Is percutaneous cholecystostomy a good alternative treatment for acute cholecystitis in high-risk patients?
Cholecystectomy is the treatment of choice for acute cholecystitis but the management of high-risk surgical patients is a difficult dilemma. Percutaneous cholecystostomy (PC) could represent a safer and less invasive option. The aim of the study was to assess the outcomes of PC in high-risk patients. This is a retrospective single-center study; data were collected from our hospital electronic record system. From February 2009 to March 2014, there were 753 patients admitted with acute cholecystitis. Of these 39 were considered high risk for surgery and underwent PC during their hospital stay. The radiological approach was transperitoneal in 29 patients and transhepatic in 10 patients. Median follow-up was 19 months. There were 27 males (69.2%) and 12 females (30.8%) with a mean age of 72 years (range 41-90 years). Twenty-seven patients had PC as definitive treatment (group A) and 12 patients as a bridge to cholecystectomy (group B). There were no postprocedure complications. Five patients in group A were readmitted once with another episode of cholecystitis after PC (18.5%), one patient in group B was readmitted with cholecystitis after two years before proceeding to cholecystectomy, and two patients were readmitted after cholecystectomy (16.6%) for intra-abdominal collections treated with percutaneous radiological drainage. Seven patients died (17.9%) as a result of severe biliary sepsis during their index hospital admission. PC is a safe approach in high-risk patients with acute cholecystitis and can provide satisfactory long-term results when cholecystectomy is not a viable option.</p

Postoperative day one serum alanine aminotransferase does not predict patient morbidity and mortality after elective liver resection in non-cirrhotic patients
Serum aminotransferases have been used as surrogate markers for liver ischemia-reperfusion injury that follows liver surgery. Some studies have suggested that rises in serum alanine aminotransferase (ALT) correlate with patient outcome after liver resection. We assessed whether postoperative day 1 (POD 1) ALT could be used to predict patient morbidity and mortality following liver resection. We reviewed our prospectively held database and included consecutive adult patients undergoing elective liver resection in our institution between January 2013 and December 2014. Primary outcome assessed was correlation of POD 1 ALT with patient's morbidity and mortality. We also assessed whether concurrent radiofrequency ablation, neoadjuvant chemotherapy and use of the Pringle maneuver significantly affected the level of POD 1 ALT. A total of 110 liver resections were included in the study. The overall in-hospital patient morbidity and mortality were 31.8% and 0.9%, respectively. The median level of POD 1 ALT was 275 IU/L. No correlation was found between POD 1 serum ALT levels and patient morbidity after elective liver resection, whilst correlation with mortality was not possible because of the low number of mortalities. Patients undergoing concurrent radiofrequency ablation were noted to have an increased level of POD 1 serum ALT but not those given neoadjuvant chemotherapy and those in whom the Pringle maneuver was used. Our study demonstrates POD 1 serum ALT does not correlate with patient morbidity after elective liver resection.</p

An analysis of the impact of electric mobility on the electric power system in 2030
LAUREA MAGISTRALEIl riscaldamento globale, il cambiamento climatico, lo sfruttamento delle risorse naturali, nonché l’aumento delle emissioni di gas inquinanti impongono la definizione di obiettivi ambiziosi in termini di sostenibilità energetica. In questo quadro generale in evoluzione, sono state prese in considerazione diverse misure per salvaguardare il pianeta e non compromettere la situazione futura. Tra questi, l'introduzione della mobilità elettrica ha un ruolo fondamentale nel raggiungimento degli obiettivi ambientali prefissati.
Lo scopo della tesi è quello di analizzare l’impatto dei veicoli elettrici sul sistema elettrico nel 2030. Il caso di studio è la regione Lombardia e il primo obiettivo è analizzare gli spostamenti attraverso l’elaborazione dei dati contenuti nella matrice OD. Lo scopo di questa sezione è quello di ottenere il profilo di distribuzione dei viaggi regionali e le relative frequenze di spostamento. Tutti i percorsi sono elaborati con una piattaforma specifica da cui vengono estratte la velocità istantanea, la lunghezza e la durata del tragitto. Implementando un algoritmo che calcola il consumo in base alla velocità di viaggio, è possibile stimare il fabbisogno energetico giornaliero dovuto ai veicoli elettrici. Sulla base delle previsioni presenti nei documenti ufficiali, il processo viene ripetuto con due diversi livelli di penetrazione di veicoli elettrici al fine di valutare due possibili scenari al 2030. A questo punto, si considerano quattro tipi di modalità di ricarica: ricarica domestica, ricarica sul luogo di lavoro e due tipi di ricarica pubblica. Per ognuna di esse sono stati definiti i principali parametri in termini di potenza di ricarica e frequenza con cui avviene il processo. Sulla base dei consumi energetici di ciascuna tratta stimati nel passaggio precedente, è stato estratto il profilo di prelievo giornaliero in potenza della regione. I risultati ottenuti evidenziano che i veicoli elettrici non sono caratterizzati da un alto fabbisogno energetico. Tuttavia, i processi di ricarica sono concentrati nelle stesse finestre temporali creando picchi significativi nell’andamento della potenza prelevata dalla rete. Una volta verificata l’accuratezza della metodologia, la stessa analisi viene applicata al caso della provincia di Milano. Infine, vengono proposte alcune azioni migliorative e strategie di controllo con l'obiettivo di limitare le problematiche di rete causate dai processi di ricarica non regolamentati.The global warming, the climate change, the exploitation of the natural resources as well as the increase of emissions of polluting gases lead to a definition of ambitious objectives in terms of energy sustainability and carbon neutral world. In this evolving general picture, several measures have been taken into account in order to protect the planet and don’t compromise the future situation. Between them, the introduction of the electric mobility has a fundamental role in the achieving of the established environmental objectives.
The aim of the thesis is to analyse the impact of the electric vehicle on the electric power system in 2030. The case of study is the Lombardy region and the first goal is to investigate the movements inside it by exploring the OD matrix. The aim of this section is to obtain the regional travel profile and the journey frequency. All the routes are processed with a specific tool and the main properties that characterize the trips are extracted such as instantaneous speed, length and duration of the path. By constructing an algorithm that computes the consumption based on the travelled speed, the daily energy requirement due to electric vehicles can be estimated. According to forecasts present in official documents, the process is repeated with two different levels of EV penetration in order to evaluate two possible scenarios in 2030. At this point, four type of charging modes are assumed: domestic charge, work charge and two types of public charge. For each one, it has been defined the main properties in terms of charging power and time when the process takes place. Based on the energy consumptions of each route estimated in the previous step, the daily power withdrawal profile of the region is extracted. The outcomes obtained underline the fact that the electric vehicles are not characterized by a high energy requirement. However, the charging processes are concentrated in the same time windows creating significant peaks in the power demand trend. After the methodology is validated, the same analysis is applied to the case of the province of Milano. Finally, some improvement actions and control strategies are proposed with the aim of limiting the grid problems caused by the uncontrolled charging processes
Design and characterization of a fractal-inspired multi-frequency piezoelectric energy converter
A promising harvesting technique, in terms of simplicity and efficiency, is the conversion of ambient kinetic energy through piezoelectric materials. This work aims to design and investigate a piezoelectric converter conform to a fractal-inspired, multi-frequency structure previously presented by the author. A physical prototype of the converter is built and experimentally examined, up to 120 Hz, in terms of modal response and power output. Three eigenfrequencies are registered and the power output is particularly good at the fundamental eigenfrequency. Also the effect of the resistive load applied to the converter is investigated

AbstractIn the recent years, the industry of model building has been the subject of the intense activity, especially after the measurement of a relatively large values of the reactor angle. Special attention has been devoted to the use of non-abelian discrete symmetries, thanks to their ability of reproducing some of the relevant features of the neutrino mixing matrix.
