Neutrophil-to-lymphocyte ratio (NLR) distribution shows an advantage compared to C-reactive protein (CRP) for the early inflammation monitoring after total hip arthroplasty.
This study aimed to verify whether Neutrophil to lymphocyte ratio (NLR) distribution has an advantage when compared to C-reactive protein (CRP) distribution for the inflammation monitoring after total hip arthroplasty (THA).116 THA patients were retrospectively selected over a 2 years period. They all had available blood tests preoperatively and at postoperative days 2, 4 and 42. Median peak values were compared between CRP and NLR. The effect of demographics on CRP and NLR was tested. At days 4 and 42, 100% and 16.3% of patients had not reached normal CRP (< 10mg/L) while 56.8% and 6.8% of patients had not reached normal NLR (<5) respectively. No effect of demographics on NLR except for age. Older patients had higher NLR (p 0.037).NLR showed a quicker return to normal than CRP. Our results show that NLR seems to be a better marker to follow inflammation after THA than CRP
Management of Patella Dislocation in Say-Barber-Biesecker-Young-Simpson’s Syndrome: A Report of Two Cases
Say-Barber-Biesecker-Young-Simpson’s syndrome is one of the Ohdo-like syndromes. It is a very rare congenital condition that is commonly defined by its main clinical features that are blepharophimosis, ptosis, mental retardation, and delayed motor development. They are often associated with skeletal manifestations that are joint laxity, long thumbs and toes, and hypoplastic and/or dislocated patellae. To our knowledge, the available literature does not report any case where attention is drawn to management of skeletal aspect of this specific syndrome, especially surgically. We report 2 cases of SBBYS syndrome with patellar dislocation that we followed for 11 years. One case (with bilateral dislocation) was managed conservatively, and the other (with unilateral dislocation) underwent conservative and surgical treatment. Both had good functional outcome at follow-up. This experience shows that patellar abnormality in this condition can be efficiently addressed conservatively and/or surgically with satisfying results
Mono-articular idiopathic heterotopic ossification in a Coronavirus infected patient admitted in the intensive care unit.
Heterotopic ossification (HO) designates a bone tissue formation within an atypical anatomical location and is commonly diagnosed in patients whom have suffered major traumas. The following case report presents a non-traumatic source of HO. A causality is deduced between the HO formation and an important inflammatory reaction originating from a Coronavirus infection. In contrast to other studies not only is the source non traumatic but the HO formation is unilateral. In systemic inflammatory reactions vigilance towards HO should be enhanced especially in patients treated during prolonged periods of time in intensive care units (ICU)
Neutrophil-to-lymphocyte ratio (NLR) distribution shows an advantage compared to C-reactive protein (CRP) for the early inflammation monitoring after total hip arthroplasty
C-reactive protein (CRP) distribution has been used to monitor early inflammation after total hip arthroplasty (THA). Neutrophil to lymphocyte ratio (NLR) is a new and cheap inflammatory marker. This study aimed to verify whether Neutrophil to lymphocyte ratio (NLR) distribution has an advantage when compared to C-reactive protein (CRP) distribution for the inflammation monitoring after total hip arthroplasty (THA). 116 THA patients were retrospectively selected over a 2 years period.
Ovakva vrsta rada ohrabruje lični razvoj osobe, razvija autonomiju i ističe inicijativu. Kroz procesni rad, mladi se osnažuju da postaju samostalni, ravnopravni članovi društva, da razvijaju svoje socijalne veštine, te testiraju vrednosti i uverenja. This work follows the process of creating the play "Who is Antigone to me?" whereas the youth of drama schools from Paraćin and Ćuprija participated during 2020. The work process was defined after many years of work on the development of the youth and children's theater scene in these cities. During his personal and professional training, the author tried to transfer all the acquired experiences and knowledge to the participants of the youth and children's scenes with which he worked. The mentioned work process was written with the desire to help the next similar initiatives, mentors who work with young people and young people who will start some new processes in which the described work will help them. This work describes the process of working on the play through 20 workshops lasting 2 hours each. During the process, the participants were performers but also authors from the very beginning of the process, which, in addition to creating the play itself, aims to build a group and personal development of all individuals involved in the process. This kind of work enables young people to talk about important topics using the theater. Together with the workshop leaders, they participate in creating the content of the play and express their needs and dilemmas during discussions and exercises. This type of work encourages a person's personal development, develops autonomy, and emphasizes initiative. Through process work, young people are empowered to become independent, equal members of society, to develop their social skills, and to test values and beliefs

Kun sissiltä meni hermot: Paavo Rintalan Sissiluutnantin epäluotettava kertoja
Guerilla Goes Insane The Unreliable Narrator in Paavo Rintala’s Guerilla Lieutenant
The 1960’s in postwar Finland was an era of many literary controversies that sometimes have even been called “literary wars”. Rintala’s novel Guerilla Lieutenant (Sissiluutnantti, 1963) not just depicted struggles during the war between Finland and The Soviet Union, but also evoked literary “struggles”, as Pekka Tarkka has described (1966). Particularly the way Rintala’s novel depicted women at the front, that is the members of women’s auxiliary corps (Lotta Svärd Organization), upset the patriotic circles and many other readers, too. It was not noticed, however, that the conception of women in the novel belongs to the first person narrator and is, therefore, highly subjective. The narrative structure in Guerilla Lieutenant has not been understood by the readers nor by the scholars. Takala is a first person (homodiegetic) narrator, and the fictional world of the novel is delivered through him only. What is more, the text hints very strongly that Takala has gone insane; in other words, he is a text book example of a classical unreliable narrator. Therefore, the mimetic interpretation of Rintala’s novel is not a relevant one. Takala is a member of a special group (“guerillas”) and suffers some kind of posttraumatic stress disorder. Consequently, he has lost his ability to lead a normal life. Takala’s mental problems are most obvious in his sexually straightforward encounters with the women at the front. In these episodes Takala depicts the women as voluptuous monsters with only one wish: to copulate with the narrator. These episodes were the primary ones to cause the controversies. Nevertheless, there are hints in the narrator’s account, for instance the animal metaphors that disclose the narrator’s fantasies about being hunted by the dreadful “lotta” women.
Secondly, but equally importantly, the article focuses on the signification and the varied conceptions and theories about unreliable narration, notion that was originally created by Wayne C. Booth (1961). The article starts by introducing Ansgar Nünning’s (1997, 1999) new definition of “unreliable narrator” that copes well without the notorious but still persistent notion of the “implied author”, again created by Booth. According to Booth’s definition, the narrator is unreliable if he/she does not speak or act in accordance with the norms and values of the implied author. By contrast, Nünning rejects the obscure notion of the implied author and believes that unreliability would be more comprehensible if the narrator’s norms and values were compared to the norms and values of the textual whole instead of those of the implied author. Thirdly, the article comments on the recent polemics between Nünning and James Phelan (2005), who most eagerly defends the concept of implied author. Nevertheless, the problem with the notion of the implied author is not just its obscurity but the fact that it is always linked to the flesh-and-blood author. Thus, it seems to carry a conception that a novel has only one correct meaning intended by its author
