    Acute myeloid leukemia in the elderly : 159 Nagoya case studies

    1999-11To obtain background information on elderly acute myeloid leukemia (AML), unselected data covering 159 patients aged 60 years or over with AML from 14 hospitals in Nagoya, Japan was analyzed retrospectively. Among these patients, 119 had de novo acute AML, 32 had AML which evolved from myelodysplastic syndrome (MDS-AML), and 8 had other types of leukemia. The survey showed that MDS-AML tended to be more prevalent in patients aged 70 years and older and that MDS-AML showed a significantly more severe degree of leukopenia and anemia than de novo AML. MDS-AML also showed a significantly lower complete remission (CR) rate than that of de novo AML [6.9% (2/29) vs 58.3% (67/11), P < 0.01] and significantly shorter survival times than those of de novo AML [median: 3.6 months vs 9.6 months, P < 0.01 (generalized Wilcoxon test; GW]. In de novo AML, the proportion of patients treated with conventional therapy (CT group) decreased significantly, and that of those with attenuated therapy (AT group) increased significantly as age elevated (P < 0.01). The CT group showed a significantly higher CR rate (65.4% vs 41.2%, P < 0.05) and a significantly longer survival period than those of the AT group [median: 11.6 months vs 4.8 months, P < 0.05 (GW)].     18 YEARS OF CONFORMATION RADIOTHERAPY AT NAGOYA UNIVERSITY HOSPITAL

    1996-03-29Conformation radiotherapy is one of the best techniques for minimizing the radiation dose absorbed by the surrounding normal tissue while delivering a high dose to a cancerous target area. The cases of all patients who underwent external irradiation at Nagoya University Hospital from 1975 to 1992 were reviewed. A total of 5740 patients with 6179 lesions were irradiated during this time, and 3795 treatment plans involved radical intended irradiation. Of the 5740 patients, 1017 had head and neck cancer, 982 had cervical cancer, 506 had lung cancer, 439 had primary brain tumors, 308 had esophageal cancer, 1213 had metastatic tumors, and 1275 had other types of tumors. The total number of treatment plans per year decreased from 442 in 1975 to 292 in 1992. Likewise, the percentage of conformation radiotherapy performed in all patients decreased from 29.4% (130/442) in 1975 to 8.6% (25/292) in 1992. It occupied 14.5% (982/6179) of all intended plans, and 20% (775/3795) of radical treatment plans. The conformation technique was used in cases of cervical cancer (72%), esophageal cancer (65%) and primary brain tumors (25%). Boost Conformation radiotherapy represented 2% of all treatment planning and 29% of the conformation radiotherapy. Boost Conformation radiotherapy has recently become more popular and now represents more than 50% of conformation radiotherapy. With respect to cases of cervical cancer, the rates of local recurrence and late complications in cases treated by conformation radiotherapy were lower than in cases treated by two parallel opposed radiotherapy.departmental bulletin pape

    An Epidemiologic Study on Congenital Malformations in Nagoya

    1975-09The subjects analysed in the present study were 41 ,835 total births in Nagoya in 1965, consisting of 41,028 live births and 807 la te stillbirths. Among the stillbirths 42 babies were the malformed and of 644 deaths in the subsequent four years 96 deaths with malformations were identified. Average linking rate of death and live birth certificates was 96.1 percent. The incidences of congenital malformations per 1,000 births were 3.30 in total births, 2.34 in live births and 52.04 in late stillbirths. Those in the cardiovascular system were 1.20 in total births, 1.17 in live births and 2.48 in late stillbirths. The corresponding figures were 0.93, 0.34 and 30.98 in the central system and 0.69, 0.56 and 7.43 in the alimentary system, respectively. Significantly greater relative risks were observed with shorter gestational period, lower birth weight and higher birth order. They were also noted in older working mothers with heavy physical loads, twin births and illegitimate births. Analyses by major anatomical system revealed significantly greater relative risks with shorter gestational period in the cardiovascular, central nervous and alimentary systems, with lower birth weight in the cardiovascular system and with higher birth order in the central nervous system. Paternal age and parental age difference exerted insignificant effects on malformations, though an increasing tendency of risk with advancing paternal age and with wider parental age difference was interestingly noted in the malformations of the central nervous system.departmental bulletin pape

    Genotype announcement in a genetic polymorphism study for health checkup examinees at Nagoya University Hospital

    2004-05On June 9, 2003, we started free genetic tests of eight polymorphisms for health checkup examinees who attended a basic course at Nagoya University Hospital. They were informed of their genotypes within four weeks after blood donation for research purposes. The genotypes were those of alcohol dehydrogenase 2 (ADH2) Arg47His, aldehyde dehydrogenase 2 (ALDH2) Glu487Lys, NAD(P)H: quinone oxidoreductase (NQO1) C609T, glutathione S transferase M1 (GSTM1), glutathione S-transferase T1 (GSTT1), interleukin-1B (IL-1B) C-31T, and tumor necrosis factor A (TNF-A) T-1031C, angiotensin-converting enzyme (ACE) Ins/Del. In the first three months, 227 (89.4%) out of 254 examinees participated in the free tests, having been informed of the research aims, after which they consented to our use of research data. To date, there have been no complaints from the participants, indicating that the announcement of polymorphism genotypes may be accepted differently from that of hereditary disease genotypes.departmental bulletin pape
