Effectiveness of vaginal tablets containing lactobacilli versus pH tablets on vaginal health and inflammatory cytokines: A randomized, double-blind study
The purpose of this study was to evaluate the effectiveness of lactobacilli on vaginal health and proinflammatory cytokines. Sixty-seven patients with bacterial vaginosis (BV), 50 with intermediate flora and 42 with normal vaginal flora were enrolled in this double-blind study. The subjects were randomized to receive probiotic lactobacilli vaginal tablets (L. brevis CD2, L. salivarius subsp. salicinius, L. plantarum) or the vaginal pH tablet (active comparator). Cervico-vaginal lavage was collected to measure the concentrations of IL-1β, TNFα and IL-6 by ELISA. Neutral sphingomyelinase activity was also quantified in both arms before and after treatment. The probiotic vaginal tablet was well tolerated and no side effects were reported. The study demonstrated a cure rate of nearly 80 %; i.e., 32 % of the women could restore normal vaginal flora and 47 % had improved Nugent score, whereas 20 % of the subjects did not clear BV in the first follow-up (after 8 days treatment). The pH tablet containing pH lowering compounds induced resolution of BV and restoration of normal vaginal flora in 74 % and 26 %, respectively. The lactobacilli tablet was found to be better than the pH tablet in preventing BV in healthy subjects. A significant reduction in IL-1β and IL-6 vaginal cytokines was observed after treatment with lactobacilli, while the active comparator did not have any effect on local proinflammatory cytokines. Vaginal neutral sphingomyelinase activity was not modified in either group. Vaginal tablets containing lactobacilli can cure BV and reduce vaginal inflammatory response. © 2012 Springer-Verlag
Evaluation of vaginal pH for detection of bacterial vaginosis
Background & objectives: Bacterial vaginosis (BV) is highly prevalent among women in reproductive age group. Little information exists on routine vaginal pH measurement in women with BV. We undertook this study to assess the utility of vaginal pH determination for initial evaluation of bacterial vaginosis. Methods: In this cross-sectional study vaginal swabs were collected from women with complaints of white discharge, back ache and pain abdomen attending a government hospital and a community health clinic, and subjected to vaginal pH determination, Gram stain, wet mount and whiff test. Nugent score and Amsel criteria were used for BV confirmation. Results: Of the 270 women included in the analysis, 154 had BV based on Nugents' score. The mean vaginal pH in women with BV measured by pH strips and pH glove was 5 and 4.9, respectively. The vaginal pH was significantly higher in women with BV. Vaginal discharge was prevalent in 84.8 per cent women, however, only 56.8 per cent of these actually had BV by Nugent score (NS). Presence of clue cells and positive whiff test were significant for BV. Vaginal pH >4.5 by pH strips and pH Glove had a sensitivity of 72 and 79 per cent and specificity of 60 and 53 per cent, respectively to detect BV. Among the combination criteria, clue cells and glove pH >4.5 had highest sensitivity and specificity to detect BV. Interpretation & conclusions: Vaginal pH determination is relatively sensitive, but less specific in detecting women with BV. Inclusion of whiff test along with pH test reduced the sensitivity, but improved specificity. Both, the pH strip and pH glove are equally suitable for screening women with BV on outpatient basis
A mechanism for IL-10-mediated diabetes in the nonobese diabetic (NOD) mouse: ICAM-1 deficiency blocks accelerated
Neonatal islet-specific expression of IL-10 in nonobese diabetic (NOD) mice accelerates the onset of diabetes, whereas systemic treatment of young NOD mice with IL-10 prevents diabetes. The mechanism for acceleration of diabetes in IL-10-NOD mice is not known. Here we show, by adoptive transfers, that prediabetic or diabetic NOD splenocytes upon encountering IL-10 in the pancreatic islets readily promoted diabetes. This outcome suggests that the compartment of exposure, not the timing, confers proinflammatory effects on this molecule. Moreover, injection of IL-10-deficient NOD splenocytes into transgenic IL-10-NOD scid/scid mice elicited accelerated disease, demonstrating that pancreatic IL-10 but not endogenous IL-10 is sufficient for the acceleration of diabetes. Immunohistochemical analysis revealed hyperexpression of ICAM-1 on the vascular endothelium of IL-10-NOD mice. The finding suggests that IL-10 may promote diabetes via an ICAM-1-dependent pathway. We found that introduction of ICAM-1 deficiency into IL-10-NOD mice as well as into NOD mice prevented accelerated insulitis and diabetes. Failure to develop insulitis and diabetes was preceded by the absence of GAD65-specific T cell responses. The data suggest that ICAM-1 plays a role in the formation of the 'immunological synapse', thereby affecting the generation and/or expansion of islet-specific T cells. In addition, ICAM-1 also played a role in the effector phase of autoimmune diabetes because adoptive transfer of diabetogenic BDC2.5 T cells failed to elicit clinical disease in ICAM-1-deficient IL-10-NOD and NOD mice. These findings provide evidence that pancreatic IL-10 is sufficient to drive pathogenic autoimmune responses and accelerates diabetes via an ICAM-1-dependent pathway

Delayed Default Dependency and Default Contagion
Delayed, hence non-simultaneous, dependent defaults are discussed in a reduced form model. The model is a generalization of a multi-factor model based on simultaneous defaults to incorporate delayed defaults. It provides a natural smoothening of discontinuities in the joint probability densities in models with simultaneous defaults. It is a dynamic model that exhibits default contagion in a multi-factor setting. It admits an efficient Monte Carlo simulation algorithm that can handle heterogeneous collections of credit names. Asymptotics of insensitive load balancing and blocking phases
We study a single class of traffic acting on a symmetric set of processor-sharing queues with finite buffers, and we consider the case where the load scales with the number of servers. We address the problem of giving robust performance bounds based on the study of the asymptotic behaviour of the insensitive load balancing schemes, which have the desirable property that the stationary distribution of the resulting stochastic network depends on the distribution of job-sizes only through its mean. It was shown for small systems with losses that they give good estimates of performance indicators, generalizing henceforth Erlang formula, whereas optimal policies are already theoretically and computationally out of reach for networks of moderate size. We characterize the response of symmetric systems under those schemes at different scales and show that three amplitudes of deviations can be identified according to whether ρ 1. A central limit scaling takes place for a sub-critical load; for ρ= 1 , the number of free servers scales like nθθ+1 (θ being the buffer depth and n being the number of servers) and is of order 1 for super-critical loads. This further implies the existence of different phases for the blocking probability. Before a (refined) critical load ρc(n)=1-an-θθ+1, the blocking is exponentially small and becomes of order n-θθ+1 at ρc(n). This generalizes the well-known quality-and-efficiency-driven regime, or Halfin—Whitt regime, for a one-dimensional queue and leads to a generalized staffing rule for a given target blocking probability.Fil: Jonckheere, Matthieu Thimothy Samson. Use of stable isotopes, organic and inorganic chemistry to identify pollution sources and weathering processes in two small tropical rivers in southwestern India
The two main objectives of this study were to assess pollution dynamic from organic and inorganic major ion chemistry and stable isotopes (δ15N and δ18O) and to determine the weathering processes using carbon isotopes in two tropical river basins, i.e. Nethravati and Swarna, along southwest coast of India. These short length river basins (around 100 km) are characterized by high annual rainfall, warm temperature, high runo" (~3300mm) draining Precambrian basement rocks composed of green-stones, granitic-gneiss, charnockite and meta sediments. Intense silicate weathering is induced by high runo" and warm temperature (Gurumurthy et al., 2012). In this study, stable isotopes (δ15N & δ18O)of organic molecules from sewage and agricultural effluents,and carbon isotopes (δ13C) of dissolved organic carbon (DOC) and dissolved inorganic carbon (DIC) were measured to trace agricultural and domestic pollution and to identify the sources of inorganic carbon and the nature of chemical weathering in these river basins. Carbon isotopes measured on DIC reveals sources of carbon into the river, such as carbonate/silicate weathering of rocks, mineralization of organic matter from C3/C4 plants, soil and atmospheric CO2. The nitrate and phosphate levels remain low, with values ranging from 5 to 9 μM, and 0 to 2 μM respectively. The δ13C DIC values range from =-9.03 +/- 0.99 for the Swarna basin to -8.08 +/-0.78 for the Nethravati basin. These values point to a mixing of carbonate and silicate weathering products with a dominance of C3 vegetation, prevalent in the Western Ghats. The DOC values for both river basins are very low and very close: 0.72 +/- 0.09 mg/L (Swarna river) and 0.62 +/-0.11 mg/L (Nethravati river). This indicates that the contributions of organic matter from the adjacent forests and the $ood plains are very low during the sampling period. The analysis of organic acids reveals low amount of Oxalate and Acetate, and trace of Malate and Tartaric acids. The dissolved and particulate organic carbon (DOC and POC) concentrations are very low in these two rivers. During the dry season, river discharge is mainly supplied by groundwater with generally low contents in dissolved and particulate fractions. Even if we observe low concentration, we measured higher DOC and POC in the Swarna river. These higher DOC concentrations are accompanied with lower SUVA value. This indicates that more labile DOC (less aromaticity) is exported within this basin during dry season. C/N values in POC also show that the organic carbon is “fresher” and is probably more autochtonous than in the Nethravati river. Indeed, C/N value are closer of an autochthonous production (C/N : 2-6) than allochthonous one (C/N: 8-20). These observations can be explained as the Svarna watershed land use is more agricultural than in Nethravati. Agricultural lands generally export signi%cant amount of nutrients to rivers and participate to enhance autochthonous productivity. Dark Matter in Fractional Gravity II: Tests in Galaxy Clusters
Recently, in Benetti et al. (Astrophys. J. 2023, 949, 65), we suggested that the dark matter (DM) component in galaxies may originate fractional gravity. In such a framework, the DM component exists, but the gravitational potential associated to its density distribution is determined by a modified Poisson equation including fractional derivatives (i.e., derivatives of noninteger type),
which are meant to describe nonlocal effects; as such, this scenario is different from theories where baryonic matter emulates DM-like effects via modifications of gravity (e.g., MONDian frameworks).
In Benetti et al., we showed that fractional gravity worked very well for reproducing the kinematics of disk-dominated galaxies, especially dwarfs; there is also preliminary evidence that the strength of fractional effects tends to weaken toward more massive systems. Here, we aim to test fractional
gravity in galaxy clusters, with a twofold aim: (i) perform an independent sanity check that it can accurately describe such large and massive structures; (ii) derive a clear-cut trend for its strength in systems with different DM masses. To this purpose, we forward model the density and pressure
distributions of the intracluster medium (ICM), working out the hydrostatic equilibrium equation in fractional gravity. Then, we perform a Bayesian analysis of the X-COP galaxy cluster sample and infer constraints on the fractional gravity parameters, for individual clusters as well as stacked
clusters. We find that fractional gravity performs remarkably well in modeling the ICM profiles for the X-COP sample. We also check that the DM concentration vs. mass relation is still consistent with the expectations of N-body simulations in the standard cosmological scenario. Finally, we
confirm the weakening of the fractional gravity effects toward more massive systems and derive the overall scaling of the fractional gravity parameters from dwarf galaxies to massive clusters, spanning six orders of magnitude in DM mass. Such an overall trend implies that fractional gravity
can substantially alleviate the small-scale issues of the standard DM paradigm, while remaining successful on large cosmological scales
