The role of structured training in learning microsurgery: a narrative review
Background and introduction. Microsurgery has transformed reconstructive surgery, offering superior aesthetic and functional outcomes, particularly in complex cases. Mastery of microsurgery is challenging due to the intricate nature of procedures and the precision required, leading to a steep learning curve. Various training methods, including traditional animal models and emerging synthetic alternatives, have been developed to enhance skill acquisition. Structured training programs, supported by national and international societies, are crucial for improving competency, with emphasis on both objective and subjective measures of performance.
Material and methods. This review utilized the PubMed database to analyze literature on microsurgery training, employing search terms like “Microsurgery” AND “training resident”. After systematic filtering, 15 relevant articles were included, encompassing studies evaluating improvements in anastomosis time and patency rates, along with resident confidence.
Results. Key findings included a 2022 study revealing a 41% patency rate among 1.792 anastomoses performed by residents after a 7-month training course with Wistar rats. Training duration significantly influenced performance. A virtual training program in low-resource settings demonstrated improved confidence in microsurgical skills, although statistical significance was limited by sample size. An Italian study indicated that structured long-term training led to significant improvements in Global Rating Scale (GRS) scores and reduced procedure times.
Discussion and conclusions. The findings emphasize the effectiveness of structured, hands-on training in enhancing microsurgical skills. While remote training offers valuable introductory knowledge, in-person practice remains essential for developing the precision required in microsurgery. An integrated training approach combining both methods could improve global access to microsurgical education, equipping practitioners with necessary skills for success in the field. Levator alae nasi muscle V-Y island flap for nasal tip reconstruction
Nasal tip reconstruction can be very challenging. It requires close attention to skin texture, colour and thickness matching, with the respect of the nasal aesthetic units and symmetry. Flaps are usually preferred to skin grafts where possible. Based on different donor areas, various flaps have been described for reconstruction of this region. Here we present a new V-Y myocutaneous island flap based on the levator alae nasi muscle (LAN muscle) blood supply. This flap may represent an alternative to the nasalis myocutaneous sliding V-Y flap previously described by Rybka. As its pivot point it is located more cranially than the nasalis flap, and it can advance more medially than the Rybka flap, with the possibility of covering larger defects of the nasal tip area, up to 1.8 cm in diameter. Over the past 5 years, 24 patients received nasal tip reconstruction with this flap following the resection of basal cell carcinomas. Good tip projection was maintained, and the aesthetic outcome was satisfactory, with well healed scars. In the times of the restrictive socio political and cultural Franco context, our author’s presence went progressively growing as the prohibitions were loosed, becoming a plain and visible presence during the Transition times if we take into account her reception in the Spanish edition world and in the Press. Considering, on one hand, first the high number of her works (re) editions as well as, afterward, the new Second Sex translation into Spanish (1999, 2005) and, on the other hand, the good and very good valuation (obviously unequal according to the ideology) of the Press, our author is now a classic in our country. It is true that in some Humanities fields her consecration is irrefutable since she is of compulsory mention, nevertheless in the Literary Studies she hasn’t benefited of the same chance. Still enduring the accusations of misoginy and those of defending masculine values –some long time accusations since they go back to the publication of the Second Sex in France, and after taken on the behalf of the postmodern French feminism–, new times in the reception of Simone de Beauvoir can be predicted since Julia Kristeva claims for the (Re) discovering of Simone de Beauvoir...En los tiempos del contexto socio político y cultural tan restrictivo, la presencia de Simone de Beauvoir fue gradualmente incrementándose a medida que el régimen totalitario relajó las prohibiciones, para llegar a ser total a partir de la Transición a tenor de su acogida en el mundo editorial y periodístico. Pues, por un lado, primero por el ingente número de ediciones y reediciones de sus obras, así como, posteriormente, por la nueva traducción al castellano del Segundo Sexo en Cátedra (1999, 2005) y, por otro lado, por su valoración –eso sí, desigual en función de la ideología– de la prensa, nuestra autora es ya un clásico en nuestro Estado. Si bien es cierto que en determinados ámbitos del saber su consagración es irrefutable, en los Estudios Literarios, sin embargo, no ha tenido la misma suerte por pesarle todavía una desconsideración motivada por las acusaciones de misoginia y de ensalzamiento de los valores masculinos, desconsideraciones surgidas ya con la publicación del Segundo Sexo y retomadas posteriormente por cuenta del feminismo postmoderno francés. Ahora bien se auguran nuevos aires en la recepción de nuestra autora en este ámbito de estudio ya que la misma Julia Kristeva apunta a un (Re)descubrimiento de Simone de Beauvoir..

A comparative study of form and theology in the works of Flannery O'Connor and Simone Weil
In this comparative study of the form and theology of Flannery O'Connor and Simone Weil I interrogate how Weil's philosophical writings and her theology illuminate O'Connor's use of both narrative and non-fictional forms, and her Catholicism. The Introduction analyses how Weil's concept of superposed reading provides a new method of approaching both O'Connor, her writings, and O'Connor
studies, and focuses on how such apparently different women interconnect. Chapter One explores how both Weil and O'Connor attempt to write their theologies on the
souls of their readers yet are each subject to constraints imposed by form. Weil's concept of locating equilibrium between incommensurates is discussed, and her
distinctively philosophical approach to fictions and fictionality is used to investigate O'Connor's notion of prophetic fictions and the writer's role. Chapter Two assesses how both writers revivify Christian paradoxes. Weil's monstrous concept of affiiction, and O'Connor's use of the grotesque genre to jolt secular man into an
awareness of the sacred are scrutinised. Chapter Three studies how both writers consider an encounter between God and man is possible through the action of grace. My Conclusion interrogates how Weil's work can deepen our understanding of O'Connor's writings, and examines how successful O'Connor is at realising a truly
Christian literature. I conclude that despite being a writer of powerful fictions, O'Connor can not be totally successful in her mission as writer-prophet because
