Quantifying the Relationship Between Skin Temperature and Skin Hydration Measured by Corneometry
No full textpeer reviewedThe role of the skin in forensic medicine is increasingly recognised, particularly in post mortem interval (PMI) determination through its electrical properties. This study investigates the relationship between stratum corneum hydration (SCH), measured by corneometry, and skin temperature (ST), as literature strongly suggests that variations in skin temperature can influence its electrical properties. Twenty-four (24) adult subjects with known cause of death and characteristics were selected and briefly removed from their mortuary cell. ST (°C) and SCH (a.u.) were measured immediately and then at 5-minute intervals for 20 minutes using the Skin-Thermometer ST 500® and Corneometer CM-825® (Courage & Khazaka electronic GmbH, Cologne, Germany) at 3 body sites (cheekbone, abdomen and index finger). The results show a significant inverse relationship between ST and SCH, with a unit decrease in SCH for each unit increase in ST (-1.21 ± 0.55, p=0.028). This relationship was site-dependent (p <0.0001) but not influenced by time since removal from the mortuary cell (RMCT) (p = 0.15), sex (p = 0.39) or PMI (p = 0.88). The results highlight the need for careful consideration of skin temperature when assessing skin hydration, in order to accurately interpret post mortem changes in skin electrical properties, particularly when determining the post mortem interval (PMI)
Contribution aux méthodes d'évaluation de l'incontinence urinaire et applications en médecine d'assurance.
No full textIntroduction. L'incontinence urinaire ne peut être ignorée par l'expert lors de son évaluation. On ne peut toutefois pas attendre de la victime un énoncé de ce préjudice de manière ouverte et presque publique dans une sphère aussi chargée d'émotion. La pudeur élude ce handicap du discours revendicateur et le médecin devra rechercher les signes évocateurs de cette affection cachée par un sentiment de honte car il se doit d'évaluer le dommage et tout le dommage. Tant dans le cadre de l'expertise d'un dommage que de l'évaluation des atteintes de la santé dans un contexte de médecine sociale, le médecin se doit de recourir à des méthodes d'évaluation adaptées (et non invasives), fiables, reproductibles et scientifiquement validées. Les méthodes d'évaluation et leurs limites d'application dans un contexte non thérapeutique doivent être analysées de même que la procédure actuelle de l'évaluation personnelle dans le cadre de l'incontinence urinaire. Nous nous intéressons tout d'abord aux notions épidémiologiques en rapport avec l'incontinence urinaire en définissant le problème et en déterminant la terminologie adaptée au français. Après une analyse de la prévalence, nous présentons les facteurs de risque associés. Après avoir déterminé les facteurs prédictifs et précisé le principe d'anticipation, nous rapportons l'impact économique et social d'un tel symptôme. Le contexte non thérapeutique de l'expertise implique des limites dans la méthodologie d'évaluation. Nous reprenons les techniques non invasives d'évaluation en analysant la littérature avec les procédures utilisées dans ce travail. Nous évaluons les techniques peu invasives et analysons l'intérêt dans le cadre évaluatif des informations fournies par les techniques modérément invasives et invasives prescrites dans un cadre thérapeutique. La procédure actuelle d'évaluation de l'incapacité personnelle dans le cadre de l'incontinence urinaire est présentée avec une définition du terme "incapacité personnelle" et en précisant les liens de causalité. Sont indissociables du principe de causalité, les notions d'état antérieur et de prédisposition. Les techniques d'évaluation et de détermination du taux en Belgique et à l'étranger sont revues et l'aspect évaluatif spécifique à la médecine pratiquée dans le cadre de la sécurité sociale est étudié. L'objectif de la recherche comportait un volet permettant de calibrer le taux d'atteinte à l'intégrité physico-psychique d'une personne présentant une incontinence urinaire, un volet relatif à une application de cette méthode de calibrage dans le cadre des prestations d'évaluation médicale pour la sécurité sociale et un volet déterminant la fidélité et la sincérité des réponses des patients soumis à l'autoquestionnaire utilisé en tant qu'instrument évaluatif.Première étude.Objectif.-La détermination du préjudice corporel relatif à l'incontinence urinaire oblige au recours à des techniques d'évaluation non invasives et des plus précises de cet état en utilisant des instruments d'exploration en concordance avec la Classification Interntionale du Fonctionnement, du handicap et de la santé(CIF). Le but de cette étude est d'identifier le test évaluatif le plus adéquat et de déterminer un modèle barémique en rapport avec l'incontinence urinaire.Matériels et méthodes.-Nous avons réalisé une étude transversale prospective comparant les résultats des tests urodynamiques successifs avec les scores du questionnaire International Consultation on Incontinence Questionnaire-Urinary Incontinence-Short Form (ICIQ-UI-SF) et le test d'incontinence (pad-test) réalisé en une heure sur 120 patients. Nous avons procédé par analyse statistique de probabilité des événements puis utilisé la régression logistique afin de calculer la probabilité d'incontinence urinaire en fonction des facteurs prédicteurs les plus significatifs. Par la suite, nous avons créé une grille basée sur les prédicteurs significatifs et leur probabilité d'une incontinence urinaire objectivée sur base d'un examen urodynamique.Résultats.-Le score moyen ICIQ-UI-SF était de 13,5±4,6 et la valeur médiane du pad test était de 8 g. L'évaluation basée sur la construction ROC (receiver operating characteristic) démontre la pertinence des résultats urodynamiques avec les scores ICIQ-UI-SF (Aire Sous Courbe 0,689) et les résultats du test d'incontinence-pad test (Aire Sous Courbe 0,693). En procédant par régression logistique, nous avons démontré que les meilleurs prédicteurs indépendants d'une incontinence urinaire objectivée par un test urodynamique étaient l'âge du patient et le score ICIQ-UI-SF. Le modèle obtenu par régression logistique nous a permis de construire une équation permettant de déterminer la probabilité d'incontinence urinaire ojectivée sur base d'un examen urodynamique à l'aide de ces prédicteurs. Utilisant ces instruments, nous avons créé une grille générant un index de probabilité.Conclusion.-En utilisant cette grille d'index de probabilité, relative au patient et à son taux maximum d'invalidité en rapport avec l'incontinence urinaire, nous devenons capable de calculer son incapacité personnelle.Seconde étude.Objectif.-Des forfaits financiers sont octroyés par la sécurité sociale sur base de critères de dépendance déterminés par une échelle d'évaluation hiérarchisée, scorée en quatre points, appelée échelle de Katz dont le cinquième item concerne la continence urinaire et fécale. L'objectif de cette étude est de démontrer que le questionnaire ICIQ-UI-SF peut être utilisé afin de déterminer un score applicable en médecine factuelle pour évaluer le degré d'incontinence demandé par l'échelle de Katz-6 et permettre la distinction entre le score 2 (est accidentellement incontinent) et le score 3 (est incontinent).Matériels et méthodes.-La capacité fonctionnelle de 63 patients (14 hommes, 49 femmes, âgés de 70 à 99 ans) a été évaluée sur base d'un Mini Mental State Examination (MMSE), d'un index de Katz pour les activités de vie quotidienne et d'un questionnaire ICIQ-UI-SF. Une analyse comparative des moyennes a été réalisée. Les scores observés lors du questionnaire ICIQ-UI-SF ont été introduites comme variables. Les données observées dans la population scorée 2 à l'item continence ont été comparées aux données de la population scorée 3 pour le même item en réalisant un graphe type box-and-whisker and dot plot. La sensibilité et la spécificité de chaque variable ont été testées et les résultats évalués en réalisant une courbe ROC (Receiver Operating Characteristic). La meilleure variable (score ICIQ-UI-SF) a été retenue comme score pivot permettant la distinction des patients pouvant être scorés 2 ou 3 sur l'item continence de l'échelle de Katz.Résultats.-Des différences significatives ont été observées entre l'échantillon scoré 2 pour l'item continence de l'échelle de Katz et l'échantillon scoré 3 pour le même item et le score du questionnaire ICIQ-UI-SF et le score global de l'échelle de Katz mais nous n'avons pas observé de différence significative en rapport avec le score MMSE. Sur base de l'analyse ROC, nous avons comparé le pouvoir discriminant du score ICIQ-UI-SF par rapport au score de l'item continence de l'échelle de Katz. Le score ICIQ-UI-SF le plus favorable en sensibilité et spécificité s'est révélé être le score de 13.Conclusion.-Nous avons démontré que la variable la plus significative (score ICIQ-UI-SF) à retenir comme score pivot pour distinguer les patients pouvant être scoré 2 ou 3 sur l'item continence de l'échelle de Katz est la valeur 13.Troisième étude.Objectif.- Nous avons recherché les éléments de personnalité dans une population ayant une incontinence urinaire. Une corrélation entre les résultats du questionnaire de personnalité Minimult et les résultats de l'ICIQ-UI-SF (International Consultation Incontinence Questionnaire Urinary Incontinence Short Form) a été réalisée. Le but du travail est d'objectiver la fiabilité et la sincérité des réponses des patients et, par ailleurs, de rechercher les éléments permettant d'établir qu'un trouble de personnalité pourrait être impliqué dans la physiopathologie de l'incontinence urinaire.Matériels et méthodes.- Nous avons réalisé une étude prospective observationnelle. Un questionnaire informatisé comprenant une double présentation des trois questions scorées de l'ICIQ-UI-SF parmi les 71 questions du Minimult a été proposé à 47 patients.Résultats.- Sur 37 patients inclus, le test non paramétrique Wilcoxon-Mann-Whitney a conforté la concordance des deux questionnaires ICIQ-UI-SF avec p=0.1792. Parmi ceux-ci, l'inventaire Minimult a permis de sélectionner 23 patients dont le score de l'échelle de validité F (avec F70 correspond à une personne "qui accentue ses symptômes" ou peut être significatif d'un trouble de l'attention ce qui justifie l'exclusion de ces patients. Nous avons comparé la différence des scores de l'ICIQ-UI-SF lors des test et re-test aux scores obtenus sur l'échelle de validité F, indicateur de surcharge, du questionnaire Minimult en utilisant l'analyse ROC. Les résultats ont démontré une AUC de 0,559 avec une sensibilité de 78,6% et une spécificité de 43,5%. Parmi cette population valide de 23 patients, aucun score L n'était supérieur à 70 ce qui indique l'absence de mensonge délibéré. L'analyse de personnalité de cette même population n'a détecté aucun trouble de personnalité dans 9 cas. Elle a démontré six cas présentant une valeur élevée sur l'échelle d'hypochondrie, cinq une valeur élevée dans l'échelle dépression, quatre une valeur élevée dans l'échelle d'hystérie. Cette triade névrotique a été retrouvée dans deux cas. La déviation psychopathique (Pd) relative à un registre antisocial était retrouvée dans cinq cas tandis que l'échelle paranoïa ainsi que l'échelle d'hypomanie était élevée dans un cas mais aucune association entre ces traits de personnalité n'était observée ce qui permettait de conclure en l'absence de problèmes de comportement. Le profil psychotique associant schizophrénie, paranoïa, dépression et hypomanie n'a été retrouvé dans aucun cas. Le tableau psychasthénie qui correspond à des traits de personnalité phobo-obsessionnels était retrouvé dans près de la moitié des cas valides.Conclusion.- Nous concluons qu'une corrélation entre les résultats d'un questionnaire de personnalité et les résultats de l'ICIQ-UI-SF est réalisable. Cette étude comparative objective la fiabilité des réponses des patients tandis que la sincérité de celles-ci est déterminée par l'absence de mensonge délibéré recherché par le Minimult. Nous confirmons et précisons les données de la littérature sur les différents tableaux cliniques de la personnalité des patients incontinents en soulignant l'importance du tableau psychasthénie qui correspond à une personne anxieuse victime d'un important ressenti de son problème. Ces éléments permettent de suspecter qu'un trouble de personnalité pourrait être impliqué dans la physiopathologie de l'incontinence urinaire.Conclusions générales. La fiabilité et la sincérité des réponses au questionnaire ICIQ-UI-SF étant démontrée par les études relatives à la validité de celui-ci et au travers d'une étude concluant entre une corrélation entre les résultats obtenus lors du questionnaire et un test de personnalité démontrant l'absence de mensonge délibéré, nous avons pu démontrer que la valeur pivot du questionnaire ICIQ-UI-SF permettant la distinction entre le score 2 et 3 de l'item continence de l'échelle de Katz est de 13. Cette observation relève d'une importance majeure dans le cadre de la médecine pratiquée dans le domaine de la sécurité sociale et en particulier lors du financement des soins aux personnes dépendantes. Sur base d'une étude prospective, nous avons déterminé les facteurs prédictifs les plus significatifs d'une incontinence urinaire et construit une formule établissant une probabilité d'incontinence. Une grille établissant cette probabilité en fonction de l'âge du patient et son score ICIQ-UI-SF a été réalisée laquelle a permis la détermination de taux d'incapacité personnelle selon des catégories de sévérité du handicap. Forensic Entomology Investigations From Doctor Marcel Leclercq (1924-2008): A Review ofCases From lg69 to 2005

He has provided his knowledge of insect biology to many forensic cases, and most of them have found the way to publication. Most of the papers he has written were focused on individual cases, and despite the abundance of entomoforensic investigations he conducted, no synthesis has been published. This paper summarizes 36 years of forensic entomological investigations in temperate Europe, mainly in Belgium. Leclercq’s work includes 132 entomological cases involving 141 human corpses found in various death scenes. Under certain conditions, insect specimens found on death scene can provide information on when (postmortem interval estimation), where and how a person died. More or less one hundred insect species associated to a dead body have been identified by Marcel Leclercq
Wall Model Based on a Mixture Density Network to Predict the Wall Shear Stress Distribution for Turbulent Separated Flows
No full textpeer reviewedMost wall shear stress models assume the boundary layer to be fully turbulent, at equilibrium, and attached. Under these strong assumptions, that are often not verified in industrial applications, these models predict an averaged behavior. To address the instantaneous and non-equilibrium phenomenon of separation, the mixture density network (MDN), the neural network implementation of a Gaussian Mixture Model, initially deployed for uncertainty prediction, is employed as a wall shear stress model in the context of wall-modeled large eddy simulations (wmLES) of turbulent separated flows. The MDN is trained to estimate the conditional probability p(τw|x), knowing certain entries x, to better predict the instantaneous wall shear stress τw (which is then sampled from the distribution). In this work, an MDN is trained on a turbulent channel flow at the friction Reynolds number Reτ of 1000 and on the two-dimensional periodic hill at the bulk Reynolds number of 10,595. The latter test case is known to feature a massive separation from the hill crest. By construction, the model outputs the probability distribution of the two wall-parallel components of the wall shear stress, conditioned by the model inputs: the instantaneous velocity field, the instantaneous and mean pressure gradients, and the wall curvature. Generalizability is ensured by carefully non-dimensionalizing databases with the kinematic viscosity and wall-model height. The relevance of the MDN model is evaluated a posteriori by performing wmLES using the in-house high-order discontinuous Galerkin (DG) flow solver, named Argo-DG, on a turbulent channel flow at Reτ=2000 and on the same periodic hill flow. The data-driven WSS model significantly improves the prediction of the wall shear stress on both the upper and lower walls of the periodic hill compared to quasi-analytical WSS models
Apixaban versus warfarin in patients with atrial fibrillation
No full textBACKGROUND: Vitamin K antagonists are highly effective in preventing stroke in patients with atrial fibrillation but have several limitations. Apixaban is a novel oral direct factor Xa inhibitor that has been shown to reduce the risk of stroke in a similar population in comparison with aspirin. METHODS: In this randomized, double-blind trial, we compared apixaban (at a dose of 5 mg twice daily) with warfarin (target international normalized ratio, 2.0 to 3.0) in 18,201 patients with atrial fibrillation and at least one additional risk factor for stroke. The primary outcome was ischemic or hemorrhagic stroke or systemic embolism. The trial was designed to test for noninferiority, with key secondary objectives of testing for superiority with respect to the primary outcome and to the rates of major bleeding and death from any cause. RESULTS: The median duration of follow-up was 1.8 years. The rate of the primary outcome was 1.27% per year in the apixaban group, as compared with 1.60% per year in the warfarin group (hazard ratio with apixaban, 0.79; 95% confidence interval [CI], 0.66 to 0.95; P<0.001 for noninferiority; P = 0.01 for superiority). The rate of major bleeding was 2.13% per year in the apixaban group, as compared with 3.09% per year in the warfarin group (hazard ratio, 0.69; 95% CI, 0.60 to 0.80; P<0.001), and the rates of death from any cause were 3.52% and 3.94%, respectively (hazard ratio, 0.89; 95% CI, 0.80 to 0.99; P = 0.047). The rate of hemorrhagic stroke was 0.24% per year in the apixaban group, as compared with 0.47% per year in the warfarin group (hazard ratio, 0.51; 95% CI, 0.35 to 0.75; P<0.001), and the rate of ischemic or uncertain type of stroke was 0.97% per year in the apixaban group and 1.05% per year in the warfarin group (hazard ratio, 0.92; 95% CI, 0.74 to 1.13; P = 0.42). CONCLUSIONS: In patients with atrial fibrillation, apixaban was superior to warfarin in preventing stroke or systemic embolism, caused less bleeding, and resulted in lower mortality. Nanok Expedition: an ice triathlon in Greenland partnering with scientists for climate research
No full textA 450 km crossing of the Greenland ice sheet on skis, over 400 km sea kayak descent along Greenland’s south and west coast and a 600 m vertical climb to open a new climbing route: this is the atypical triathlon led by the two Belgian explorers of Nanok Expedition, Gilles Denis and Nathan Goffart, between April and September 2022. Beyond their sportive objective, their goal was to serve climate research and engage with and communicate to a non-scientific audience about climate research. In partnership with five scientific institutions, they collected unique data and observations of the elevation changes of the ice sheet, as well as rare in situ samples of snow and fjords water to investigate the influence of ice sheet melting on dust input to the ecosystem. The data and samples collected will be analyzed by the universities and research centers. The expedition was documented with a movie “La Traversée” now shared to a general audience on national television and on different university campus. The two explorers are bridging different scientific disciplines (geophysics, geochemistry, glaciology) and bring the science at the table of a non-scientific audience by sharing their passion. Beyond the science, this is a source of inspiration to communicate about the importance of polar regions in the climate system

The San Jose lodge of Elks has
planned a novel feature for Its Independence day celebration. On the
morning ot the Fourth an automobile- parade, with the machines dee-
orated with national colors, is to be
held. "?
Reasons lor tho pngcant and -expectations of Its success are aet forth
in tho following extract from the
committee's letter of Invitation:
"Thla committee ls desirous of
surpassing all efforts heretofore
mady In this State respecting the
auto feature ln parades, and to ex-
eel particularly in tho number of
cars and elegance and originality of
decoration. You can use national
colors If it is more convenient to yon,
but wo want you In tho parade
whether you decorate or not. Thla
will bo strictly an automobile parade,
with sufficient speed to keep up radiation.
"Wo can confidently expect from
100 to 1E>0 automobiles In line, and
ask you to augment this number by
falling in with tho boys and nssiaHng
to make this parade the parade ot
the year.
"The procession will start from the
Hotel Vendome grounds at 10:30
o'clock on tbe morning of July 4th."

Effect of metoprolol CR/XL in chronic heart failure: Metoprolol CR XL Randomised Intervention Trial in Congestive Heart Failure (MERIT-HF)
No full textBACKGROUND: Metoprolol can improve haemodynamics in chronic heart failure, but survival benefit has not been proven. We investigated whether metoprolol controlled release/extended release (CR/XL) once daily, in addition to standard therapy, would lower mortality in patients with decreased ejection fraction and symptoms of heart failure. METHODS: We enrolled 3991 patients with chronic heart failure in New York Heart Association (NYHA) functional class II-IV and with ejection fraction of 0.40 or less, stabilised with optimum standard therapy, in a double-blind randomised controlled study. Randomisation was preceded by a 2-week single-blind placebo run-in period. 1990 patients were randomly assigned metoprolol CR/XL 12.5 mg (NYHA III-IV) or 25.0 mg once daily (NYHA II) and 2001 were assigned placebo. The target dose was 200 mg once daily and doses were up-titrated over 8 weeks. Our primary endpoint was all-cause mortality, analysed by intention to treat. FINDINGS: The study was stopped early on the recommendation of the independent safety committee. Mean follow-up time was 1 year. All-cause mortality was lower in the metoprolol CR/XL group than in the placebo group (145 [7.2%, per patient-year of follow-up]) vs 217 deaths [11.0%], relative risk 0.66 [95% CI 0.53-0.81]; p=0.00009 or adjusted for interim analyses p=0.0062). There were fewer sudden deaths in the metoprolol CR/XL group than in the placebo group (79 vs 132, 0.59 [0.45-0.78]; p=0.0002) and deaths from worsening heart failure (30 vs 58, 0.51 [0.33-0.79]; p=0.0023). INTERPRETATION: Metoprolol CR/XL once daily in addition to optimum standard therapy improved survival. The drug was well tolerated
