Dermatomyositis Sine Myositis with Membranoproliferative Glomerulonephritis
Dermatomyositis (DM) is an autoimmune disease that is characterized by involvement of proximal musculature and skin. We report a 52-year-old woman with a 6-year history of dermatomyositis sine myositis, who developed lower extremity edema and proteinuria. Pathological examination of renal biopsy showed membranoproliferative glomerulonephritis. She received steroid, cyclophosphamide, and mycophenolate mofetil. Over the 9 to 10 months after the beginning of treatment, the proteinuria was improved
Systemic Lupus Erythematosus Presenting With Autoimmune Myelofibrosis in a Young Woman: A Case Report
ABSTRACT Systemic lupus erythematosus (SLE) is a multifaceted autoimmune disease characterized by different clinical manifestations and often hinders rapid diagnosis. Myelofibrosis, a rare blood disorder characterized by fibrosis replacement in bone marrow and impaired hematopoiesis, is a rare complication of SLE. Case presentation: This report describes a 34‐year‐old woman who presented with unexplained bicytopenia, initially thought to be attributed to primary myelofibrosis. Subsequent evaluation revealed additional systemic and laboratory findings suggestive of SLE. Her platelet (PLT) count was 7 × 103/μL, platelets increased to 30 × 103/μL after receiving intravenous immunoglobulin (IVIG) and to 75 × 103/μL after receiving Rituximab. The interaction between SLE and bone marrow pathology poses a diagnostic problem and highlights the necessity of comprehensive evaluation when young patients present with unexplained cytopenias. This case emphasizes the necessity of evaluating autoimmune causes such as SLE in patients with myelofibrosis and cytopenia to facilitate rapid diagnosis and appropriate care and thereby prevent consequences

Targeting the TLR4 signaling pathway by polyphenols: A novel therapeutic strategy for neuroinflammation
A wide array of cell signaling mediators and their interactions play vital roles in neuroinflammation associated with ischemia, brain trauma, developmental disorders and age-related neurodegeneration. Along with neurons, microglia and astrocytes are also affected by the inflammatory cascade by releasing pro-inflammatory cytokines, chemokines and reactive oxygen species. The release of pro-inflammatory mediators in response to neural dysfunction may be helpful, neutral or even deleterious to normal cellular survival. Moreover, the important role of NF-kappa B factors in the central nervous system (CNS) through toll-like receptor (TLR) activation has been well established. This review demonstrates recent findings regarding therapeutic aspects of polyphenolic compounds for the treatment of neuroinflammation, with the aim of regulating TLR4. Polyphenols including flavonoids, phenolic acids, phenolic alcohols, stilbenes and lignans, can target TLR4 signaling pathways in multiple ways. Toll interacting protein expression could be modulated by epigallocatechin-3-gallate. Resveratrol may also exert neuroprotective effects via the TLR4/NF-kappa B/STAT signaling cascade. Its role in activation of cascade via interfering with TLR4 oligomerization upon receptor stimulation has also been reported. Curcumin, another polyphenol, can suppress overexpression of inflammatory mediators via inhibiting the TLR4-MAPK/NF-kappa B pathway. It can also reduce neuronal apoptosis via a mechanism concerning the TLR4/MyD88/NF-kappa B signaling pathway in microglia/macrophages. Despite a symphony of in vivo and in vitro studies, many molecular and pharmacological aspects of neuroinflammation remain unclear. It is proposed that natural compounds targeting TLR4 may serve as important pharmacophores for the development of potent drugs for the treatment of neurological disorders

Exploring the Role of FDG PET CT Scan in Detecting High Grade Diffuse Large B-Cell Lymphoma
Introduction. Diffuse B-cell lymphoma (DLBCL) is the most common type of non-Hodgkin’s lymphoma. Currently, the standard method for evaluating patients at the initial stages of cancer diagnosis in Mashhad oncology centers involves computed tomography scans (CT scans), histopathological evaluation of tissue, bone marrow sampling, and cytogenetic studies, all of which are time-consuming and costly. It is worth mentioning that at present, the most recommended approach for determining lymphoma staging is the FDG-PET/CT scan, which combines labeled glucose with CT scan and offers a more accurate alternative. The objective of this study is to explore the potential of FDG-PET/CT scan as a tool for detecting high-grade lymphoma.Methods. In this study, patients with different types of DLBCL who underwent FDG-PET Scan for staging at Razavi Hospital, Mashhad, Iran between 2017 and 2021 were examined. The necessary clinical and paraclinical information, including the stage of the disease, the involved site at the time of diagnosis, the result of immunohistochemical examination, and the response to treatment were collected. FDG-PET Scan information including the extent of involvement and metabolic activity of the tumor before the start of treatment, pathological characteristics of the tumor, clinical behavior, and response to treatment in the form of response rate (RR), disease-free survival (DFS) and overall survival (OS) of the patients. Was also investigated. Aggressive histology in the present study was classified based on morphological characteristics and immunohistochemical staining, prognostic indicators, clinical behavior and response to treatment. Data were analyzed using SPSS software at a significance level of p<0.05.Results. Comparing the two groups of patients with high grade histology (n=12) and NOS (n=14), the results showed that SUV max values in patients with aggressive lymphoma were 27.5 ± 15.6 (median 25.6) and in patients with NOS lymphoma was 15.4 ± 9.8 (median 14.4) (p=0.01). The overall survival of patients in the aggressive group was 10 months and in the non-aggressive group was 24 months (p=0.002). Also, the cut — off -point of 21.1 for SUV max has a sensitivity of 66 % and a specificity of 72 % in differentiating aggressive from non-aggressive types.Conclusion. The results revealed that FDG PET CT Scan can provide valuable insights into differentiating lymphomas with a more aggressive type from their usual types, as those with heightened metabolic activity (SUVmax) are often indicative of aggressive behaviors
