Recent advances in paediatric sleep disordered breathing
No full textThis article reviews the latest evidence pertaining to childhood sleep disordered breathing (SDB), which is associated with negative neurobehavioural, cardiovascular and growth outcomes. Polysomnography is the accepted gold standard for diagnosing SDB but is expensive and limited to specialist centres. Simpler tests such as cardiorespiratory polygraphy and pulse oximetry are probably sufficient for diagnosing obstructive sleep apnoea (OSA) in typically developing children, and new data-processing techniques may improve their accuracy. Adenotonsillectomy is the first-line treatment for OSA, with recent evidence showing that intracapsular tonsillectomy results in lower rates of adverse events than traditional techniques. Anti-inflammatory medication and positive airway pressure respiratory support are not always suitable or successful, although weight loss and hypoglossal nerve stimulation may help in select comorbid conditions.Educational aims: · To understand the clinical impact of childhood sleep disordered breathing (SDB).· To understand that, while sleep laboratory polysomnography has been the gold standard for diagnosis of SDB, other diagnostic techniques exist with their own benefits and limitations.· To recognise that adenotonsillectomy and positive pressure respiratory support are the mainstays of treating childhood SDB, but different approaches may be indicated in certain patient groups.</p

Improving the piezoelectric properties of thick-film PZT: the influence of paste composition, powder milling process and electrode material
No full textThis paper details improvements of the d33 coefficient for thick-film Lead–Zirconate–Titanate (PZT) layers. In particular, the effect that the powder milling process has on particle size, shape and distribution has been investigated. Ball milled, jet milled and attritor milled powders were obtained from Morgan Electro-Ceramics Ltd. These powders were mixed with various ratios of lead borosilicate glass in the range of 5–20% by weight and an appropriate quantity of Electro-Science Laboratories (ESL) 400 solvent to formulate a screen printable thixotropic paste. The use of a polymer top electrode to reduce the number of firing cycles the PZT layer is subjected to was also investigated. The results show that the highest values of d33 were obtained from the ball milled powder with 10% glass content, but the most consistent results were obtained from the attritor milled samples. The samples printed with a polymer top electrode have shown an average increase of around 15% in the value of d33

No full textThis chapter discusses the 2007 Grand Chamber judgment of Evans v. The United Kingdom, in which the European Court of Human Rights stated that the destruction of frozen embryos against the will of the female contributor, who had invested her last egg cells in these embryos, did not violate the Convention. The chapter argues in favour of a different outcome. The alternative approach to Evans follows a method that stands on two legs. The first leg is a three-step model for addressing conflicts between human rights, that the author has developed in previous research. The second leg is an integrated perspective of international human rights law, that brings in insights from women’s rights and disability rights.</p
