Posterior corneal curvature changes following Refractive Small Incision Lenticule Extraction
Sri Ganesh, Utsav Patel, Sheetal Brar Phaco and Refractive Surgery Department, Nethradhama Superspeciality Eye Hospital, Bengaluru, Karnataka, India Purpose: To compare the posterior corneal curvature changes, in terms of corneal power and asphercity, following Refractive Small Incision Lenticule Extraction (ReLEx SMILE) procedure for low, moderate, and high myopia.Methods: This retrospective, non randomized, comparative, interventional trial; included 52 eyes of 26 patients, divided in three groups: low myopia (myopia ≤3 D [diopters] spherical equivalent [SE]), moderate myopia (myopia >3 D and <6 D SE), and high myopia (myopia ≥6 D SE). All patients were treated for myopia and myopic astigmatism using ReLEx SMILE. The eyes were examined pre-operatively and 3 months post-operatively using SCHWIND SIRIUS, a three-dimensional rotating Scheimpflug camera with a Placido disc topographer to assess corneal changes with regard to keratometric power and asphericity of the cornea.Results: A statistically significant increase in mean keratometric power in the 3, 5, and 7 mm zones of the posterior corneal surface compared with its pre-ReLEx SMILE value was detected after 3 months in the moderate myopia group (pre-operative [pre-op] -6.14±0.23, post-operative [post-op] -6.29±0.22, P<0.001) and high myopia group (pre-op -6.19±0.16, post-op -6.4±0.18, P<0.001), but there was no significant change in keratometric power of the posterior surface in the low myopia group (pre-op -5.87±0.17, post-op -6.06±0.29, P=0.143). Asphericity (Q-value) of the posterior surface changed significantly (P<0.001) after ReLEx SMILE in the moderate myopia group in the 3, 5, and 7 mm zones, and in the high myopia group in the 3 and 7 mm zones; but there was no significant change in the Q-value in the low myopia group in all three zones (pre-op 0.23±0.43, post-op -0.40±0.71, P=0.170), and in the high myopia group in the 5 mm zone (P=0.228).Conclusion: ReLEx SMILE causes significant changes in posterior corneal keratometric power and asphericity in moderate and high myopia, but the effect is subtle and insignificant in low myopia. "More Societal than Generational": Examining the Construction and Resistance of Generational Messages in the Workplace
Author email: [email protected] Millennial generation, those born between 1980-2000, have drawn vast, sometimes fanatical, criticism in popular media. Slated as narcissistic praise hounds, they are cast as demanding graduate divas who are about to attack the workplace and everything ‘you hold sacred’ (Clark, 2008; Safer, 2007). The abundance of such messages about this generation in formats ‘tailored, targeted, and consumed’ by the public is problematic given that generational constructs are by many perceived as sacrosanct (Myers et al, 2010).
The proliferation of such criticism is by no means innocuous given the very likely impact that they will have on Millennial work opportunities. For many scholars the field of Millennial research suffers from a lack of empirical and cross sectional data to establish more calculated and careful generational constructs, – instead relying on or reacting to popular negative stereotypes. While some Millennial scholarship has begun to move beyond criticisms of popular media, Millennial research is by many considered contradictory at best and confusing at worst (Kowske et al, 2010). Additional difficulties arise when the scramble to publish more research-based work has led to methodologies which are inherently flawed because they reinforce the very same monolithic generational categories they are supposed to assess.
This study, undertaken in New Zealand, explores critical approaches as a means of examining the construction of generational messages and the establishment of generational difference. As a starting point, this small-scale examination analyses the very way in which generational messages are constructed and resisted within the workplace through an analysis of interviews undertaken with 26 employees of a Small to Medium Enterprise (SME) in the information technology sector.
Unlike many generational studies, this project did not seek to draw conclusions by framing differences and measuring responses across generational lines, but rather took a bottom-up approach to understand how participants themselves constructed and resisted messages about generational difference. The project asked two research questions: First, how are generational messages constructed in the context of the workplace? And second, how are generational messages resisted in the workplace? Through axial coding this research categorized five themes under which participants constructed generational difference. These five themes are Technology, Voice, Fairness, Informality, and Stimulus. Broadly speaking, these themes were underpinned by a belief that Millennials have a great demand for respect, democratic process, and the reduction of power distances.
Given the critical approach, the study also observed resistance as a component of the discursive process. As such this research outlines the partiality of resistance and outlines strategies of resistance employed by employees. In line with the idea that construction and resistance are mutually implicated as negotiation, participants were frequently observed simultaneously constructing and resisting generational difference, both synchronically and diachronically. Through axial coding this study also categorized three strategies of resistance. These three strategies are established as Dismissal, the Third Person Effect, and the Decline Metaphor.
This research highlights the usefulness of adopting critical approaches by illustrating the way in which generational meaning is perpetually produced, reproduced, negotiated, and resisted by participants (Murphy, 1998). While there are several factors which are indicative of the Millennial generation, this thesis establishes the hegemonic character of most constructions of generational difference. Given the fragmented and complex state of society, this thesis posits that the usefulness of the monolithic birth-cohort generation has long since passed and we should instead look to understanding generations in terms of their consumption of similar cultural capital

Boiga beddomei is phylogenetically allied to a clade (node support 100%) consisting of B. nuchalis, B. ceylonensis of Sri Lanka and B. whitakeri sp. nov.; exhibiting 7.6–8.5, 8.9–9.3, 13.3–13.5% divergence at cyt b with its sister species B. nuchalis and B. whitakeri sp. nov. and B. ceylonensis respectively; and a maximum intraspecific divergence of 2.5%. Comments. The lectotypification by Ganesh et al. (2020) and the different phylogenetic placements revealed in the present study (see Fig. 1) implies that B. beddomei is a species endemic to the Northern Western Ghats of India. Thus, the Sri Lankan population previously attributed to ‘ B. beddomei ’ (sic) represented by the Sri Lankan sequence (fide Pyron et al. 2013) becomes obsolete as it is herein referred to as Boiga cf. ranawanei (also see Samarawickrama et al. 2005). The phylogenetic position of this Western Ghats population further corroborates the appropriate lectotype designation by Ganesh et al. (2020) that restricts B. beddomei as a species placed within the B. ceylonensis clade, clustering with B. ceylonensis and B. nuchalis —the two species Wall (1909) had originally associated B. beddomei with. Whereas the published sequence (i.e. B. cf. ranawanei) clusters with species not present in this clade (B. flaviviridis and B. thackerayi). Wallach et al. (2014) stated that the type locality is “restricted to Sri Lanka fide Taylor (1950 b:576)”, citing “Taylor, E.H. (1950b). A brief review of Ceylonese snakes. University of Kansas Science Bulletin 33(14), 519–603, 3 figs., pls. 12–25.” Our perusal of Taylor’s original publication reveals its heading to be “Snakes of Ceylon ”, the journal, issue and page numbers being the same as those mentioned in Wallach et al. (2014). Taylor only provided notes on ‘ B. beddomei ’ based on his new collections (EHT-HMS 30674–75 from 12 miles north of Trincomalee), but did not discuss its taxonomic history. The only place Taylor mentions the type locality is when he gives in chresonymy list “type locality: Ceylon ” citing Wall (1909). This again is not correct, as Wall (1909) also had the specimen from Matheran (the lectotype after Ganesh et al. 2020). Wall (1909) was mistaken that R.H. Beddome collected the types whereas they were in fact procured by Major Barrett (see Boulenger, 1896). Wall’s subsequent treatments of B. beddomei also included data from both Indian and Sri Lankan populations (Wall 1919, 1921). Wallach et al. (2014) themselves list the type localities correctly, including Matheran of India. Thus, the supposed terra typica restricta attributed to Taylor (1950) by Wallach et al. Temporal evolution in quality of life following melphalan percutaneous hepatic perfusion for patients with metastatic uveal melanoma
Background: recent studies have demonstrated a clear survival benefit of melphalan percutaneous hepatic perfusion (M-PHP) for liver metastases from ocular melanoma (mUM). Importantly, the survival benefit relating to each treatment cycle must be carefully balanced with changes in patient Quality of Life. We contribute our single centre findings on the temporal change in the quality of life after M-PHP therapy. Methods: we performed a retrospective analysis of the change in quality of life (QoL) using the Functional Assessment of Cancer Therapy - General (FACT-G) with mUM patients receiving M‐PHP at our institutions (n = 13). The FACT-G score which comprises physical (PWB), social (SWB), emotional (EWB) and functional (FWB) wellbeing were measured pre-procedure and at day 1, day of discharge, 7, 14 and 28 days after M-PHP therapy. Paired t-test was used to assess for any significant change in each QoL domain. Results: the baseline FACT-G score was 93.4 out of 108 +/- 14.7 (mean +/- std). Quality of life scoring significantly decreased immediately after the procedure (day 1; 84.8 +/- 13.1, p = 0.01) and gradually improved over time, with no significant difference in total score by the day of discharge; 88.7 +/- 13.7, c.f baseline p = 0.22). By day 28, there was a trend for overall improved QoL (96.6 +/-9.3, p = 0.25). Sub-score analysis revealed that by day 28 there was a statistically significant improvement in EWB (p = 0.02), a trend towards improved scores in PWB and SWB domains. FWB was slightly below baseline by day 28 (23.8 vs 23.2). Conclusions: quality of life following M-PHP decreases immediately after therapy and is not significantly different from baseline by the day of discharge. By day 28 there is a trend towards improved overall quality of life. This study could provide the basis for determining the optimal time between treatment cycles
Quality of life after melphalan percutaneous hepatic perfusion for patients with metastatic uveal melanoma
Background: recent studies indicate that melphalan percutaneous hepatic perfusion (M-PHP) for liver metastases from ocular melanoma (mUM) improves survival. Importantly, this benefit must be carefully balanced with changes in a patient's quality of life (QoL). This study examines the QoL changes post-M-PHP.Methods: retrospective analysis of the change in QoL using the Functional Assessment of Cancer Therapy-General (FACT-G) with mUM patients receiving M-PHP ( n = 20). The FACT-G scores, which comprise physical (PWB), social (SWB), emotional (EWB) and functional (FWB) wellbeing were measured pre-procedure and at day 1, day of discharge (mean = 2.4 days), 7, 14 and 28 days after M-PHP therapy. Wilcoxon signed-rank test gauged QoL domain changes.Results: baseline FACT-G median (IQR) scores were 101.8 (21.8). QoL scoring significantly decreased immediately after the procedure [day 1; 85 (27.5); P = 0.002] and gradually improved over time. By day 28, QoL almost returned to pre-procedure levels [100.3 (13.8); P = 0.31]. Subscore analysis revealed that the initial drop in QoL at day 1 post-procedure was attributable to the PWB (28 vs. 24; P = 0.001) and FWB domains (26 vs. 18.5; P < 0.001). By day 28 there was a statistically significant improvement in EWB ( P = 0.01).Conclusion: QoL following M-PHP decreases immediately after therapy and is not significantly different from baseline by the day of discharge. By day 28 there is improved emotional well-being. This study could help to optimize the time between treatment cycles when combined with toxicity data and blood count recovery.</p
