    Effect of Psychological First Aid Intervention Program on Post-traumatic Growth¬ among Recently Diagnosed Cancer Patients

    Abstract: Cancer diagnosis and related treatment represent one of the most painful and distressing events that human beings can meet in their life. This causes a serious crisis in the lives of patients, their families, and those around them. This study aims to evaluate the effect of psychological first aid intervention program on post-traumatic growth among recently diagnosed cancer patients. Study design: A quasi-experimental design was utilized. Setting: The study conducted in the following hospitals at Ismailia city: Suez Canal University Hospitals and Ismailia Oncology Hospital. Subject: A sample of 52 recently diagnosed cancer patients. Tools of data collection: Two tools were used: Tool (I) demographic characteristics and medical background. Tool (II) Post-traumatic Growth Inventory (PTGI). Results: Findings revealed that, recently diagnosed cancer patients had low level of post-traumatic growth in pre-program phase while, post and follow up phases, the results revealed that patients had high level of post-traumatic growth. Conclusion: Psychological first aid intervention program had significant effect on improvement of post-traumatic growth of recently diagnosed cancer patients in post, and follow up study phases compared with pre-program phase. Recommendation: Psychological first aid based interventions can be combined with care programs of hospitals and daily clinical works as a complementary intervention.     Effect of Mindfulness Training Program on Internet Gaming Addictive Behavior among Suez Canal University Students

    Abstract: Background: Mindfulness is a psychological and coping strategy used to adjust maladaptive behaviors such as internet gaming addictive behavior and improve awareness and reduce stress, anxiety and depression. Aim of the study: Evaluate the effect of mindfulness training program on internet gaming addictive behavior among university students. Research design: Quasi-experimental research design with pre-post assessment was used. Setting: The study was conducted at Suez Canal university- Ismailia city in a randomly selected six faculties divided in to two medical, two practical and two theoretical faculties. Sample: A systematic random sample of 203 university students were selected in this study. Tools for data collection: Data was collected using five tools namely; knowledge about internet gaming, video game usage questionnaire, game addiction scale, problematic video game play revised scale and mindfulness attention awareness in addition to demographic characteristics sheet. Results: There was a highly statistically significant relation between low levels of highly engaged, problem gamers, and addicted gamers and high level of mindfulness. It is evident that game addiction increased with low level of mindfulness. After implementation of mindfulness training program, the majority of intervention group had high level of mindfulness compared to pre intervention. The statistical significant independent positive predictor of mindfulness attention awareness was the gaming addiction. Conclusion: The mindfulness training program had a positive effect on decreasing gaming addictive behavior among university students in intervention group. Recommendations: Dissemination of this mindfulness training program to other students in different faculties at Suez Canal University.     The Relationship between Workplace Incivility Behaviors and Nurses' Intention to Share Knowledge

    Abstract: Workplace incivility behaviors poisoning the organizational climate disrupts the working harmony of health care team and impairs interprofessional communication and collaboration. Aim: To examine the relationship between workplace place incivility behaviors and nurses' intention to share knowledge at Itay El-Baroud General Hospital. Methods: A descriptive, correlational research design was utilized in all inpatient medical, surgical and critical care units at Itay El-Baroud General hospital, El-Beheira, Egypt, for all head nurses and their assistants and staff nurses. Tools: two tools were used: Tool Ι: Nursing Incivility Scale (NIS) and a demographic characteristics data sheet; Tool Π: Intention to Share Knowledge Questionnaire. Results: staff nurses got moderate mean percent score for both workplace incivility and intention to share knowledge. Moreover, the first source of incivility was general environment; and the last one was patient/visitor. Whereas; the first dimension for nurses’ intention to share knowledge was attitude toward knowledge sharing, and the last dimension was anticipated extrinsic rewards. Conclusion: There are negative highly significant correlations between total workplace incivility behaviors and all its dimensions and total nurses’ intention to share knowledge and all its dimensions. Recommendations: Develop and disseminate workplace incivility policy; develop positive workplace culture; and provide incentives and rewards.     SARS-CoV-2 vaccination modelling for safe surgery to save lives: data from an international prospective cohort study

    Background: Preoperative SARS-CoV-2 vaccination could support safer elective surgery. Vaccine numbers are limited so this study aimed to inform their prioritization by modelling. Methods: The primary outcome was the number needed to vaccinate (NNV) to prevent one COVID-19-related death in 1 year. NNVs were based on postoperative SARS-CoV-2 rates and mortality in an international cohort study (surgical patients), and community SARS-CoV-2 incidence and case fatality data (general population). NNV estimates were stratified by age (18–49, 50–69, 70 or more years) and type of surgery. Best- and worst-case scenarios were used to describe uncertainty. Results: NNVs were more favourable in surgical patients than the general population. The most favourable NNVs were in patients aged 70 years or more needing cancer surgery (351; best case 196, worst case 816) or non-cancer surgery (733; best case 407, worst case 1664). Both exceeded the NNV in the general population (1840; best case 1196, worst case 3066). NNVs for surgical patients remained favourable at a range of SARS-CoV-2 incidence rates in sensitivity analysis modelling. Globally, prioritizing preoperative vaccination of patients needing elective surgery ahead of the general population could prevent an additional 58 687 (best case 115 007, worst case 20 177) COVID-19-related deaths in 1 year. Conclusion: As global roll out of SARS-CoV-2 vaccination proceeds, patients needing elective surgery should be prioritized ahead of the general population. © The Author(s) 2021.     Intraoperative endomanometric laparoscopic Nissen fundoplication improves postoperative outcomes in large sliding hiatus hernia with severe gastroesophageal reflux disease. A retrospective cohort study.

    Background: Laparoscopic Nissen Fundoplication (LNF) is the gold standard surgical intervention for gastroesophageal reflux disease (GERD). LNF can be followed by recurrent symptoms or complications affecting patient satisfaction. The aim of this study is to assess the value of the intraoperative endomanometric evaluation of esophagogastric competence and pressure combined with LNF in patients with large sliding hiatus hernia (> 5 cm) with severe GERD (DeMeester score >100). Materials and methods: This is a retrospective, multicenter cohort study. Baseline characteristics, postoperative dysphagia and gas bloat syndrome, recurrent symptoms, and satisfaction were collected from a prospectively maintained database. Outcomes analyzed included recurrent reflux symptoms, postoperative side effects, and satisfaction with surgery. Results: 360 patients were stratified into endomanometric LNF (180 patients, LNF+) and LNF alone (180 patients, LNF). Recurrent heartburn (3.9% vs. 8.3%) and recurrent regurgitation (2.2% vs. 5%) showed a lower incidence in the LNF+ group (P=0.012). Postoperative score III recurrent heartburn and score III regurgitations occurred in 0% vs. 3.3% and 0% vs. 2.8% cases in the LNF+ and LNF groups, respectively (P=0.005). Postoperative persistent dysphagia and gas bloat syndrome occurred in 1.75% vs. 5.6% and 0% vs. 3.9% of patients (P=0.001). Score III postoperative persistent dysphagia was 0% vs. 2.8% in the two groups (P=0.007). There was no redo surgery for dysphagia after LNF+. Patient satisfaction at the end of the study was 93.3% vs. 86.7% in both cohorts, respectively (P=0.05). Conclusions: Intraoperative high-resolution manometry (HRM) and endoscopic were feasible in all patients, and the outcomes were favorable from an effectiveness and safety standpoint

    Biliary fistula and late recurrence of liver hydatid cyst: Role of cysto-biliary communication: A prospective multicenter study

    Background: Hydatid cyst disease (HCD) is common in certain locations. Surgery is associated with postoperative biliary fistula (POBF) and recurrence. The primary aim of this study was to identify whether occult cysto-biliary communication (CBC) can predict recurrent HCD. The secondary aim was to assess the role of cystic fluid bilirubin and alkaline phosphatase (ALP) levels in predicting POBF and recurrent HCD. Aim: To identify whether occult CBC can predict recurrent HCD. The secondary aim was to assess the role of cystic fluid bilirubin and ALP levels in predicting POBF and recurrent HCD. Methods: From September 2010 to September 2016, a prospective multicenter study was undertaken involving 244 patients with solitary primary superficial stage cystic echinococcosis 2 and cystic echinococcosis 3b HCD who underwent laparoscopic partial cystectomy with omentoplasty. Univariable logistic regression analysis assessed independent factors determining biliary complications and recurrence. Results: There was a highly statistically significant association (P ≤ 0.001) between cystic fluid biochemical indices and the development of biliary complications (of 16 patients with POBF, 15 patients had high cyst fluid bilirubin and ALP levels), where patients with high bilirubin-ALP levels were 3405 times more likely to have biliary complications. There was a highly statistically significant association (P ≤ 0.001) between biliary complications, biochemical indices, and the occurrence of recurrent HCD (of 30 patients with recurrent HCD, 15 patients had high cyst fluid bilirubin and ALP; all 16 patients who had POBF later developed recurrent HCD), where patients who developed biliary complications and high bilirubin-ALP were 244.6 and 214 times more likely to have recurrent hydatid cysts, respectively. Conclusion: Occult CBC can predict recurrent HCD. Elevated cyst fluid bilirubin and ALP levels predicted POBF and recurrent HCD

    The Effect of Self-Learning Module on the Burden of Care for Children with Cerebral Palsy

    Cerebral Palsy is one of the most complex of the common permanent disabling conditions and disorder of movement, muscle tone and posture that is caused by damage that occurs most often before birth and during developing brain. Aim: the current study aimed to evaluate the effect of self-learning module on the burden of care for children with cerebral palsy. Design: A quasi-experimental research design (pre-posttest) was used. Setting: The study was carried out at Sett Khadra Child Health Center in Helwan referred from Helwan general hospital affiliated to Ministry of Health and Physiotherapy Unit in Mustafa Hassan Pediatric Hospital affiliated to Fyoum University Hospital. Sample consisted of 70 children and their accompaying caregivers suffering from cerebral palsy disease. Tool: consists of three tools to assess: I: Structured interview questionnaire sheet to assess caregivers’ knowledge regarding cerebral palsy and daily living activity II: Observational checklist to assess caregivers\u27 reported practices regarding care of their children with cerebral palsy) pre/post test (III: Caregiver’s attitude scale sheet to assess caregivers\u27 attitude regarding cerebral palsy. Results: there were statistical significance difference between caregivers’ characteristic and their total knowledge and total practices regarding cerebral palsy pre and post module. Also, there were statistically significant positive correlation between caregivers’ total attitude and their total practice and total knowledge in pre and post self-learning module. Conclusion: Based upon the results of the current study, it can be concluded that self-learning module had appositive effect on caregivers\u27 knowledge, practices and attitude regarding care of their children with cerebral palsy which led to decrease burden of care for children with cerebral palsy.     Scaling-up production of cost-effective and eco-friendly bio-fertilizer and its application on Barley green fodder via IoT hydroponic system

    Abstract Background Plant-associated microbes (endophytes) have a significant relationship to enhance plant growth and crop productivity by producing proficient bioactive metabolites. Since endophytes promoted plant growth either directly by releasing active metabolites such as phytohormones or indirectly by suppressing the growth of phytopathogens, so, in this work, biomass yield of local endophytic Trichoderma harzianum was maximized at shake-flask scale and scaled up via 7-L Bioflo310 fermenter using continuous exponential fed-batch fermentation mode. Subsequently, the effect of these cells as bio-fertilizer was assessed using two-barley grain genotypes (Russian and Egyptian seeds) via an intelligent hydroponic system based on Internet of Things (IoT). Results To reduce the cost of a biomass production line, agro-waste media containing potato, onion, garlic, pea, and cabbage peels were chosen as the culturing medium. The pea peel medium was found to be the best producer of biomass (2.2 g/L). The cultivation factors were evaluated to improve this biomass yield. The results showed that the maximum biomass production (4.9 g/L) was reported by adjusting the medium pH at 5.0 that inoculated with 10% of spore suspension, then incubated at 30°C, and 200 rpm. Then, this biomass yield was scaled up kinetically (505.4 g/L) by using exponential fed-batch fermentation mode via a 7-L bioreactor. The stimulation impacts of this endophytic T. harzianum on the growth of different barley genotypes (Russian and Egyptian seeds) were determined using a controlled hydroponic chamber. The total chlorophyll, carotenoid, and carbohydrate amounts in treated Russian showed the proficient stimulation percentage (81.05, 80, 40.8%) compared to the Egyptian barley groups (76.39, 73.5, 25.9%) respectively. Also, the maximum carbohydrate content (83.95 ± 1.7%) was recorded in the case of Russian barley. Conclusion Via this work, the optimal combination conditions for the cost-effective biomass production of endophytic T. harzianum were designed industrially via a fed-batch fermentation system using the cheapest culturing medium. Furthermore, by applying this promising bio-fertilizer, the total cost of barley production via an IoT hydroponic growing system was reduced.
