    Impaired calcium influx underlies skewed T helper cell differentiation in children with IgE-mediated food allergies

    Background: Reasons for Th2 skewing in IgE-mediated food allergies remains unclear. Clinical observations suggest impaired T cell activation may drive Th2 responses evidenced by increased atopic manifestations in liver transplant patients on tacrolimus (a calcineurin inhibitor). We aimed to assess differentiation potential, T cell activation and calcium influx of naïve CD4+ T cells in children with IgE-mediated food allergies. Methods: Peripheral blood mononuclear cells from infants in the Starting Time for Egg Protein (STEP) Trial were analyzed by flow cytometry to assess Th1/Th2/Treg development. Naïve CD4+ T cells from children with and without food allergies were stimulated for 7 days to assess Th1/Th2/Treg transcriptional factors and cytokines. Store operated calcium entry (SOCE) was measured in children with and without food allergies. The effect of tacrolimus on CD4+ T cell differentiation was assessed by treating stimulated naïve CD4+ T cells from healthy volunteers with tacrolimus for 7 days. Results: Egg allergic infants had impaired development of IFNγ+ Th1 cells and FoxP3+ transitional CD4+ T cells compared with non-allergic infants. This parallels reduced Tbet, IFNγ and FoxP3 expression in naïve CD4+ T cells from food allergic children after in vitro culture. SOCE of naïve CD4+ T cells was impaired in food allergic children. Naïve CD4+ T cells treated with tacrolimus had reduced IFNγ, T-bet, and FoxP3, but preserved IL-4 expression. Conclusions: In children with IgE-mediated food allergies, dysregulation of T helper cell development is associated with impaired SOCE, which underlies an intrinsic impairment in Th1 and Treg differentiation.     Oral Somatosensory Awareness

    Oral somatosensory awareness refers to the somatic sensations arising within the mouth, and to the information these sensations provide about the state and structure of the mouth itself, and objects in the mouth. Because the oral tissues have a strong somatosensory innervation, they are the locus of some of our most intense and vivid bodily experiences. The salient pain of toothache, or the habit of running one's tongue over one's teeth when someone mentions "dentist", provide two very different indications of the power of oral somatosensory awareness in human experience and behaviour. This paper aims to review the origins and structure of oral somatosensory awareness, focussing on quantitative, mechanistic studies in humans. We first extend a model of levels of bodily awareness to the specific case of the mouth. We then briefly summarise the sensory innervation of oral tissues, and their projections in the brain. We next describe how these peripheral inputs give rise to perceptions of objects in the mouth, such as foods, liquids and oral devices, and also of the mouth tissues themselves. Finally, we consider the concept of a conscious mouth image, and the somatosensory basis of "mouth feel". The theoretical framework outlined in this paper is intended to facilitate scientific studies of this important site of human experience

    Nowadays methods of standard and non-standard computerized perimetry using stationary devices are widely used in functional screening of glaucoma. The information about new portable devices for perimetry has appeared in foreign literature in recent years, describing such advantages as economic availability, autonomy and mobility, which open up new possibilities for their use. New possibilities include cloud storage of data, the use of telemedicine technologies, artificial intelligence, examination of patients with disabilities, including those who are bedridden, as well as examination outside of medical institutions. All of this can allow glaucoma patients to receive ophthalmic care when in-person visits are unavailable, including social distancing needed during a pandemic or quarantine. This review of the literature describes the latest portable devices and applications for perimetry, attempts to classify them according to similar parameters, and assesses their advantages and disadvantages, as well as the prospects for their use in functional screening of glaucoma.В настоящее время для функционального скрининга глаукомы широко используются методы стандартной и нестандартной компьютерной периметрии с использованием стационарных приборов. В зарубежной литературе последних лет появилась информация о новых портативных устройствах, предназначенных для периметрии, преимуществами которых, помимо экономической доступности, являются автономность и мобильность, открывающие новые возможности их применения. К новым возможностям относятся также облачное хранение данных, использование технологий телемедицины, искусственного интеллекта, обследование пациентов с ограниченными возможностями, в том числе лежачих, а также обследование вне медицинских учреждений. Все это позволит обеспечить глаукомных больных офтальмологической помощью во время их дистанцирования, в том числе социального, вследствие пандемии и карантина. Обзор литературы посвящен описанию новейших портативных устройств и приложений, предназначенных для периметрии, попытке их классифицировать по схожим параметрам, оценке преимуществ и недостатков, а также перспектив использования их для функционального скрининга глаукомы
