10 years follow-up of 60 cleft lip patients treated for secondary nasal defects
The purpose is to evaluate the long term results of 60 of 180 patientstreated with an "Anatomical Balance" method of secondary nasal correction,which means detouch and dismount progressively the septum,the alar cartilages, the bone vault and finally the soft tissues for thenre-build and re-orient and, if necessary, graft the nose structure withautologous material (1-2). 60 patients treated from 1993 to 2002, 39mono and 21 bi-lateral, medium age 24.5 years. Our procedures weredirected to the nasal tip, preserving and re-orienting the nasal structureswith an open approach and applying a personal technique called BCRCC(3) (14% of the patients), re-modelling and re-positioning of the domes(90% of the cases). Cartilage and bone have been used as grafts in80% of our patients The follow-ups made on our 60 patients after 1 to10 years seem to support the results that guarantee a better shaping ofthe tip without the need of a second surgical time. No major immediateand long term complication was observed except columellar scars (4%Symmetry and nasal function improved in all our cases (98.9%). Overthe years the first surgical step in cleft li patients has been the correctionof the lip, and then the remaining structures (Salyer 1992, Chase 1983,Mulliken 1992). Finally we can conclude that the primary treatment isof the lip-nasal deformity (Gubisch 1995, Salyer 2004). The treatmentof secondary nasal defects with the "anatomical Balance" method givean improvement of the tip projection by means of various techniques,one of them to be known as the Bilateral Controlateral Rotation of theupper Cephalic segment of the lateral Crura (BCRCC) (Nocini et al 2001)avoiding further surgical treatments.References[1] J.B. TRATTATO DI CHIRURGIA PREPROTESICA E INGEGNERIA TISSUTALE
Nella nostra trentennale esperienza di Clinici Odontostomatologici e di Chirurghi Oro-Maxillo-Facciali, la chirurgia preprotesica ha subito un’evoluzione tanto rapida ed importante tale da trasformare radicalmente molti dei suoi canoni e parametri inizialmente stabiliti.
E per noi motivo di soddisfazione potervi presentare questo testo interamente dedicato alla chirurgia preprotesica, articolato campo della Chirurgia Oro-Maxillo-Facciale.
Questo libro, che riflette un'attualità in costante evoluzione, riporta la preziosa esperienza e le capacità tecniche e didattiche del Professor Nocini e dei suoi Coautori, i quali hanno maturato nel corso di lunghi anni di apprendistato, sotto la nostra attenta e partecipe direzione, un bagaglio di conoscenze cliniche e scientifiche che superano il semplice nozionismo.
Nel raccomandarvi questo testo, noi siamo certi che diventerà un utile strumento per un corretto inquadramento diagnostico delle diverse patologie e per facilitare l'orientamento nella rosa dei possibili trattamenti chirurgici preprotesici, finalizzati all'ottenimento del miglior risultato funzionale ed estetico per il paziente.
Noi auspichiamo una condivisione tra l’Odontostomatologia e la Chirurgia Oro-Maxillo-Facciale evitando quindi la frammentazione in diverse super-specialità, poiché questo potrebbe risultare un impoverimento della cultura delle future generazioni di professionisti.

Implementing post-orthodontic compliance among adolescents wearing removable retainers through Whatsapp: a pilot study

All patients were scheduled for quarterly check-ups for monitoring of orthodontic stability by measurement of intercanine width at the beginning of the study (t0) and every 4 months t1, t2, t3) for 1 year of observation. Patients in the SG additionally participated in a WhatsApp chat group, where they would send, on a weekly basis, snapshots showing his/her occlusion. Every month, the orthodontist acting as the moderator awarded the best five snapshots by publishing a ranking in the chat.Results: Participants in the SG featured smaller changes from the intercanine widths at baseline (at debonding) compared to the CG patients at all times during 1 year of follow-up. Follow-up attendance was regular in both groups in the first 8 months of follow-up. After that, patient compliance decreased in the CG, with eight patients missing check-up appointments.Conclusion: Engaging adolescent patients directly through WhatsApp activity seems to increase regularity in wearing removable retainers, attendance to follow-up schedule, and yield better long-term outcome in terms of orthodontic stability and compliance.Keywords: apps, technologies, orthodontics relapse, orthodontic compliance, patient compliance, young patient
La correlazione tra microbiota del cavo orale e quadri di parodontopatia nei pazienti oncologici con ricostruzioni con lembi liberi microchirurgici fascio-cutanei per neoplasie del cavo orale: Stato dell’arte e prospettive future.
Obiettivo
Valutare il ruolo del microbiota associato a conclamati stati di parodontopatia in pazienti affetti da neoplasie maligne del cavo orale, sottoposti a chirurgia oncologica maggiore con ricostruzione con lembi microvascolari fascio cutanei. Valutare quindi possibili problematiche di deiscenze delle ricostruzioni nell’immediato post operatorio o di recidive anche a lungo termine date da questo stato di alterazione del microbiota.
Metodi
Nell studio sono stati inclusi 121 pazienti affetti da neoplasie del cavo orale, sottoposti a chirurgia demolitiva maggiore e ad un contestuale intervento ricostruttivo con lembi microvascolari fascio- cutanei (ALT e RFFF). È stata eseguita una valutazione perioperatoria per la presenza di una parodontopatia come fattore indicante un quadro di alterazione del microbiota e le possibili conseguenze su possibili influenze nella riuscita della ricostruzione e da un punto di vista di recidive neoplastiche.
Risultati
Il ruolo del microbiota nei pazienti con neoplasie del cavo orale si è evidenziato essere presente in un discreto numero di pazienti, tutti i pazienti che hanno avuto un insuccesso del lembo presentavano importanti parodontopatie e dei pazienti che durante il periodo di follow up hanno mostrato una recidiva XX su XX presentavano parodontopatie e conseguentemente ciò fa supporre delle alterazioni del microbiota.
Conclusioni
Nonostante il campione limitato, i risultati suggeriscono che i pazienti con neoplasie del cavo orale presentano in maniera frequente anche parodontopatie e che tale condizione influisce in maniera minima sulla riuscita della ricostruzione microvascolare ma, ha una rilevanza statistica più evidente per il rischio di recidive neoplastiche sia in sede primitivi che in regione latero-cervicale linfonodale
New Clinical Analysis and Device for Botox Injections
Background: Clinical botulinum toxin (BT) injections are done into recognized anatomical sites with no precise reference lines to make records of our treatments and to plan and position the toxin. Botulinum toxin injections are also characterized for different level of pain on different sites of injection. Objective: The aim of this study is to provide a treatment guide to improve precision and make records, and to evaluate the different levels of pain during the injection. Methods: The same surgeon injected BT into 2 groups of patients; in 25, the authors applied the grid, and in 25 patients the authors did not apply the grid. In the first group of patients, the authors used the new device (Juvapen); for the injection in the second group, the authors performed the injection by the standard procedure. At the end of each procedure, the authors evaluated the level of pain by visual method. Results: Results included 97% degree of symmetry and a 100% patient satisfaction. The mean level of pain in the first group of patient was 3, and in the second group it was 8. Limitations: The authors will apply the grid with the device on a wider population of patients. Conclusions: Facial BT grid analysis lines allow an individualized treatment improving the long-term results. Juvapen permits a less painful injection
Bilateral reconstruction of the mandibular body with symphyseal preservation using a single fibula free flap: operative technique
BACKGROUND: Mandibular osteonecrosis may occur in 5% of the patients who undergo radiotherapy for the treatment of head and neck malignancies. Resection and microvascular reconstruction is the treatment of choice in complicated osteoradionecrosis, however multifocal presentation may complicate the management of the disease given the poor quality and limited availability of adequate recipient vessels. OPERATIVE TECHNIQUE: A 74-year-old man affected by multifocal severe osteoradionecrosis of the mandible underwent bilateral resection of the mandibular bodies while preserving the symphysis. The defects were reconstructed with a single fibula flap composed by two bony segments connected by a central segment, corresponding to the symphyseal region, in which the bone was dissected and removed. The anastomosis was performed on a single side of the neck. Healing was uneventful and the adopted technique allowed for a quick functional and esthetic recovery. CONCLUSION: The presented technique provided a safe and efficacious, although technically challenging, solution in a case presenting multifocal osteonecrosis of the jaw. The morbidity of the procedure was limited because the tissue resection and reconstruction processes were minimized. GRAPHICAL ABSTRACT: [Image: see text
Prevenzione dell’osteonecrosi dei mascellari associata all’uso di bisfosfonati
l'esteso utilizzo di bisfosfonati nella pratica clinica. soprattutto in ambito oncologico. ha determinato un numero crescente di casi di una grave complicanza a carico delle ossa mascellari, che si manifesta con l'esposizione in cavo orale di ampie aree di osso necrotico. Gli effetti devastanti della malattia a livello del cavo orale e le scarse conoscenze in letteratura, hanno reso necessario l'allestimento di protocolli di prevenzione e gestione dentale nei pazienti maggiormente esposti al rischio.
