Review of \u3ci\u3eTrue Tales of the Prairies and Plains\u3c/i\u3e By David Dary
The cover of True Tales of the Prairies and Plains notes that author David Dary is a past winner of the Cowboy Hall of Fame Award and the Western Writers of America Spur Award. One must assume that his previous books made better contributions to the scholarship of the American West than does this one. Fine for a casual reader or one interested in entertaining vignettes about the West, this work has little to offer the serious scholar.
The book is broken into several sections with such titles as Over the Trails and Rails, Buried Treasure Legends (so much for True Tales), The Lawless, Lawmen, and Justice, Buffalo, Horses, and Other Creatures, and The Famous and the Obscure. Within each section are a number of relevant short entries. Geographically, Dary presents stories from throughout the Great Plains. Texas, Kansas, Oklahoma, the Dakotas, Nebraska, and other Plains states receive much attention. Two of the strongest entries come from the Famous and the Obscure section, and both concern future presidents and their lives on the Plains. The story of young Dwight D. Eisenhower\u27s solicitation of an appointment to West Point or Annapolis is recounted in Two Letters from Abilene. (Interestingly, Eisenhower hoped for the Naval Academy but had to settle for West Point because he was too old for Annapolis.) The Badlands Rancher Who Became President explores Theodore Roosevelt\u27s ranching experiences in North Dakota in the 1880s
Review of \u3ci\u3eFrontier Medicine: From the Atlantic to the Pacific, 1492-1941\u3c/i\u3e by David Dary
This survey of medicine in the U.S. from European contact to World War II rambles from generalities to anecdotes in a manner much like the cowboy Dary describes in his preface, who started up one canyon and came out another. While the premise of the book might seem self-evident (that the practice of frontier medicine began long before the Oregon Trail), it offers little insight into the idea of the frontier, omitting Turner\u27s Frontier Thesis entirely, and relies more on chronology than context for its narrative. For these reasons, Frontier Medicine will be more useful to casual readers than to scholars as an accessible summary of U.S. medical history with some major gaps. 215: Management of hypertension by telemedicine: feasibility study and results on 100 patients
IntroductionAccording to the results of the national Health and study conducted in 2006-07, the prevalence of hypertension among 18 to 74 years old population is estimated at 31%. Among them, 20% are not treated. Among the ones who are treated, 49% are not controlled.ObjectivesThe goal of this observational study is to assess the feasibility to follow up blood pressure by telemedicine for 100 hypertensive patients to know the blood pressure profile and to adapt the treatment in real time to improve control. This work won the award “1 Mission 1 million”.Protocol of the studyThe follow up by telemedicine of the blood pressure figures is done by self measurement with 6 daily measures for not controlled hypertensive patients. The follow up is organized in 3 phases: inclusion, observation, treatment. Blood pressure is measured by oscillometry, result are transmitted directly and available for consultation on a secure web site.Patient profileMean age is 68 years old, 44 men and 56 women, 19% are not treated. 18% are treated for atrial fibrillation. Blood pressure at inclusion is 175/92.Observational Phase2 groups: 19 patients with a normal blood pressure (135/75), 81 patients not controlled (163/86) who continue the follow up by telemedicine during the treatment phase.Treatment PhaseThe adaptation of the treatment is achieved within 11 days with an average of 178/93 at inclusion, 163/86 during the observational phase and 144/78 after the last therapeutic adaptation. The decrease in blood pressure between the 2 phases is 19 mm HG for the systolic blood pressure and 8 mm for the diastolic one. It is obtained by adding another drug (47%) or changing the therapeutic class (22%) with an increase in dosage. 46% reach the objectives (<140) and 54% stay with 154 systolic on average.ConclusionTelemedicine is a simple method for follow up of blood pressure allowing a rapid adaptation of treatment to improve blood pressure control. 0030: Telemonitoring for arrhythmias on rural outpatients: feasibility and results on 167 patients
PurposeCardiac arrhythmias are undiagnosed because of their frequent transient and asymptomatic characteristic. This observational study on 167 patients assesses diagnosis and therapeutic advantages of electrocardiogram (ECG) remote monitoring on an at-risk population living in rural areas.MethodsA total of 167 patients referred for evaluation of cardiac arrhythmia underwent simultaneous ECG ambulatory recording with a Holter monitor. Arrhythmias were automatically detected and the tracings transmitted by phone to a secure server for heart rate (HR) analysis.This study main objective was to evaluate the detection of 6 different kinds of arrhythmias including supraventricular tachycardia (TC), atrial fibrillation (AF)/flutter, pause >3 seconds, atrioventricular block, ventricular TC or inappropriate sinusal TCAbstract 0030 - Figure: ArrythmiaResults167 patients with sinus rhythm. 39% men and 61% women, mean age of 64 years old. 9% had already experienced a stroke and 7% suffered coronary disease. 35% were taking an anti-arrythmic and 17% were taking anticoagulant. CHADS2Vasc2 = 2.Monitoring lasted 11 days, 11 hours per day, 248 ECG were transmitted. Results:-absence of abnormality (44%)-onset of AF (31%)-discovery of regular sinusal TC (25%) treated with heart rate control medicationsScreening and early detection of AF is one of the strategy main priorities to optimise anti-arrhythmia and anticoagulant treatment in real time. Early results show that 4 patients have an indication of implant placement and 3 for radiofrequency ablation. Both system and method are a quick and interesting solution to data transfer between medical experts and local family practitioners in order to take the most appropriate decisionsConclusionA long-term single-lead monitoring in detection of arrhythmia events on referred patients for ecg ambulatory monitoring is a simple method. The quality of life of rural population and retired people who require medical attention without moving could be significantly improved in term of diagnosis, treatment and drug prescriptions to prevent stroke (figure above)
0032: Analysis of blood pressure variability in the systolic hypertension with telemonitoring: feasibility and results on 108 patients
PurposeBlood pressure (BP) and its variability (BPV) are associated with an increased risk for cardiovascular mortality. This observational study explores the benefits of patients telemonitoring using self-measured BP to optimise treatment and its usefulness in the variability analysisMethodsPatients with uncontrolled hypertension were enrolled during an appointment. 2 SMBP were taken in the morning, at midday and in the evening at set times, the results being sent to a secure server. After 5 days treatment was started if the mean reading was more than 140/90. Variability analysis has been realized during all the follow up. The evaluation made at the end of the first 5 days to obtain the mean, standart Deviation (SD), coefficient of variation (CV), hight and low BPResults63 women and 45 men. BP = 176/96 at inclusion dropping to 160/ 88 after 5 days under the same treatment. Therapeutic adjustments achieved over 12.7 days with significant decrease in BP to 143/82: -17 systolic and -6 diastolic (p<0.0001)Results of variability are SD=16.63±5 and CV=0.105±0.03. There is no difference according age (<70 years CV 0.104/>70 years CV 0.106, p=0.582) or the level of BP (BP< 160mmHg CV 0.109/ BP< 160mm Hg CV 0.100; p= 0.100). At first variability is of 0.105 (32 measures, 4.7 days) with a not significant increase at the end: 0.112 (52 measures; p=0.098)When variability >0.10, the risk of low BP increases (104/118, p<0.0001; CV=0.136/0.087, p<0.0001), for the same mean BP(146/142, p=0.152), number of treatment (2.71/2.86, p=0.443) and for the same age (69,8/66,8, p=0.152) SBP of all measures is lowest at midday: 164mm Hg morning, 153mm Hg midday (-11mm, p<0.0001), 163mm Hg evening with the same difference at the end of follow up (147/138). The gap increases >74 years (-14mm de hg; 170/156)ConclusionsAwareness of the variance between average clinic and average telemonitoring BP may influence the diagnostic and management of hypertension.Telemonitoring of BP allows the real time measure of the mean, SD, CV and hight and low BP after modification or new treatment.The real time analysis allows the control of hypertension to improve (figure next page)

Retelling Slavic myths and Polish legends in the works of Witold Jabłoński
Celem artykułu jest przybliżenie sylwetki łódzkiego pisarza, Witolda Jabłońskiego, współczesnego prozaika i tłumacza, znanego zarówno miłośnikom fantastyki historycznej, jak i popularnego w ostatnich latach gatunku fantastyki słowiańskiej. Część interpretacyjną poprzedza biogram pisarza, przez całe życie związanego z Łodzią, mieszkającego tutaj i tworzącego. W drugiej części artykułu przyglądamy się pogańskim motywom w powieściach z cyklu Saga Słowiańska, tj. Dary Bogów, Popiel oraz Wanda, starając się prześledzić bogate tropy intertekstualne w retellingowych narracjach będących znakiem rozpoznawczym tego autora. Jabłoński wymieniany jest dzisiaj po Andrzeju Sapkowskim i Andrzeju Pilipiuku jako jeden z najpoczytniejszych autorów fantastyki. O jego popularności świadczyć może fakt, iż Dary Bogów przygotowano jako słuchowisko w serwisie Audioteka.pl, a bohaterom głosu użyczyli popularni aktorzy. Analiza ma na celu wskazanie, do jakich mitów słowiańskich i polskich legend państwowych sięga autor i za pomocą jakich narzędzi je przetwarza. Podczas interpretacji sięgamy do różnych metod pracy z tekstem: intertekstualnych, kulturoznawczych, ale odwołujemy się również do religioznawstwa, antropologii i komparatystyki religijnej.The article provides an overview of the profile of Witold Jabłoński, a contemporary Polish writer and translator from Lodz, known to enthusiasts of both historical fantasy and the recently popular genre of Slavic fantasy. The interpretative section is preceded by the writer’s biography, showcasing his lifelong connection to Lodz, where he resides and creates. In the second part of the article, we delve into pagan motifs in the novels from the Slavic Saga series, namely Dary Bogów, Popiel and Wanda, attempting to trace rich intertextual tropes in the re-telling narratives that characterize this author. Jabłoński is mentioned today alongside Andrzej Sapkowski and Andrzej Pilipiuk as one of the most widely read fantasy authors. The fact that Dary Bogów has been adapted into an audiobook on Audioteka.pl, featuring popular actors lending their voices to the characters, is indicative of his popularity. The article analyzes Jabłoński’s novels primarily using intertextual and cultural studies tools, but also draws upon religious studies, anthropology, and comparative religious studies
A critical review of a psychology textbook for secondary schools
The author presents a critical review on the psychology textbook for secon¬dary schools: a short survey of the contents, the positive and negative aspects of the book, and critical remarks. In the second part, the author threats the subject of three levels of psychical life: the psycho-physiological, the psycho-social, and the intellectual-spiritual levels. He is of the opinion that the intellectual-spiritual dimension of the human being has been the most neglected, especially since it is the most important one through which the individual gains dignity and man is defined as an autotranscendental being
FAIR INEQUALITY: HOW CONTEMPORARY RUSSIANS UNDERSTAND AN INCREASING INCOME GAP BETWEEN DIFFERENT PEOPLE?
The article deals with a question of interpretations of income inequalities between diffe-rent people in contemporary Russia. Analyzing different kinds of statistical data and secon-dary data of previous sociological researches, the author concludes, that contemporary Russians are more tolerant to the fact of income inequality, than it used to be in 1990s. Author also makes a hypothesis, that my explain this change: nowadays there are fewer people, who believe in their hopeless poverty, in Russia. Using the results of his own survey, author exami-nes this hypothesis and concludes, that contemporary Russians really became more loyal to the fact of inequality, than they used to be in 1990s. The main conclusion of the article is that modern Russians often raise the question of fairness of criteria for income inequality between different people, but they do not think, that the fact of inequality is wrong or bad for their society.</span
