Identification of potentially druggable molecular alterations in skin adnexal malignancies
Skin adnexal cancers (SAC) are a heterogeneous group of rare malignancies with histological differentiation towards epithelial adnexa, which lack effective systemic treatments. The aim of this work is to identify any potentially druggable genomic alterations for possible targeted therapies. Cases of primary or recurrent/metastatic (RM) SAC between 2002 and 2014 were identified by searching the institutional cancer registration database. Histological sections of all referral cases were reviewed by a dedicated pathologist to confirm diagnosis. Immunohistochemistry was performed to assess the expression of androgen receptors (AR) and human epidermal growth factor receptor type 2 (HER2). Targeted next-generation sequencing (T-NGS) was performed to identify targetable mutations (panel of 50 genes analyzed by Cancer Hotspot Panel, Ion-Torrent Personal Genome Machine). Mutational analysis of the PTCH1 gene not present in the T-NGS panel was assessed by Sanger sequencing. A total of 45 cases with available histological samples were identified (35 primary, 10 RM). The most frequent histological type was porocarcinoma (n = 12). Globally, 14 cases (31%) were AR+ (6/10 RM, 60%; 8/35 primary, 23%). HER2 was shown as 2+ in eight of 42 (19%) cases (2/9 RM, 22%; 6/33 primary, 18%). DNA was adequate for T-NGS analysis in 25 cases. In the majority of cases (17 cases, 68%) at least one mutation in oncogenes or tumor suppressor genes was found: the most frequent ones involved TP.53 (13 cases, 76% of mutated SAC) and PIK3CA (three cases, 18%). The rate of PTCH1 mutation was 30%. These findings support the use of molecular screening in patients with advanced SAC

Sixty-seven patients with Crohn's disease underwent surgery in our department in the period 1989-1996. In this paper we evaluated only the first 20 patients. Ten were female and 10 male. There was single localization in 13 patients and multiple in 7. Surgery was limited to the macroscopically involved intestinal segment. Reconstruction of the intestinal tract has been carried out with a double layer polyglycolic acid end-to-end manual anastomosis
The new right in Argentina, anti-politics and the role of social networks
Este artículo analiza la relación entre la extrema derecha, la anti-política y la influencia de las redes sociales en la nueva derecha. Se examina cómo los discursos de odio y la discriminación se han vuelto comunes en los discursos políticos contemporáneos, y cómo las nuevas tecnologías de comunicación han sido utilizadas para propagar estos mensajes en la sociedad. El artículo también considera la influencia histórica de las elites locales en la apropiación de las riquezas autóctonas y en la alineación geopolítica y cultural con los grandes centros de poder globales. A través del análisis de estos temas, se concluye que la democracia como sistema político está en riesgo debido a la lógica del sistema globalizado, que prioriza la concentración de riqueza y poder en manos de unos pocos en detrimento de la mayoría de la población.This article analyzes the relationship between the far-right, anti-politics, and the influence of social media on the new right. It examines how hate speech and discrimination have become common in contemporary political discourse, and how new communication technologies have been used to propagate these messages in society. The article also considers the historical influence of local elites in the appropriation of autogenous wealth and in the geopolitical and cultural alignment with the major centers of global power. Through the analysis of these themes, it is concluded that democracy as a political system is at risk due to the logic of the globalized system, which prioritizes the concentration of wealth and power in the hands of a few at the expense of the majority of the population.Fil: Fiore Viani, Jorge Gonzalo.
