    The population's indicators predict a constant increase in the demand for health care and welfare related services, at least with reference to the most developed countries. In Italy the provision of the aforementioned services is devoluted to the local tier of government. This work focuses on the problem of financing the greater expenditure for elderly not self-sufficient people. The issues at stake concern on the one hand the analysis of the expenditure side and on the other hand the problem of a regional public sector financing. Two alternative policies are implemented in a general economic equilibrium framework. They regard two local taxes, respectively the regional additional taxation on income (IRPEF)and the regional taxation on production (IRAP). The study points out the efficiency and effectiveness effects of the manoeuvres, focusing both on the market distortions and on the regressive effects.     La fornitura di prestazioni sanitarie è caratterizzata da costo incerto. L’incertezza sul costo e sul risultato delle cure cambia la forma del contratto fra le parti, ma non è tuttavia l’unica causa di fallimento del mercato. Esiste infatti un problema di asimmetria informativa: il fornitore del servizio può osservare caratteristiche di costo del paziente e può utilizzare tale informazione a suo vantaggio in due direzioni diverse: scegliendo di trattare solo pazienti con determinati tipi di patologie oppure alterando la distribuzione di probabilità degli stati del mondo e scegliendo di curare con preferenza determinati tipi di pazienti. Questi due comportamenti, che nel nostro lavoro definiremo scrematura di mercato orizzontale e verticale, alterano la concorrenza fra gli ospedali e determinano importanti ripercussioni sul sistema. Lo scopo del presente lavoro è quello di modellare tali comportamenti all’interno di uno schema in cui l’acquirente (la Asl) acquista prestazioni da due ospedali ad un costo predefinito e i pazienti scelgono in quale struttura farsi ricoverare in un modello di concorrenza spaziale à la Hotelling. Il modello mostra che all’interno del sistema di rimborso italiano esistono le condizioni per la presenza di scrematura orizzontale e verticale e che il secondo tipo di scrematura ha effetti così perversi sull’intero sistema da poter mettere a rischio l’intera architettura del mercato sanitario interno

    The role of education in psychological response to adverse health shocks

    The prevalence of common mental disorders is on the rise: in the last decade mental disorders have become one of the major contributors to the global burden of disease and the leading cause of disability worldwide. While the association between depressive symptoms and physical health has been the subject of many studies, little is known about the potential pathways through which physical health affects mental health and how this relationship varies across different socioeconomic groups. This study aims at investigating on the role that a higher educational level may have not only in protecting people from depressive and anxiety symptoms but also on its role in mediating the relationship between mental and physical health shocks. For the scope of our analysis, we relied on hospital administrative records collected from Liguria, a north-western Italian region. We evaluate the impact of education in protecting individuals from mental disorders when they experienced an adverse health event, such as severe hospitalization or an onset of a chronic condition. Our results suggest that among those who suffer from severe physical health issues, highly educated individuals are less likely to experience depressive and anxiety symptoms compared to those with a lower level of education, even though, in presence of an adverse health shock, the protecting role of education slightly decreases
