Aripiprazole in the treatment of challenging behaviour in adults with autism spectrum disorder
Background Autism spectrum disorders (ASD) are associated with repetitive behaviours and often also with hyperactivity, aggression, self-injurious behaviour, irritability and lability of mood. There is emerging evidence that aripiprazole, an antipsychotic with partial agonist dopaminergic effect, may be effective in the treatment of these challenging behaviours. Nevertheless, there is little evidence for their efficacy in adults with ASD. Objectives The aim of this article is to present preliminary data on the use of aripiprazole in the treatment of challenging behaviour in the setting of ASD. Methods We present a consecutive series of five inpatients of normal intelligence with challenging behaviour associated with ASD, diagnosed according to ICD-10 criteria, which was resistant to treatment with other medical and behavioural interventions and which was treated with aripiprazole. Results Four out of five patients were classified as "much improved" or "very much improved" according to the Clinical Global Impression-Improvement scale. Aripiprazole caused akathisia, at a dose of 30 mg in the one patient who was not classified as a responder but was otherwise well tolerated. Conclusions This is the first case series of adults with ASD presenting with challenging behaviour who have been treated with aripiprazole. While the results are promising, controlled trials are required to confirm the findings
The anatomy of extended limbic pathways in Asperger syndrome: A preliminary diffusion tensor imaging tractography study
It has been suggested that people with autistic spectrum disorder (ASD) have altered development (and connectivity) of limbic circuits. However, direct evidence of anatomical differences specific to white matter pathways underlying social behaviour and emotions in ASD is lacking. We used Diffusion Tensor Imaging Tractography to compare, in vivo, the microstructural integrity and age-related differences in the extended limbic pathways between subjects with Asperger syndrome and healthy controls. Twenty-four males with Asperger syndrome (mean age 23 ± 12 years, age range: 9-54 years) and 42 age-matched male controls (mean age 25 ± 10 years, age range: 9-54 years) were studied. We quantified tract-specific diffusivity measurements as indirect indexes of microstructural integrity (e.g. fractional anisotropy, FA; mean diffusivity, MD) and tract volume (e.g. number of streamlines) of the main limbic tracts. The dissected limbic pathways included the inferior longitudinal fasciculus, inferior frontal occipital fasciculus, uncinate, cingulum and fornix. There were no significant between-group differences in FA and MD. However, compared to healthy controls, individuals with Asperger syndrome had a significantly higher number of streamlines in the right (p = .003) and left (p = .03) cingulum, and in the right (p = .03) and left (p = .04) inferior longitudinal fasciculus. In contrast, people with Asperger syndrome had a significantly lower number of streamlines in the right uncinate (p = .02). Within each group there were significant age-related differences in MD and number of streamlines, but not FA. However, the only significant age-related between-group difference was in mean diffusivity of the left uncinate fasciculus (Zobs = 2.05) (p = .02). Our preliminary findings suggest that people with Asperger syndrome have significant differences in the anatomy, and maturation, of some (but not all) limbic tracts
Slight asymmetry in the winding angles of reinforcing collagen can cause large shear stresses in arteries and even induce buckling
Many models of the mechanical response of arteries assume a reinforcement with two families of helically wound fibres of collagen of opposite pitch. Motivated by experimental observations, the consequences for the internal pressurisation of arteries of a slight asymmetry in the winding angles is investigated here. It is shown that a torsional shear stress is generated as a result of this flaw, with some common models of the mechanical response of arteries exhibiting significant shear stresses. If the shear stress is significant, then the corresponding model would not seem to be robust, given that an infinitesimal change in a model parameter results in a large change in system response, although it is also shown that there is a ‘magic-angle’ for fibre winding that eliminates torsional shear stress for many of the commonly used models. Finite Element simulations are used to further illustrate the main consequences of fibre asymmetry for some of the more common models of arterial response. If the fibre asymmetry is localised in a region, then simulations show that there is the possibility of significant bending of the artery centred in this region at physiological blood pressure
Tikuna bilineata Needham & Murphy, comb. n.
Tikuna bilineata (Needham & Murphy) comb. n. Choroterpes bilineata Needham & Murphy 1924: 48; Traver 1947: 156. The genus Tikuna was established by Savage et al. (2005) and Choroterpes atramentum Traver, 1947, was designated as the type species. Following this publication, two species from South and Central America remain incorrectly placed in Choroterpes. The first, Choroterpes bilineata Needham & Murphy, 1924, originally described from female imagos and male subimagos from La Chorrera, Putumayo Dist., Peru (now a part of Colombia), also belongs to the genus Tikuna, requiring the new combination Tikuna bilineata. The holotype and paratypes (Cornell University) were studied by one of us (H.M. Savage); additional specimens deposited at the U. S. National Museum of Natural History, Florida A&M University, and the Zoologische Sammlung des Bayerischen Staates were recorded from Brazil, Ecuador, Surinam, and Venezuela. New records for T. bilineata follow: BRAZIL: 1 female imago, Pará State, Rio Cururu, about 100 km above Mission Cururu, at light, 25 –I– 1962, E.J. Fittkau. ECUADOR: Pastaza Prov., coll. J. Graded Hypercapnia-Calibrated BOLD: Beyond the Iso-metabolic Hypercapnic Assumption
Calibrated BOLD is a promising technique that overcomes the sensitivity of conventional fMRI to the cerebrovascular state; measuring either the basal level, or the task-induced response of cerebral metabolic rate of oxygen consumption (CMRO2). The calibrated BOLD method is susceptible to errors in the measurement of the calibration parameter M, the theoretical BOLD signal change that would occur if all deoxygenated hemoglobin were removed. The original and most popular method for measuring M uses hypercapnia (an increase in arterial CO2), making the assumption that it does not affect CMRO2. This assumption has since been challenged and recent studies have used a corrective term, based on literature values of a reduction in basal CMRO2 with hypercapnia. This is not ideal, as this value may vary across subjects and regions of the brain, and will depend on the level of hypercapnia achieved. Here we propose a new approach, using a graded hypercapnia design and the assumption that CMRO2 changes linearly with hypercapnia level, such that we can measure M without assuming prior knowledge of the scale of CMRO2 change. Through use of a graded hypercapnia gas challenge, we are able to remove the bias caused by a reduction in basal CMRO2 during hypercapnia, whilst simultaneously calculating the dose-wise CMRO2 change with hypercapnia. When compared with assuming no change in CMRO2, this approach resulted in significantly lower M values in both visual and motor cortices, arising from significant dose-dependent hypercapnia reductions in basal CMRO2 of 1.5±0.6%/mmHg (visual) and 1.8±0.7%/mmHg (motor), where mmHg is the unit change in end-tidal CO2 level. Variability in the basal CMRO2 response to hypercapnia, due to experimental differences and inter-subject variability, is accounted for in this approach, unlike previous correction approaches, which use literature values. By incorporating measurement of, and correction for, the reduction in basal CMRO2 during hypercapnia in the measurement of M values, application of our approach will correct for an overestimation in both CMRO2 task-response values and absolute CMRO2
Prenatal stress and limbic-prefrontal white matter microstructure in children aged 6-9 years: A preliminary diffusion tensor imaging study
Objectives. Maternal prenatal stress is associated with elevated risk of adverse behavioural outcomes in offspring. This association may involve developmental disruption to limbic-prefrontal white matter circuitry, of which the uncinate fasciculus is the major tract. One potential candidate for modulating brain development is maternal prenatal stress. We provide the first prospective study of prenatal stress and white matter microstructure in children. Methods. A total of 22 healthy children (mean age 7 years) of mothers recruited in pregnancy underwent diffusion tensor magnetic resonance imaging. We examined correlations between prenatal stressful life events and white matter microstructural organisation indices (fractional anisotropy (FA) and perpendicular diffusivity (Dperp)) of the uncinate fasciculus and a "control" tract. Results. Maternal prenatal stressful life events were correlated positively with right uncinate fasciculus FA, and negatively with right uncinate fasciculus Dperp in their child, with a similar trend with left uncinate fasciculus Dperp. Prenatal stress was not associated with control tract properties; sociodemographic/obstetric variables were not associated with FA/Dperp of either tract. Conclusions. Variation in maternal prenatal stress may be associated with differences in the development of white matter within brain networks underlying child social behaviour. © 2014 Informa Healthcare
