospital stay (MD –2.58 d, 95% CI –5.09 to –0.07), lower incidence of acute kidney injury (odds ratio 0.34, 95% CI 0.16-0.69), and greater urine output (MD 0.47 L/d, 95% CI 0.25-0.69) and sodium levels (MD 2.85 mEq/L, 95% CI 1.90-3.80). No significant differences were present regarding duration of intensive care unit stay (MD –0.09 d, 95% CI –0.33 to 0.15), arrhythmia incidence (odds ratio 0.58, 95% CI 0.33-1.02), and serum creatinine values (MD –0.08 mg/dL, 95% CI –0.20 to 0.04). Conclusions: The outcomes of the present meta-analysis suggest the promising role of tolvaptan administration in the management of fluid retention in patients after cardiac surgery. Future large-scale clinical trials should be conducted to fully elucidate its efficacy and to assess the optimal treatment protocol to be applied in the clinical setting. © 2018 Elsevier Inc
