Ninjin'yoeito Improves Genitourinary Symptoms in Patients with Frailty
Urinary symptoms such as urinary frequency and urinary incontinence tend to appear with age and significantly reduce the quality of life. Moreover, many women have concurrent genital and urethral pain. Elderly women are more likely to experience genitourinary symptoms. Further, some patients with frailty experience frequent urination, overactive bladder, and urinary incontinence. Frailty may occur due to age-related tissue fragility. Hence, we hypothesized that frailty treatment would improve genitourinary symptoms. Ninjin'yoeito (NYT), a traditional Chinese herbal medicine composed of 12 crude drugs was used for frailty treatment. Comparing postoperative outcomes of two YAG laser treatment in superficial dyspareunia of breast cancer survivors through propensity score matching analysis with 2 years follow-up
Objective: Neodymium:Thorium-Aluminum-Garnet (Nd:YAG) laser and vagina as energy-based devices (EBDs) for superficial dyspareunia in postmenopausal women (PMW) suffering from genitourinary syndrome of menopause (GSM). We aimed to evaluate the long-term efficacy of an erbium laser (VEL) combination treatment (VEL+Nd:YAG).
METHODS: This study included a breast cancer survivor (BCS), sexually active her PMW reporting superficial dyspareunia as a GSM. 256 patients with either VEL+Nd:YAG or VEL were enrolled. We then retrospectively compared the 2-year postoperative data using propensity score matching analysis.
Symptoms were assessed using the Subjective Visual Analogue Scale (VAS) of Superficial Dyspareunia (VAS) before each laser treatment, immediately after each laser treatment, and for 24 months after the end of treatment

Comparison between VEL therapy and MUS in the treatment of stress and mixed urinary incontinence
Abstract
Objective
Stress urinary incontinence (SUI) and mixed urinary incontinence (MUI) significantly impact one's quality of life. In Japan, the standard treatments for urinary incontinence involve the use of tension-free vaginal tape (TVT) or transobturator tape (TOT) sling procedures, which include the insertion of synthetic materials. However, issues associated with artificial mesh have prompted the exploration of alternative treatment approaches. Therefore, this study investigates the potential benefits of laser therapy using an erbium-doped yttrium aluminum garnet laser directed into the vagina and urethra, comparing its effects with the conventional treatments.
Methods
The study enrolled patients who underwent TVT, TOT, or laser therapy (n = 50 in each group). Evaluations were conducted using the 1-hour pad test, the International Consultation on Incontinence Questionnaire Short Form (ICIQ-SF), and the overactive bladder symptom score both before and 12 months after treatment. In the case of laser therapy, a vaginal probe was inserted after applying a local anesthetic to the vaginal wall, followed by 20 minutes of irradiation at a wavelength of 2940 nm. This laser treatment was administered three times, with two-month intervals between sessions.
Results
Based on the results of the 1-hour pad test and ICIQ-SF, all three groups—TVT, TOT, and laser therapy—demonstrated comparable improvements in SUI symptoms. However, among patients with MUI, some individuals in the TVT and TOT groups experienced exacerbation, whereas those in the laser therapy group generally exhibited improvement.
Conclusions
This study confirms the efficacy of laser therapy in the treatment of urinary incontinence. It is worth noting that this research is the first to report on the impact of laser therapy on urinary incontinence in Japanese women

A comparative analysis of urethral sling surgery and non-ablative vaginal Erbium:YAG laser treatment in 327 patients with stress urinary incontinence.
Stress urinary incontinence (SUI) is a condition characterized by urine leakage due to increased abdominal pressure. In this study, we compared tension-free vaginal tape (TVT) and non-ablative vaginal Erbium:YAG laser treatment (VEL) for women with SUI using propensity score (PS) analysis. We included 102 patients in the TVT group, 113 patients in the VEL group, and 112 patients in the control group, all aged 35 to 50, from multiple medical institutions.
Both the TVT and VEL groups showed significant improvements in the 1-hour pad test and ICIQ-SF scores compared to the control group. PS analysis revealed similar improvements in the 1-hour pad test and ICIQ-SF scores for both groups. However, the VEL group exhibited a significantly greater improvement in OABSS compared to the TVT group.
Odds ratio analysis for the 1-hour pad test did not reveal any significant differences between the TVT and VEL groups. These findings suggest that VEL could be considered as an alternative treatment option to TVT for managing SUI
Retrospective Study on Prognostic Factors for Post-void Residual Volume Following Intravesical Injection of Botulinum Toxin for Severe Overactive Bladder: A Urodynamic Investigation
Severely overactive bladder patients who visited Yokosuka Women's Urology and Urology Clinic (abbreviated as Yokosuka Women's Urology) in Japan received intravesical injection treatment of botulinum toxin (abbreviated as BOTOX bladder treatment) covered by national health insurance, in accordance with Japanese guidelines. During this treatment, necessary and sufficient therapy and examinations are conducted following the criteria set by the country and guidelines provided by the Japanese Urological Association. The analysis of these examination data is of utmost importance.
Currently, studies on predictive factors for post-treatment effects and side effects in this treatment are based on data sets involving American subjects, calculated by researchers from a university in Taiwan. There have been no research reports from facilities in other countries. Therefore, this retrospective trial aims to investigate the predictive factors in an Asian population under the same conditions, conducted at medical facilities in Japan. It should be noted that post-void residual refers to the condition where 200ml or more of urine remains in the bladder immediately after voiding, as defined by the International Continence Society
