The use of antiepileptic medication in adults with intellectual disabilities: a serious conundrum
Until the early 1990s, six major drugs existed in the treatment of epilepsy. By 2004, ten new antiepilepsy drugs (AEDs) were made available in the UK. Currently, the available number of AEDs has grown to over 20. There are a number of concerns posed by these medications, ranging from teratogenic concerns to negative impact upon cognitive function to worsening behavior, all which can have an effect upon quality of life of the person with epilepsy (PWE). While treatment guidelines have suggested that the newer AEDs tend to be better tolerated and affect cognitive functioning to a lesser degree than older AED options, the evidence base applied to people with intellectual disability (ID) has not been suitably explored. About 25 % of PWE also have an ID. This group has communication issues, premature mortality, more treatment resistance, difficulties in making informed choices, and greater risks of physical and mental health comorbidities. There is no specific prescribing guidance for this large, heterogeneous, and vulnerable group.
Identifying adverse effects to treatment is extremely challenging in PWE with an ID due to the presence of complex comorbidities and often profound communication barriers. This chapter looks to consolidate the current evidence and good practice needed to prescribe specifically in a PWE and ID. It takes into consideration and brings together a wide range of contextual good practice literature from both the ID research world, such as concepts of mental capacity, and the epilepsy world, such as the latest epilepsy medication and drug effects, thus helping straddle the diverse yet fast-developing nature of both the epilepsy and ID scientific worlds. To help present concepts and to highlight clinical complexity, ten case vignettes have been presented. It would be considered an essential read to any clinical practitioner who manages PWE and ID
A national survey of Rett syndrome : age, clinical characteristics, current abilities, and health
As part of a wider study to investigate the behavioral phenotype of a national sample of girls and women with Rett syndrome (RTT) in comparison to a well-chosen contrast group and its relationship to parental well-being, the development, clinical severity, current abilities and health of 91 participants were analyzed in relation to diagnostic, clinical and genetic mutation categories. Early truncating mutations or large deletions were associated with greater severity. Early regression was also associated with greater severity. All three were associated with lower current abilities. Epilepsy and weight, gastrointestinal and bowel problems were common co-morbidities. Participants with classic RTT had greater health problems than those with atypical RTT. A substantial minority of respondents reported fairly frequent signs of possible pain experienced by their relative with RTT. Overall, the study provides new data on the current abilities and general health of people with RTT and adds to the evidence that the severity of the condition and variation of subsequent disability, albeit generally within the profound range, may be related to gene mutation. The presence of certain co-morbidities represents a substantial ongoing need for better health. The experience of pain requires further investigation

A Future Vision For The Engineering Design Environment: A Future Sociotechnical Scenario
This paper presents a future vision for the working practices of designers within a manufacturing organisation. By its very nature the engineering design environment is highly distributed in nature and is characterised by a large number of information sources, which together with the designers forms a complex sociotechnical system. In discussions with designers it is apparent that changes are required to this environment to reflect the changes in the design process and organisations. We have developed a scenario that incorporates many of the features requested by designers and managers to improve the design environment. Race for the Kerr field
Roy P. Kerr has discovered his celebrated metric 45 years ago, yet the problem to find a generalization of the Schwarzschild metric for a rotating mass was faced much earlier. Lense and Thirring, Bach, Andress, Akeley, Lewis, van Stockum and others have tried to solve it or to find an approximative solution at least. In particular Achilles Papapetrou, from 1952 to 1961 in Berlin, was interested in an exact solution. He directed the author in the late autumn of 1959 to work on the problem. Microwave and RF photonic applications of integrated Kerr micro-combs
We report applications of integrated Kerr micro-combs to RF photonic systems and demonstrate a wide range of advanced functions including a microwave photonic intensity differentiator, filters and true time delays. The on-chip Kerr optical comb is CMOS-compatible and serves as a high-performance multi-wavelength source for transversal filter functions, thus greatly reducing the cost, size, and complexity of the system. © 2018 The Author(s).</p
Change over a 16 month period in the psychological well-being of mothers of girls and women with Rett syndrome
Purpose: There is an emerging research literature on the experiences of family members of girls and women with Rett syndrome (RTT), but a lack of longitudinal data. Methods. Fifty mothers whose daughters had RTT were surveyed 16—17 months after an earlier cross-sectional study. Measures completed at both time points focused on maternal positive and negative psychological well-being and their daughters’ behavioural and emotional problems and RTT behavioural phenotype severity. Results: Maternal stress, anxiety and depression demonstrated at least moderate levels of stability. Maternal positive perceptions were also moderately stable over 16—17 months. Longitudinal analyses suggested that their daughters’ behavioural and emotional problems rather than RTT behavioural phenotype severity predicted later maternal well-being. Conclusion: Mothers with RTT daughters experience chronic stress (persisting over time) but also ongoing positive perceptions. Practitioners should recognise positive perceptions, and also consider targeted behavioural parent training to reduce behaviour problems in individuals with RTT
Political Precaution, Pandemics and Protectionism
Despite strong scientific evidence and representations made by international scientific organizations, a considerable number of countries have imposed import bans on pork in response to the H1N1 pandemic. The imposition of these barriers is contrary to WTO rules. The motivation for the imposition of these barriers does not appear to have arisen from producers’ requests or consumer lobbying – political precaution provides the motivation. There appears to be little control over political precaution in the rules of international trade. Hence, the balance between the strong rules of trade desired by firms wishing to engage in international commerce and the need, at times, for politicians to respond to requests for protection may be changing in favour of more protection.
