    Effects of two probiotic strains in ovariectomized rats with and without experimental periodontitis: a microtomographic, histomorphometric, immunohistochemical, and immunoenzymatic study

    O aumento na produção de citocinas pró-inflamatórias é um fator importante na patogênese e progressão da doença periodontal e da osteoporose. A modulação da expressão de marcadores do tecido ósseo e de permeabilidade intestinal por meio de probióticos pode ser uma potencial alternativa de tratamento quando ambas as condições estão associadas. O propósito deste estudo foi avaliar o efeito de dois agentes probióticos, Bifidobacterium animalis subs. Lactis HN019 (HN019) e Lacticaseibacillus casei 01 (LC01) em ratas ovariectomizadas (OVX) com e sem doença periodontal (DP) induzida por ligadura. Foram utilizadas 66 ratas divididas em 6 grupos (n=11): C-OVX (controle), C-OVX-HN019 (controle, com probiótico HN019), C-OVX-LC01 (controle, com probiótico LC01), DP-OVX (doença periodontal) e DP-OVX-HN019 (doença periodontal, com HN019), DP-OVX-LC01 (doença periodontal, com LC01). No dia 0 do experimento, todos os animais foram submetidos à OVX. Após 8 semanas, os animais dos grupos HN019 e LC01 receberam administração sistêmica da cepa probiótica (B. lactis HN019 ou L. casei 01), durante 8 semanas. 14 semanas após o início do experimento, foi induzida DP nos animais dos grupos DP-OVX, DP-OVX-HN019 e DP-OVX-LC01. Estes animais também receberam administrações tópicas da terapia probiótica no dia de colocação das ligaduras, e aos 3 e 7 dias após. Os animais foram eutanasiados 16 semanas após o início do experimento. Nos tecidos periodontais foram analisados a perda óssea e de inserção conjuntiva por meio de micro-CT e histomorfometria, e marcadores de metabolismo ósseo (fosfatase ácida resistente ao tartarato - TRAP, osteoprotegerina - OPG e ligante do receptor do ativador do fator nuclear kappa B - RANKL), por imunohistoquímica. Nos tecidos intestinais foram analisadas as alturas de vilosidade e profundidades de cripta, por meio de histomorfometria, e a expressão de marcadores de permeabilidade intestinal (E-caderina e Claudina-1), por imunohistoquímica. Os níveis de estradiol foram avaliados por meio de ensaio imunoenzimático. Os dados obtidos foram submetidos à análise estatística (p < 0,05). O grupo DP-OVX apresentou maior perda de osso alveolar, maiores razões RANKL/OPG, maior expressão de TRAP e menores níveis de estradiol quando comparado ao grupo C-OVX (p < 0,05). Considerando os grupos com DP e tratados com probióticos, apenas o grupo DP-OVX-LC01 apresentou maior área e volume de osso nas análises microtomográficas e histométricas quando comparado ao grupo DP-OVX (p < 0,05). Na análise de parâmetros que refletem a qualidade da microarquitetura óssea (análise microtomográfica), os grupos DP-OVX-LC01 e DP-OVX-HN019 demonstraram menor espaçamento de trabéculas ósseas, maior número de trabéculas ósseas e maior densidade de conectividade óssea quando comparados ao grupo DP-OVX (p < 0,05). O grupo DP-OVX-LC01 apresentou menor porosidade óssea quando comparado ao grupo DP-OVX-HN019 (p < 0,05). Uma menor expressão de TRAP e da razão RANKL/OPG, bem como maiores expressão de E-caderina e Claudina-1 e maiores níveis de estradiol foram observados nos grupos DP-OVX-LC01 e DP-OVX-HN019 quando comparados ao grupo DP-OVX (p < 0,05). No ambiente intestinal, considerando os grupos com DP e tratados com probióticos, apenas o grupo DP-OVX-LC01 demonstrou maior altura de vilosidades quando comparado ao grupo DP-OVX (p < 0,05). Um efeito da terapia probiótica foi também observado nos grupos sem DP. Apenas o grupo C-OVX-LC01 apresentou maior quantidade e melhor qualidade ósseas (análises microtomográficas), bem como maior expressão de OPG nos tecidos periodontais e maior profundidade de criptas e altura de vilosidades intestinais quando comparado ao grupo C-OVX (p < 0,05). Ambos os grupos C-OVX-LC01 e C-OVX-HN019 apresentaram menores níveis de estradiol no soro e maior expressão de E-caderina e Claudina-1 nos tecidos intestinais quando comparados ao grupo C-OVX (p < 0,05). Dentro dos limites desse estudo, pode-se concluir que: i) a terapia probiótica aumentou os níveis de estradiol e reduziu a permeabilidade intestinal em ratas ovariectomizadas e seus efeitos no osso alveolar e na morfologia intestinal foram cepa dependentes; ii) a cepa probiótica Lacticaseibacillus casei 01 foi mais efetiva que a cepa probiótica Bifidobacterium animalis subs. Lactis HN019 para a redução da destruição de osso alveolar decorrente da doença periodontal experimental em ratas ovariectomizadas.The increased production of pro-inflammatory cytokines is an important factor in the pathogenesis and progression of periodontal disease and osteoporosis. Modulation of the expression of markers on bone tissue and intestinal permeability using probiotics may be a potential treatment alternative when both conditions are associated. The purpose of this study was to evaluate the effect of two probiotic agents, Bifidobacterium animalis subs. lactis HN019 (HN019) and Lacticaseibacillus casei 01 (LC01) in ovariectomized (OVX) rats with ligature-induced periodontitis. Sixty-six female rats were divided into 6 groups (n=11): C-OVX (control, without probiotic), C-OVX-HN019 (control, with probiotic B. lactis HN019), C-OVX-LC01 (control, with probiotic LC01), DP-OVX (periodontal disease) and DP-OVXHN019 (periodontal disease, with HN019), DP-OVX-LC01 (periodontal disease, with probiotic LC01). On day 0 of the experiment, all animals were submitted to OVX. After 8 weeks, the animals in the HN019 and LC01 groups received systemic administration of the probiotic strain (B. lactis HN019 or L. casei 01) for 8 weeks. 14 weeks after the beginning of the experiment, the periodontitis was induced in the animals of the DP-OVX, DP-OVX-HN019 and DP-OVX-LC01 groups, using ligatures around the lower first molars. These animals also received topical administrations of probiotic therapy on the day of placement of the ligatures, as well as at 3 and 7 days after. All animals were euthanized 16 weeks after the beginning of the experiment. In periodontal tissues, the bone loss and interproximal attachment loss was assessed using micro-CT, histomorphometric analysis, and determination of bone metabolism markers (tartrate-resistant acid phosphatase - TRAP, osteoprotegerin - OPG and nuclear factor kappa B activator receptor ligand - RANKL) by immunohistochemistry. In intestinal tissues, the villous height and crypt depth were measured using histomorphometry and the expression of intestinal permeability markers (E-cadherin and Claudin-1), using immunohistochemistry. Estradiol levels were evaluated using an immunoenzimatic assay. The data obtained were submitted to statistical analysis (p < 0.05). The DP-OVX group showed increased alveolar bone loss, higher RANKL/OPG ratios, increased TRAP expression, and lower estradiol levels compared to the C-OVX group (p < 0.05). Among the DP groups treated with probiotics, only the DP-OVX-LC01 group exhibited greater area and volume of bone in microtomographic and histometric analyses compared to the DP-OVX group (p < 0.05). In the analysis of parameters reflecting the quality of bone microarchitecture (microtomographic analysis), both the DP-OVX-LC01 and DP-OVX-HN019 groups demonstrated reduced trabecular spacing, increased number of trabeculae, and higher bone connectivity density compared to the DP-OVX group (p < 0.05). The DP-OVX-LC01 group exhibited lower bone porosity compared to the DP-OVX-HN019 group (p < 0.05). Lower TRAP expression and RANKL/OPG ratio, as well as higher expression of E-cadherin and Claudin-1 and higher estradiol levels, were observed in the DP-OVX-LC01 and DP-OVX-HN019 groups compared to the DP-OVX group (p < 0.05). In the intestinal environment, considering the DP groups treated with probiotics, only the DP-OVX-LC01 group demonstrated greater villous height compared to the DP-OVX group (p < 0.05). A probiotic therapy effect was also observed in the non-DP groups. Only the C-OVX-LC01 group showed greater quantity and better quality of bone (microtomographic analyses), as well as higher expression of OPG in periodontal tissues and greater depth of crypts and height of intestinal villi compared to the C-OVX group (p < 0.05). Both the C-OVXLC01 and C-OVX-HN019 groups exhibited lower serum estradiol levels and higher expression of Ecadherin and Claudin-1 in intestinal tissues compared to the C-OVX group (p < 0.05). Within the limits of this study, it can be concluded that: i) probiotic therapy increased estradiol levels and reduced intestinal permeability in ovariectomized rats, and its effects on alveolar bone and intestinal morphology were strain-dependent; ii) the probiotic strain Lacticaseibacillus casei 01 was more effective than the probiotic strain Bifidobacterium animalis subs. lactis HN019 in reducing alveolar bone destruction resulting from experimental periodontal disease in ovariectomized rats

    Effects of probiotic therapy on experimental periodontitis in rats with metabolic syndrome induced by a high-fat diet

    O conceito de modular a microbiota intestinal por meio do uso de bactérias específicas para melhorar o metabolismo do hospedeiro ganhou considerável interesse nos últimos anos. Alguns estudos pré-clínicos e clínicos demonstraram que a utilização de probióticos (PROB) pode ser uma promissora alternativa terapêutica/preventiva para a síndrome metabólica (SM) e para a doença periodontal (DP). Ambas as condições estão intimamente associadas e afetam milhares de pessoas em todo o mundo. Até o presente momento, nenhum estudo avaliou o impacto da terapia probiótica na associação SM-DP. O propósito deste estudo foi avaliar potenciais efeitos do agente probiótico Bifidobacterium animalis subsp. lactis HN019 (B. lactis HN019) no desenvolvimento da periodontite experimental associada ou não às co-morbidades da SM em ratos Wistar Hannover. 96 ratos machos Wistar Hannover foram aleatoriamente divididos de acordo com o protocolo alimentar recebido: ração hipercalórica rica em gordura para indução da SM e ração comum, (Grupos Controle - C). Os grupos foram ainda subdivididos de acordo com a presença ou não de DP (induzida por ligaduras) e administração ou não de terapia probiótica, constituindo as seguintes situações: i) Grupo C (ratos alimentados com dieta padrão, n = 12); ii) Grupo CP (ratos alimentados com dieta padrão que receberam administração de PROB - n = 12); iii) DP (ratos alimentados com dieta padrão e com DP - n = 12); iv); Grupo DPP (ratos alimentados com dieta padrão, com DP e que receberam administração de PROB - n = 12); v) Grupo SM (ratos alimentados com dieta hipercalórica rica em gordura, n = 12); vi) Grupo SMP (ratos alimentados com dieta hipercalórica rica em gordura e que receberam administração de PROB, n = 12); vii) Grupo SMDP (ratos alimentados com dieta hipercalórica rica em gordura e com DP, n = 12); viii) Grupo SMDPP (ratos alimentados com dieta hipercalórica rica em gordura e com DP, que receberam terapia PROB, n = 12). A SM foi induzida por meio do consumo de ração hipercalórica (60% gordura). O consumo da dieta hipercalórica foi iniciado 8 semanas antes da terapia probiótica e permaneceu até o dia da eutanásia dos animais. Na 14a. semana do estudo, a DP foi induzida por meio da colocação de ligaduras nos primeiros molares inferiores de cada animal durante 14 dias. A cepa probiótica B. lactis HN019 foi adicionada diariamente à água dos animais durante 8 semanas, na dosagem de 10¹º Unidades Formadoras de Colônias. Todos os animais foram submetidos à eutanásia 16 semanas após o início do experimento. Foram avaliados: Parâmetros metabólicos (glicemia, insulina, resistência à insulina, perfil lipídico, ácidos graxos e tolerância à glicose) e antropométricos (índice de massa corporal, circunferência abdominal e peso); Pressão arterial (aferida por método invasivo no momento da eutanásia); Níveis de citocinas pró e antiinflamatórias no tecido gengival por meio de imunoensaios enzimáticos (ELISA e Luminex®); Atividade de enzimas envolvidas no estresse oxidativo, catalase (CAT) e mieloperoxidase (MPO), por meio de testes ELISA; Nível ósseo alveolar (NOA), volume ósseo (VO) e porosidade óssea (PO) por meio de microtomografia computadorizada; Análise histológica descritiva das hemimandílulas; Análise microbiológica intestinal (qRT-PCR). Os dados obtidos foram submetidos à análise estatística (p<0,05). Os grupos SM, SMP, SMDP e SMDPP apresentaram aumento significativo de peso final, IMC e circunferência abdominal quando comparados ao grupo C (p<0,05). O grupo SM apresentou, ainda, aumento nos parâmetros de glicemia, HOMA-IR, colesterol total, triglicerídeos e ácidos graxos quando comparado ao grupo C (p<0,05). A terapia probiótica foi capaz de atenuar a dislipidemia presente nos animais com SM, uma vez que os níveis séricos de colesterol total do grupo SMDPP foram estatisticamente menores quando comparados àqueles do grupo SMDP. Da mesma forma, SMDPP e SMP apresentaram menores taxas de triglicerídeos quando comparados aos grupos SMDP e SM, respectivamente (p<0,05). Os dados da análise microtomográfica demonstraram o impacto de PROB na redução da severidade da DP e os efeitos da SM no agravamento da DP. Os grupos SMDP e SMDPP apresentaram NOA significativamente (p<0,05) maior que àqueles dos grupos DP e DPP, respectivamente. O grupo SMDP apresentou menor VO e maior PO quando comparado aos grupos DP (p<0,05). Os grupos SMDPP e DPP apresentaram menor NOA e menor PO quando comparados aos grupos SMDP e DP, respectivamente (p<0,05). A análise imunoenzimática demonstrou maiores níveis de IL-1&beta; e maior razão RANKL/OPG no grupo SMDP quando comparado ao grupo SMDPP (p<0,05). O grupo DPP apresentou menores níveis de TNF-&alpha; e IL-6 quando comparado ao grupo DP (p<0,05). Além disso, o grupo SMDP apresentou maior razão RANK-L/-OPG quando comparado ao grupo DP, assim como menor concentração de TGF-&beta; (p<0,05). Os animais dos grupos DP e DPP apresentaram níveis reduzidos de CAT quando comparados ao grupo C (p<0,05). Quando a DP foi associada à SM, houve redução significativa de CAT em comparação aos animais apenas com SM (p<0,05). Para MPO, uma tendência de redução foi observada nas comparações dos grupos DPP e DP, SMP e SM; e SMDPP e SMDP. os valores de MPO no presente estudo foram significativamente superiores nos grupos DP e SM quando comparados ao grupo C (p<0,05) A análise microbiológica revelou maior alteração na proporção de Firmicutes e Bacteroidetes para o grupo SMP quando comparado ao grupo C (p<0,05). O grupo SM apresentou maior redução de Lactobacilos e Bifidobacterium ao longo do experimento quando comparado ao grupo C (p<0,05). O uso de PROB promoveu maior redução de Bacteroidetes no grupo SMDPP quando comparo ao grupo SMDP (p<0,05). Dentro das limitações do presente estudo, é possível concluir que B. lactis HN019 reduziu a severidade da DP e modulou parâmetros imunoinflamatórios nos tecidos periodontais em ratos com ou sem SM. Além disso, B. lactis HN019 atenuou parâmetros de dislipidemia em ratos com SM.The concept of modulating the intestinal microbiota through the use of specific bacteria to improve the host\'s metabolism gained considerable interest in recent years. Some preclinical and clinical studies showed that the use of probiotics (PROB) can be a promising therapeutic/preventive alternative for metabolic syndrome (MS) and periodontal disease (PD). These conditions are closely linked and affect thousands of people around the world. To date, no study has assessed the impact of probiotic therapy on the MS-PD association. The aim of this study was to evaluate potential effects of the probiotic agent Bifidobacterium animalis subsp. lactis HN019 (B. lactis HN019) in the development of experimental periodontitis associated or not with the comorbidities of MS in Wistar Hannover rats. 96 male Wistar Hannover rats were randomly divided according to the received food protocol: high-fat high-calorie ration for the induction of MS and common ration (C - control groups). The groups were subdivided according to the presence or absence of PD (ligature-induced) and the administration or not of probiotic therapy: i) C group (rats fed with a standard diet, n = 12); ii) CP group (rats fed with a standard diet and that received PROB - n = 12); iii) PD (rats fed with a standard diet and with PD - n = 12); iv); PDP group (rats fed with a standard diet, with PD and that received PROB - n = 12); v) MS group (rats fed a high-fat high-calorie diet, n = 12); vi) MSP group (rats fed with a high-fat high-calorie diet and that received PROB administration, n = 12); vii) MSPD group (rats fed with a high-fat high-calorie diet and with PD, n = 12); viii) MSPDP group (rats fed with a high-fat high-calorie diet with PD and that received PROB therapy, n = 12). SM was induced through the consumption of high calorie ration (60% fat). The consumption of the high calorie diet started 8 weeks before the probiotic therapy and remained until the day the animals were euthanized. On the 14th week of the study, PD was induced by placing ligatures on the lower first molars of each animal for 14 days. The probiotic strain B. lactis HN019 was added daily to the water of the animals for 8 weeks, in the dosage of 10 ¹º Colony Forming Units. All animals were euthanized 16 weeks after the beginning of the experiment. Metabolic (blood glucose, insulin, insulin resistance, lipid profile, fatty acids, and glucose tolerance) and anthropometric parameters (body mass index (BMI), abdominal circumference and weight); Blood pressure (measured by invasive method at the time of euthanasia); Levels of pro and anti-inflammatory cytokines in the gingival tissue using enzymatic immunoassays (ELISA and Luminex®); Activity of enzymes involved in oxidative stress , catalase (CAT) and myeloperoxidase (MPO), using ELISA tests; Alveolar bone level (ABL), bone volume (BV) and bone porosity (BP) using computerized microtomography; Descriptive histological analysis of hemimandible; Intestinal microbiological analysis (qRT-PCR), were evaluated. The data were statistically analyzed (p<0.05). The MS, MSP, MSPD, and MSPDP groups showed a significant increase in final weight, BMI, and abdominal circumference when compared to group C (p<0.05). The MS group also showed an increase in the blood glucose, HOMA-IR, total cholesterol, triglycerides, and fatty acids parameters when compared to group C (p<0.05). Probiotic therapy was able to attenuate the dyslipidemia present in animals with MS since the serum levels of total cholesterol in the MSPDP group were statistically lower when compared to those in the MSPD group. Likewise, MSPDP and MSP showed lower triglyceride rates when compared to the MSPD and MS groups, respectively (p<0.05). The data from the microtomographic analysis demonstrated the impact of PROB in reducing the severity of PD and the effects of MS in worsening PD. The MSPD and MSPDP groups showed a significantly higher ABL (p<0.05) than those of the PD and PDP groups, respectively. The MSPD group showed lower BV and higher BP when compared to the PD groups (p<0.05). The MSPDP and PDP groups showed lower ABL and lower BP when compared to the MSPD and PD groups, respectively (p<0.05). The immunoenzymatic analysis showed higher levels of IL-1&beta; and a higher RANKL / OPG ratio in the MSPD group when compared to the MSPDP group (p<0.05). The PDP group showed lower levels of TNF-&alpha; and IL-6 when compared to the PD group (p<0.05). Besides, the MSPD group showed a higher RANK-L / OPG ratio when compared to the PD group, as well as a lower concentration of TGF-&beta; (p<0.05). The animals in the PD and PDP groups showed reduced levels of CAT when compared to group C (p<0.05). When PD was associated with MS, there was a significant reduction in CAT compared to animals with MS alone (p<0.05). For MPO, a downward trend was observed in the comparisons of the PDP and PD, MSP and MS groups; and MSPDP and MSPD. The MPO values in the present study were significantly higher in the PD and MS groups when compared to C group (p<0.05) The microbiological analysis revealed a greater change in the proportion of Firmicutes and Bacteroidetes for the SMP group when compared to group C (p<0.05). The SM group showed a greater reduction in Lactobacilos and Bifidobacterium throughout the experiment when compared to group C (p<0.05). The use of PROB promoted a greater reduction of Bacteroidetes in the MSPDP group when compared to the MSPD group (p<0.05). Considering the limitations of the present study, it is possible to conclude that B. lactis HN019 reduced the severity of PD and modulated immunoinflammatory parameters in the periodontal tissues in rats with or without MS. Furthermore B. lactis HN019 attenuated dyslipidemia in rats with MS

    Metabolic syndrome and periodontitis: a pathophysiological connections study and effects of probiotic microorganisms administered as a therapeutic and/or preventive strategy in rodents

    O propósito deste estudo foi avaliar diferentes mecanismos imunológicos, microbiológicos, genéticos e metabólicos envolvidos na correlação entre a perda óssea alveolar decorrente da doença periodontal (DP) experimental e a síndrome metabólica (SM) induzida por dieta hipercalórica rica em gorduras (HFD) em ratos, bem como os possíveis efeitos do agente probiótico Bifidobacterium animalis subsp. lactis HN019 como estratégia terapêutica e/ou preventiva para a associação DP-SM. Foram utilizados 96 ratos Wistar Hannover machos divididos em grupos com SM e sem SM. Esses dois grandes grupos foram aleatoriamente subdivididos em subgrupos com e sem DP induzida por ligaduras e de acordo com o protocolo alimentar recebido (grupos tratados com probiótico - P e grupos não tratados com probióticos), constituindo as seguintes situações: SM (com SM, n = 12) e C (sem SM, n = 12); SMP (com SM e tratado com P, n = 12) e CP (sem SM e tratado com P, n = 12); SMDP (Com SM e com DP, n = 12) e DP (sem SM e com DP, n = 12); SMDPP (com SM, com DP e tratado com P, n = 12) e DPP (sem SM, com DP e tratado com P, n = 12) . Os animais dos grupos SM receberam HFD durante 16 semanas para indução da SM. Nos grupos CP, DPP, SMP e SMDPP, o probiótico B. lactis HN019 foi adicionado diariamente na água dos animais na proporção de 1 x 101o UFC/mL. Todos os animais ingeriram a mesma quantidade de probiótico durante todo o período experimental. O consumo de probióticos não ultrapassou 8 semanas e foi iniciado na 8a semana experimental. Na 14a semana do estudo, a DP foi induzida por meio da colocação de ligaduras que permaneceram durante 14 dias nos primeiros molares inferiores dos animais dos grupos DP, DPP, SMDP e SMDPP. Todos os animais foram eutanasiados 16 semanas após o início do experimento. Foram avaliados: parâmetros metabólicos, antropométricos e pressão arterial invasiva; níveis de proteinúria; aspectos histopatológicos e histomorfometricos dos tecidos periodontais, hepáticos, intestinais e esplênico; perfil de adipocinas de tecido gorduroso branco (qRT-PCR) e perfil microbiológico das fezes (qRT-PCR). Os dados foram analisados estatisticamente (p < 0,05). A SM promoveu diversas alterações nos parâmetros imunológicos, microbiológicos, genéticos e metabólicos analisados. O grupo SM apresentou valores de peso, índice de massa corporal, circunferência abdominal, colesterol total, triglicerídeos, ácidos graxos, glicemia em jejum, HOMA-IR, pressão arterial média, expressão gênica de receptor de leptina (LEPR), nicotinamida fosforibosil transferase (NAMPT) e proteína ligante de ácidos graxos tipo 4 (FABP4), percentual de hepatócitos gordurosos (esteatose hepática) e perda de inserção conjuntiva periodontal significativamente maiores que aqueles do grupo C (p < 0,05). No ambiente intestinal, observou-se menor altura de vilosidades - AV (íleo) e de profundidade de criptas - PC (duodeno e íleo) no grupo SM quando comparado ao grupo C. A colocação de ligaduras foi capaz de reproduzir os eventos de perda óssea e de inserção conjuntiva nos tecidos periodontais. Os grupos SMDP e DP apresentaram maior perda de osso alveolar e de inserção conjuntiva do que os grupos SM e C, respectivamente (p < 0,05). Considerando os dados de perda óssea alveolar, correlações fortes foram encontradas com os seguintes parâmetros: esteatose hepática, expressão gênica de NAMPT, ácidos graxos, PC e proteinúria; correlações moderadas foram encontradas com a expressão gênica de LEPR; e correlações fracas foram encontradas com os níveis de Bacteroidetes nas fezes. O grupo SMDP, o qual apresentava diversos parâmetros imunológicos, microbiológicos, genéticos e metabólicos alterados, apresentou maior perda de osso alveolar do que o grupo DP (p < 0,05). A terapia probiótica foi capaz de reduzir nos grupos SMP e SMDPP as alterações de diversos parâmetros que se correlacionaram com a perda óssea alveolar. Diferenças significativas foram observadas entre os grupos SM e SMP em triglicerídeos, LDL, ácidos graxos, LEPR, NAMPT, AV (íleo), PC (íleo), razão AV/PC (íleo) e percentual de hepatócitos gordurosos. Quando se compara os grupos SMDP e SMDPP, diferenças significativas foram observadas em colesterol total, triglicerídeos, ácidos graxos, FABP4, LEPR, NAMPT, percentual de hepatócitos gordurosos, AV (duodeno, jejuno e íleo), razão AV/PC (duodeno, jejuno e íleo) e proporções de Firmicutes e Bacteroidetes nas fezes. O grupo SMDPP apresentou menor perda de osso alveolar e de inserção conjuntiva quando comparado ao grupo SMDP (p < 0,05). Dentro dos limites desse estudo, pode-se concluir que: i) alguns fatores microbiológicos, genéticos e metabólicos podem explicar a maior susceptibilidade ao desenvolvimento de DP com maior gravidade em animais com SM; ii) a terapia probiótica utilizando B. lactis HN019 em animais com SM e DP é capaz de modular diversos fatores microbiológicos, genéticos e metabólicos, promovendo impacto significativo na redução da gravidade dos danos ocasionados pela DP nos tecidos periodontais.The purpose of this study was to evaluate different immunological, microbiological, genetic and metabolic mechanisms involved in the correlation between alveolar bone loss resulting from experimental periodontal disease (PE) and metabolic syndrome (MS) induced by a high-fat hypercaloric diet (HFD) in rats, as well as the possible effects of the probiotic agent Bifidobacterium animalis subsp. lactis HN019 as a therapeutic and/or preventive strategy for the PE-MS association. Ninety-six male Wistar Hannover rats were divided into groups with and without MS. These two large groups were randomly subdivided into subgroups with and without ligature-induced PE and according to the food protocol received (groups treated with probiotic - P and groups not treated with probiotics), constituting the following situations: MS (with MS, n = 12) and C (without SM, n = 12); MSP (with SM and treated with P, n = 12) and P (without SM and treated with P, n = 12); MSPE (With SM and with PD, n = 12) and PE (without SM and with PD, n = 12); MSEPP (with MS, with PE and treated with P, n = 12) and EPP (without SM, with DP and treated with P, n = 12). The animals in the MS groups received HFD for 16 weeks to induce MS. In the P, EPP, MSP and MSEPP groups, the probiotic B. lactis HN019 was added daily to the animals\' water at a rate of 1 x 101o CFU/mL. All animals ingested the same amount of probiotic throughout the experimental period. The consumption of probiotics did not exceed 8 weeks and started in the 8th experimental week. In the 14th week of the study, PE was induced by placing ligatures that remained for 14 days on the lower first molars of the animals in the PE, EPP, MSEP and MSEPP groups. All animals were euthanized 16 weeks after the start of the experiment. The following were evaluated: metabolic, anthropometric and invasive blood pressure parameters; proteinuria levels; histopathological and histomorphometric aspects of periodontal, hepatic, intestinal and splenic tissues; white fat tissue adipokine profile (qRT-PCR) and stool microbiological profile (qRT-PCR). Data were statistically analyzed (p < 0.05). MS promoted several changes in the immunological, microbiological, genetic and metabolic parameters analyzed. The MS group presented values of weight, body mass index, waist circumference, total cholesterol, triglycerides, fatty acids, fasting glucose, HOMA-IR, mean arterial pressure, leptin receptor (LEPR) gene expression, nicotinamide phosphoribosyl transferase (NAMPT) and fatty acid binding protein type 4 (FABP4), percentage of fatty hepatocytes (hepatic steatosis) and loss of periodontal connective attachment were significantly higher than those in group C (p < 0.05). In the intestinal environment, lower villus height - AH (ileum) and crypt depth - CD (duodenum and ileum) were observed in the MS group when compared to the group. The placement of ligatures was able to reproduce the events of bone loss and connective attachment in periodontal tissues. The MSEP and PE groups showed greater loss of alveolar bone and conjunctival attachment than the MS and C groups, respectively (p < 0.05). Considering the alveolar bone loss data, strong correlations were found with the following parameters: hepatic steatosis, NAMPT gene expression, fatty acids, P and proteinuria; moderate correlations were found with LEPR gene expression; and weak correlations were found with levels of Bacteroidetes in feces. The MSEP group, which had several altered immunological, microbiological, genetic and metabolic parameters, showed greater alveolar bone loss than the PE group (p < 0.05). Probiotic therapy was able to reduce changes in several parameters that correlated with alveolar bone loss in the MSP and MSEPP groups. Significant differences were observed between the SM and SMP groups in triglycerides, LDL, fatty acids, LEPR, NAMPT, AH (ileum), CD (ileum), AH/CD ratio (ileum) and percentage of fatty hepatocytes. When comparing the SMDP and SMDPP groups, significant differences were observed in total cholesterol, triglycerides, fatty acids, FABP4, LEPR, NAMPT, percentage of fatty hepatocytes, AH (duodenum, jejunum and ileum), AH/CD ratio (duodenum, jejunum and ileum) and proportions of Firmicutes and Bacteroidetes in feces. The SMDPP group showed less loss of alveolar bone and conjunctival attachment when compared to the MSEP group (p < 0.05). Within the limits of this study, it can be concluded that: i) some microbiological, genetic and metabolic factors can explain the greater susceptibility to the development of more severe PE in animals with MS; ii) probiotic therapy using B. lactis HN019 in animals with MetS and PE is able to modulate several microbiological, genetic and metabolic factors, promoting a significant impact on reducing the severity of damage caused by PE in periodontal tissues

    Horizontal bone augmentation of the alveolar ridge with bovine bone graft associated with autogenous bone or blood concentrates: a controlled and randomized clinical study

    O objetivo deste estudo clínico controlado e aleatorizado foi avaliar os efeitos do enxerto ósseo bovino desproteinizado (EOBD) associado ao enxerto ósseo autógeno particulado (EOAP) ou à Fibrina Rica em Plaquetas e Leucócitos (L-PRF) + Fibrina Rica em Plaquetas injetável (i-PRF) para aumento horizontal de rebordo alveolar em maxilas atróficas. Vinte e oito pacientes, apresentando regiões edêntulas na maxila e com necessidade de aumento ósseo horizontal prévio a instalação de implantes dentários, foram incluídos neste estudo e divididos, aleatoriamente, em dois grupos de acordo com o tratamento de regeneração óssea guiada (ROG) proposto: Grupo A (GA) - EOBD + EOAP ou Grupo B (GB) - EOBD + L-PRF + iPRF. No grupo GA, o EAOP foi coletado na região de sínfise mandibular e misturado na proporção de 1:1 com o EOBP. No grupo GB, foram realizadas coletas de sangue venoso, as quais foram centrifugadas para obtenção de L-PRF (~700g - RCFmax / 12 min) e i-PRF (~700g - RCFmax / 3 min). Para cada 0,5g de EOBD, foram adicionadas duas membranas de L-PRF + i-PRF. Em ambos os grupos, a região enxertada foi protegida por membrana de colágeno suíno estabilizada com auxílio de tachinhas de titânio. Foram realizadas tomografias computadorizadas de feixe cônico (CBCTs) no pré-operatório (T0), imediatamente após o procedimento cirúrgico da ROG (T1) e após 260 dias da ROG (T2) para mensuração das alterações lineares e volumétricas do rebordo alveolar. No momento da instalação dos implantes dentários, foram realizadas mensurações do torque de inserção (N.cm) e quociente de estabilidade (ISQ). Biópsias ósseas da área enxertada foram também obtidas para análises histomorfométrica e microtomográfica (micro-CT). Todos os implantes foram submetidos à cicatrização submersa e reabertos após 180 dias, quando, então, novas medidas de ISQ foram obtidas. Não foram observadas diferenças estatisticamente significativas entre os grupos GA e GB para: medidas volumétricas (redução de volume da área enxertada) e lineares (mudanças na altura óssea, espessura de tecidos moles e espessura óssea) obtidas na análise tomográfica, valores do torque de inserção do implante, médias de ISQ e porcentagem de tecido ósseo neoformado na análise histomorfométrica. A análise com micro-CT revelou um aumento significativo de valores do grupo GB em comparação ao grupo GA (p<0,05) para volume ósseo (BV/TV), densidade de conectividade (Conn.Dn) e número de trabéculas (Tb.N). Não foram observadas diferenças significativas entre os grupos para os valores de espessura de trabéculas (Tb.Th) e porosidade óssea (Po.Tot). Quando comparado ao grupo GB, foi observado um aumento significativo (p<0,05) no grupo GA para os valores de biomaterial residual e separação trabecular (Tb.Sp). A análise histométrica demonstrou maior área de biomaterial residual e tecido conjuntivo no grupo GA quando comparado ao grupo GB, com diferença significativa para área de biomaterial residual (p<0,0001). A taxa de sobrevivência dos implantes dentários foi de 97,61% para GA e 100% para GB. Os resultados obtidos indicam que os tratamentos realizados em GA e GB foram eficazes para aumento ósseo horizontal em áreas de atrofia maxilar. Na associação com xenoenxertos, o uso de concentrados sanguíneos pode ser uma alternativa viável ao enxerto de osso autógeno para o aumento ósseo horizontal em região de maxila atrófica, sendo capaz de promover maiores benefícios na microarquitetura do tecido ósseo neoformado.The aim of this randomized controlled clinical study was to evaluate the effects of deproteinized bovine bone mineral (DBBM) combined with particulate autogenous bone graft (PABG) or with Leukocyte-Platelet Rich Fibrin (L-PRF) + injectable Platelet Rich Fibrin (i-PRF) for horizontal ridge augmentation in atrophic maxilla. Twenty-eight patients, presenting edentulous regions in the maxilla and requiring horizontal bone augmentation prior to dental implant placement, were included in this study and randomly divided into two groups according to the proposed guided bone regeneration (GBR) treatment: Group A (GA) - DBBG + PABG or Group B (GB) - DBBG + L-PRF + i-PRF. In GA, PABG was collected from the mandibular symphysis and mixed in a 1:1 ratio with DBBG. In GB, venous blood samples were collected and centrifuged to obtain L-PRF (~700g - RCFmax / 12 min) and i-PRF (~700g - RCFmax / 3 min). For each 0.5g of DBBG, two membranes of L-PRF + i-PRF were added. In both groups, the grafted region was protected by a collagen membrane stabilized with the aid of titanium tacks. Cone beam computed tomography (CBCT) scans were performed preoperatively (T0), immediately after the GBR surgical procedure (T1), and 260 days post-GBR (T2) to measure linear and volumetric changes in the alveolar ridge. At the time of dental implant placement, insertion torque (N.cm) and implant stability quotient (ISQ) measurements were taken. Bone biopsies from the grafted area were also obtained for histomorphometric and micro-CT analyses. All implants underwent submerged healing and were reopened after 180 days, when new ISQ measurements were obtained. No statistically significant differences were observed between GA and GB for volumetric measurements (graft volume reduction) and linear measurements (changes in bone height, soft tissue thickness, and bone thickness) obtained in the tomographic analysis, insertion torque values, mean ISQ, and percentage of newly formed bone tissue in the histomorphometric analysis. The micro-CT analysis revealed a significant increase in values for GB compared to GA (p<0.05) for bone volume (BV/TV), connectivity density (Conn.Dn), and trabecular number (Tb.N). No significant differences were observed between the groups for trabecular thickness (Tb.Th) and bone porosity (Po.Tot). Compared to GB, a significant increase (p<0.05) was observed in GA for residual biomaterial values and trabecular separation (Tb.Sp). The histometric analysis showed a larger area of residual biomaterial and connective tissue in group GA compared to group GB, with a significant difference in the area of residual biomaterial (p<0.0001). The survival rate of dental implants was 97.61% for GA and 100% for GB. The results indicate that the treatments performed in GA and GB were effective for horizontal bone augmentation in areas of maxillary atrophy. In association with xenografts, the use of blood concentrates can be a viable alternative to autogenous bone grafts for horizontal bone augmentation in the region of the atrophic maxilla, promoting greater benefits in the microarchitecture of the newly formed bone tissue

    Effects of platelet aggregates produced by three centrifugation protocols on bone neoformation in defects created in rat calvaria

    Este estudo avaliou o potencial regenerativo de agregados plaquetários obtidos por 3 protocolos diferentes produzidos em equipamentos de centrifugação horizontal (H-PRF) e de ângulo fixo (L-PRF e A-PRF) na neoformação óssea em defeitos de tamanho crítico (DTCs) em calvária de ratos. Previamente à execução do experimento principal, animais adicionais foram utilizados para validação dos protocolos de centrifugação utilizados neste estudo por meio de análise por pipetagem sequencial. Para o experimento principal, 32 ratos foram divididos em quatro grupos: C, L-PRF, A-PRF e H-PRF. Após coleta de sangue dos animais por punção cardíaca, agregados plaquetários foram preparados por protocolos de centrifugação vertical (L-PRF e A-PRF, preparados em centrífuga de ângulo fixo [33° de angulação do rotor, 55mm de raio na altura do coágulo, 86mm de raio máximo]) e horizontal (H-PRF, preparado em centrífuga de rotor basculante). Para o preparo da L-PRF, o sangue foi centrifugado a uma Força Centrífuga Relativa máxima (RCFmax) de 701g (Força G) por 12 minutos; para o preparo da A-PRF, o sangue foi centrifugado a uma RCFmax de 216g, por 14 minutos; o preparo da H-PRF foi realizado pela centrifugação do sangue a uma RCFmax de 700g por 8 minutos. Foram criados DTCs de 5mm de diâmetro nas calvárias dos animais por osteotomia com broca trefina em baixa rotação e irrigação abundante com solução salina estéril. Marcações de amálgama foram feitas para auxiliar a identificação das margens originais do defeito durante o processamento laboratorial. Os defeitos dos grupos C, L-PRF, A-PRF e H-PRF foram preenchidos com coágulo sanguíneo, L-PRF, A-PRF e H-PRF, respectivamente. Aos 14 e 30 dias foram realizadas injeções dos fluorocromos calceína (CA) e alizarina (AL), respectivamente. A eutanásia ocorreu aos 35 dias pós-operatórios. Análises microtomográficas, por microscopia de fluorescência e histomorfométricas foram realizadas. Os dados foram estatisticamente analisados (ANOVA, Tukey, p<0,05). Os grupos L-PRF, A-PRF e H-PRF apresentaram maiores valores de volume ósseo percentual (BV/TV), área de osso neoformado (AON) e maior precipitação de CA e AL do que o grupo C (p<0.05). O grupo H-PRF apresentou maiores valores de BV/TV, número de trabéculas ósseas (Tb.N), AON e maior precipitação de AL do que os grupos A-PRF e L-PRF (p<0,05). Pode-se concluir que: i) os agregados plaquetários L-PRF, A-PRF e H-PRF potencializam a neoformação óssea em DTCs em calvária de ratos; ii) o protocolo de centrifugação horizontal promoveu maior neoformação óssea no tratamento de DTCs criados em calvária de ratos.This study evaluated the regenerative potential of platelet aggregates obtained by 3 different protocols produced in horizontal (H-PRF) and fixed-angle (L-PRF and A-PRF) centrifuge in bone neoformation in critical size defects (CSDs) in rat calvaria. Prior to the execution of the main experiment, additional animals were used to validate the centrifugation protocols used in this study by sequential pipetting analysis. For the main experiment, 32 rats were divided into four groups: C, L-PRF, A-PRF and H-PRF. After collecting blood from the animals by cardiac puncture, platelet aggregates were prepared by vertical centrifugation protocols (L-PRF and A-PRF, prepared in a fixed-angle centrifuge [33° rotor angle, 55mm radius at the clot height, 86mm maximum radius]) and horizontal (H-PRF, prepared in a swing-out rotor centrifuge). To prepare the L-PRF, the blood was centrifuged at a Relative Centrifugal Force (RCF)-max of 701g (G-force) for 12 minutes; to prepare the A-PRF, the blood was centrifuged at 216g RCFmax for 14 minutes; H-PRF preparation was performed by centrifuging the blood at 700g RCFmax for 8 minutes. 5mm diameter DTCs were created in the animals\' calvaria by osteotomy with a trephine drill at low rotation and abundant irrigation with sterile saline solution. Amalgam markings were made to help identify the original margins of the defect during laboratory processing. Defects in groups C, L-PRF, A-PRF and H-PRF were filled with blood clot, L-PRF, A-PRF and H-PRF, respectively. At 14 and 30 days, injections of the fluorochromes calcein (CA) and alizarin (AL) were performed, respectively. Euthanasia took place 35 days after surgery. Microtomographic, fluorescence microscopy and histomorphometric analyzes were performed. Data were statistically analyzed (ANOVA, Tukey, p<0.05). The L-PRF, A-PRF and H-PRF groups showed higher values of percent bone volume (BV/TV), newly formed bone area (NFBA) and greater precipitation of CA and AL than the C group (p<0.05). The H-PRF group showed higher values of BV/TV, number of trabeculae (Tb.N), NFBA and higher precipitation of AL than the A-PRF and L-PRF groups (p<0.05). It can be concluded that: i) platelet aggregates L-PRF, A-PRF and H-PRF potentiate bone neoformation in DTCs in rat calvaria; ii) the horizontal centrifugation protocol promoted greater bone neoformation in the treatment of CSDs created in rat calvaria

    The effects of probiotic administration on the development of periodontal disease and adverse effects of pregnancy in pregnant mice

    O objetivo desta pesquisa foi avaliar o impacto da administração de probióticos (PROB) pré-natal na expressão de marcadores epigenéticos em camundongos prenhes e periodontite experimental (DP). Foram utilizados 48 camundongos prenhes, divididas em 4 grupos (n=12): C (animais sem DP e não tratados com probióticos); DP (animais com DP e não tratados com probióticos); CP (animais sem DP e tratados com probióticos) e PDP (animais com DP e tratados com probióticos). Após confirmação da prenhez, os animais dos grupos DP e PDP receberam gavagens com Porphyromonas gingivalis w83 100&micro; L 109 unidades formadoras de colônias (UFC)/mL durante 15 dias. Os Grupos CP e PDP, também receberam Bifidobacterium animalis subsp. lactis HN019 na concentração 109 UFC/mL até o dia da eutanásia, administrados na água durante 29 dias, com início 14 dias antes do acasalamento dos animais. Os animais dos grupos C, CP, DP e PDP foram submetidos à eutanásia no 30º dia do experimento. Os tecidos coletados foram utilizados para avaliação dos seguintes parâmetros: 1) perda óssea alveolar na região de 2o molar superior por meio de análise com microtomografia computadorizada por raio-x; 2) perfil imunoinflamatório (IL-6, TNF-&alpha;, INF-y, IL-10, IL-1&beta;, IL-8) presente nos tecidos placentários e tecidos periodontais por meio de ensaios imunoenzimáticos (luminex). Os dados obtidos serão submetidos à análise estatística (p < 0,05). Os grupos DP e PDP apresentaram peso materno menores quando comparados aos grupos C e CP, respectivamente. O tamanho fetal foi menor no grupo DP quando comparado aos grupos que receberam suplementação probiótica. Comparando os grupos DP e PDP entre si, o grupo PDP apresentou maior volume ósseo, número de trabéculas com menor separação entre si e diminuição na porosidade óssea. O grupo PDP apresentou menores níveis de IL-1&beta; quando comparado ao grupo DP. Observa-se maior papila interdentária no grupo DP19 bem como maior zona juncional na arquitetura do tecido placentário. A administração do probiótico parece ter favorecido o crescimento de bactérias comensais e suprimido o crescimento de periodonto-patógenos, diminuindo assim o desenvolvimento da doença periodontal e maiores danos aos tecidos.The aim of this research was to evaluate the impact of prenatal probiotic administration (PROB) on the expression of epigenetic markers in pregnant mice and experimental periodontitis (DP). Forty-eight pregnant mice were used, divided into 4 groups (n=12): C (animals without DP and untreated with probiotics); DP (animals with DP and untreated with probiotics); CP (animals without DP and treated with probiotics); and PDP (animals with DP and treated with probiotics). After confirming pregnancy, animals from DP and PDP groups received gavages with Porphyromonas gingivalis w83 100&micro;L 109 colony-forming units (CFU)/mL for 15 days. Groups CP and PDP also received Bifidobacterium animalis subsp. lactis HN019 at a concentration of 109 CFU/mL until euthanasia day, administered in the water for 29 days, starting 14 days before animal mating. Animals from groups C, CP, DP, and PDP were euthanized on the 30th day of the experiment. The collected tissues were used to evaluate the following parameters: 1) alveolar bone loss in the upper second molar region through analysis with X-ray microcomputed tomography; 2) immunoinflammatory profile (IL-6, TNF-&alpha;, INF-y, IL-10, IL-1&beta;, IL-8) present in placental tissues and periodontal tissues through enzyme-linked immunosorbent assays (Luminex). The obtained data will be subjected to statistical analysis (p < 0.05). DP and PDP groups showed lower maternal weight compared to C and CP groups, respectively. Fetal size was smaller in the DP group compared to groups receiving probiotic supplementation. Comparing DP and PDP groups, the PDP group showed greater bone volume, number of trabeculae with less separation between them, and decreased bone porosity. The PDP group showed lower levels of IL-1&beta; compared to the DP group. A greater interdental papilla was observed in the DP19 group as well as a larger junctional zone in the placental tissue architecture. Probiotic administration appears to have favored the growth of commensal bacteria and suppressed the growth of periodontal pathogens, thus reducing the development of periodontal disease and tissue damage

    O objetivo geral deste estudo foi analisar se a periodontite, durante a gestação, associada ou não à suplementação pré-natal e perinatal com probiótico (PROB), afeta a susceptibilidade da prole à perda óssea alveolar quando desafiada ou não com Porphyromonas gingivalis w83 na vida adulta. Foram utilizados 64 camundongos prenhes divididos em 4 grupos (n=16): CM (animais sem DP e não tratados com probióticos); DPM (animais com DP e não tratados com probióticos); CMP (animais sem DP e tratados com probióticos) e DPMP (animais com DP e tratados com probióticos). A prole concebida por cada um destes grupos foi dividida em 2 subgrupos: P1 (sem indução de DP; n=24) e P2 (com indução de DP; n=24). Após o acasalamento e confirmação da prenhez, os animais dos grupos DPM e DPMP receberam gavagens com 5x109 unidades formadoras de colônias (UFC)/mL de Porphyromonas gingivalis w83 durante 37 dias. Nos animais da prole (subgrupos P2), a DP foi induzida seguindo o mesmo protocolo adotado para os grupos DPM e DPMP, sendo as gavagens realizadas durante 21 dias. Os animais dos grupos CMP e DPMP receberam 1 x 109 UFC/mL de Bifidobacterium animalis subsp. lactis HN019 administrados na água durante 56 dias, com início 14 dias antes do acasalamento. No 40º dia experimental (19º dia de prenhez), 8 animais de cada um dos grupos maternos CM, CMP, DPM e DPMP foram submetidos à eutanásia para coleta de líquido amniótico e tecido placentário. Os demais animais dos grupos CM, CMP, DPM e DPMP foram submetidos à eutanásia no 63o . dia do experimento. Os animais dos subgrupos P1 e P2 foram submetidos à eutanásia 63 dias após o nascimento. Foram avaliados: 1) perfil imunoinflamatório (IL-6, TNF-&alpha;, INF-&gamma;, IL-10, IL-1&beta;, IL-8) presente nos tecidos periodontais e líquido amniótico por meio de ensaios imunoenzimáticos (Luminex); 2) composição microbiana intestinal e bucal por meio de sequenciamento metagenômico do gene 16S; 3) expressão imunofluorescente de DNA metiltransferase 3b (DNMT3b), acetilação de histonas 3 e 4 (AcH3 e AcH4), e-caderina, molécula de adesão juncional (JAM) e receptores do tipo Toll (TLR-2 e TLR-4) nos tecidos intestinais, placentários e periodontais; 4) severidade da inflamação presente na região interproximal do 2º. molar superior por meio de análises histomorfométricas; 5) perda óssea alveolar na região de 2o. molar superior por meio de microtomografia computadorizada por raio-x (Micro-CT). Os dados obtidos foram submetidos à análise estatística (p0,05). Uma menor expressão de TLR-2 e TLR-4 foi observada no grupo DPMP quando comparado ao grupo DPM (p0.05). At 63 experimental days, considering the maternal groups, it was observed that the DPMP group showed less alveolar and conjunctival bone loss when compared to the DPM group (p<0.05), as well as higher expressions of e-cadherin and JAM and lower levels of IL-1&beta;, IL-17, TNF-&alpha;, DNMT3b and H3Ac in periodontal tissues (p<0.05). In intestinal tissues, the DPMP group, when compared to the DPM group, showed greater villus height, lower expression of H3Ac and H4Ac and higher expressions of e-cadherin and JAM (p<0.05). In the P1 offspring groups, no significant differences were observed in bone volume and conjunctival attachment level between all experimental groups. However, the DPM-P1 group presented, in periodontal tissues, higher expression of DNMT3b, H3Ac and TLR-2, as well as lower expression of JAM when compared to the other P1 groups (p<0.05). In intestinal tissues, the DPM-P1 group showed higher expression of DNMT3b, H3Ac and TLR-2, as well as lower expression of JAM when compared to the CM-P1 and CMP-P1 groups (p<0.05). In the P2 offspring groups, the DPMP-P2 group presented greater bone volume and less loss of conjunctival attachment when compared to the DPM-P2 group (p<0.05). The DPMP-P2 group showed lower expression of DNMT3b and H3Ac in periodontal and intestinal tissues, as well as greater height of intestinal villi and greater expression of JAM in the small intestine when compared to the DPM-P2 group (p<0.05). The main results obtained in the analysis of the oral and intestinal microbiome were: i) Maternal groups - differences (p<0.05) were observed in the comparisons CM vs DPM (beta diversity - intestinal), CM vs CMP (beta diversity - oral and intestinal ) and DPM vs DPMP (beta - intestinal diversity); ii) P1 offspring groups - differences were observed in the comparisons DPM-P1 vs CM-P1 (beta - intestinal diversity), CM-P1 vs CMP-P1 (alpha and beta - intestinal diversity) and DPM-P1 vs DPMP-P1 ( alpha and beta diversity - intestinal); iii) P2 offspring groups - differences were observed in the comparisons CM-P2 and DPM-P2 (beta diversity - oral and intestinal), CM-P2 vs CMP-P2 (beta diversity - oral and intestinal) and DPM-P2 vs DPMP- P2 (beta diversity - intestinal). It can be concluded that i) the challenge with P. gingivalis w83 during pregnancy promoted microbiological, immunological, tissue barrier and epigenetic changes in the maternal and offspring intestinal and periodontal environments; ii) offspring from mothers challenged with Porphyromonas gingivalis w83 during pregnancy were more susceptible to loss of periodontal tissues when challenged in adulthood with Porphyromonas gingivalis w83; iii) prenatal and perinatal probiotic supplementation modulated microbiological, immunological, tissue barrier and epigenetic patterns in maternal and offspring periodontal and intestinal tissues, as well as reduced offspring susceptibility to periodontal destruction when challenged with Porphyromonas gingivalis w83 in adulthood

    Comparison of the effects of platelet aggregates produced by high and low-speed centrifugation protocols on the healing of critical-size defects in rat calvaria: a microtomographic and histomorphometric study

    O presente estudo avaliou a cicatrização de defeitos de tamanho crítico (DTC) criados em calvária de ratos e tratados com agregados plaquetários produzidos por protocolos de alta velocidade (Fibrina Rica em Plaquetas e Leucócitos - L-PRF) e baixa velocidade (Fibrina Rica em Plaquetas e Leucócitos Avançada - A-PRF) de centrifugação. Vinte e quatro ratos foram divididos em 3 grupos: Controle (C), L-PRF e A-PRF. DTC de 5 mm de diâmetro foram criados na calvária dos animais. Os defeitos do Grupo C foram preenchidos com coágulo sanguíneo somente. Os defeitos dos grupos L-PRF e A-PRF foram preenchidos com 0,2 ml de L-PRF e A-PRF, respectivamente. Todos os animais foram submetidos à eutanásia aos 35 dias pós-operatórios. Análises histomorfométricas e microtomográficas foram realizadas. Os dados foram estatisticamente analisados (Anova, Tukey, p<0,05). Os grupos L-PRF e A-PRF apresentaram valores de Volume Ósseo e de Área de Osso Neoformado significativamente maiores que àqueles do grupo C, bem como menores valores de Porosidade Óssea (p<0,05). Não foram observadas diferenças significativas entre os grupos A-PRF e L-PRF para esses parâmetros analisados. Pode-se concluir que i) L-PRF e A-PRF potencializaram a cicatrização de DTC em calvária de ratos; ii) protocolos de alta e baixa velocidade de centrifugação não produziram matrizes de agregados plaquetários com impactos biológicos diferentes na quantidade de neoformação óssea.The current study evaluated the healing of critical-size defects (CSD) created in rat calvaria treated with platelet aggregates produced by high-speed (Leukocyte- and Platelet-Rich Fibrin - L-PRF) and low-speed (Advanced Platelet-Rich Fibrin - A-PRF) protocols of centrifugation. Twenty-four rats were divided into 3 groups: Control (C), LPRF, and A-PRF. Five mm diameter CSD were created on the animals\' calvaria. The defects of the L-PRF and A-PRF groups were filled with 0.01 ml of L-PRF and A-PRF, respectively. The C group defects were filled with a blood clot only. All animals were euthanized on the 35th postoperative day. Histomorphometric and microtomographic analysis were then performed. The L-PRF and A-PRF groups had significantly higher bone volume and neoformed bone area than those of the C group, as well as lower bone porosity values (p<0.05). No significant differences were observed between A-PRF and L-PRF groups for the analyzed parameters. It can be concluded that i) L-PRF and A-PRF potentiated the healing of CSD in rat calvaria; ii) high and low-speed centrifugation protocols did not produce PRF matrices with different biological impacts on the amount of bone neoformation

    Effects of the probiotic Bifidobacterium animalis subsp. lactis HN019 as an adjunct to treatment of experimental periodontitis in rats

    Probióticos do gênero Lactobacillus estão sendo amplamente investigados no tratamento da periodontite. Contudo, os efeitos de microrganismos do gênero Bifidobacterium ainda são pouco conhecidos. Este estudo avaliou os efeitos do probiótico Bifidobacterium animalis subsp. lactis (B. lactis) HN019 como adjuvante à raspagem e alisamento radicular (RAR) no tratamento da periodontite experimental (PE) em ratos. No dia 0 do experimento, 32 ratos foram alocados em 4 grupos: C (controle), PROB (probiótico), PE-RAR e PE-RARPROB. Nos grupos PE-RAR e PE-RAR-PROB, a PE foi induzida pela colocação de ligaduras de seda ao redor dos primeiros molares inferiores dos animais. No 14&deg; dia, as ligaduras foram removidas e realizou-se a RAR. Nos animais dos grupos PROB e PE-RARPROB, o probiótico B. lactis HN019 foi administrado diariamente (10 mL/dia de 109 unidades formadoras de colônia) por 15 dias tendo seu início no 14&deg; dia do experimento. Os animais de todos os grupos foram submetidos à eutanásia 29 dias após o início do experimento. As hemimandíbulas e amostras de intestino delgado foram coletadas. Foram realizadas análises histomorfométricas, microtomográficas e imunohistoquímicas. Foram investigados, também, os efeitos microbiológicos de B. lactis HN019 no biofilme associado às ligaduras durante o desenvolvimento da PE. Todos os dados foram analisados estatisticamente. O Grupo PE-RAR-PROB apresentou menores reabsorção óssea alveolar e perda de inserção conjuntiva quando comparado ao Grupo PE-RAR, bem como menor número de osteoclastos, maior expressão de citocinas anti-inflamatórias e menor expressão de citocinas pró-inflamatórias (p <0,05). No grupo PE-RAR-PROB, os valores médios da profundidade da cripta do jejuno e duodeno foram significativamente maiores que aqueles do grupo PE-RAR. A proporção de bactérias aeróbias/anaeróbias foi maior nas amostras de biofilme de animais tratados com B. lactis HN019 em relação àquelas de animais não tratados (p <0,05). Dentro dos limites deste estudo, pode-se concluir que a utilização de B. lactis HN019 como adjuvante à RAR promove benefícios histológicos, microtomográficos e imunológicos adicionais no tratamento da PE em ratos, bem como melhora a morfologia intestinal.Lactobacillus probiotics have been investigated in periodontitis. However, the effects of the genus Bifidobacterium on periodontitis are hardly known. This study evaluated the effects of the probiotic Bifidobacterium animalis subsp. lactis (B. lactis) HN019 as an adjunct to scaling and root planing (SRP) in rats with experimental periodontitis (EP). At baseline, 32 rats were assigned to 4 groups: C (control), PROB, EP-SRP and EPSRP- PROB. In groups EP-SRP and EP-SRP-PROB, the mandibular first molars of the animals received a ligature. At day 14, the ligatures were removed and SRP was performed. Animals of groups PROB and EP-SRP-PROB were orally administered with 10 mL/day of 109 colony forming units of B. lactis HN019 for 15 days, starting at day 14. Animals were euthanized at day 29. The jaws and samples of the duodenum, jejunum, and ileum were resected. Histomorphometric, microtomographic and immunohistochemical analyses were performed. Microbiological effects of B. lactis on biofilm were also evaluated. Data were statistically analyzed. Group EP-SRP-PROB presented reduced alveolar bone resorption and attachment loss when compared with Group EP-SRP (p<0.05). Group EP-SRP-PROB showed significantly fewer osteoclasts, increased expression of anti-inflammatory cytokines and reduced expression of proinflammatory cytokines compared with Group EP-SRP (p<0.05). In group EP-SRPPROB, the mean values of crypt depth of the jejunum and dudoenum were significantly higher than the ones from group EP-SRP. B. lactis promoted a higher ratio between aerobic and anaerobic bacteria in biofilm samples (p<0.05). Within the limits of this study it can be concluded that the use of B. lactis HN019 as an adjunct to SRP promotes additional histologic, microtomographic and immunologic benefits in the treatment of EP in rats and improves the intestinal morphology

    Use of multispecies probiotic as an adjuvant in the treatment of peri-implant mucositis: a randomized controlled trial

    O propósito deste estudo foi avaliar o efeito adjuvante da terapia probiótica no tratamento de indivíduos com mucosite peri-implantar. Em um estudo clínico aleatorizado, duplo-cego e placebo-controle, 36 indivíduos com mucosite peri-implantar foram tratados com o uso de probiótico multiespécies associado ao debridamento mecânico (Grupo Teste) ou apenas debridamento mecânico (Grupo Controle). No Grupo Teste, a sessão de tratamento foi concluída com uma aplicação tópica profissional de gel de carboximeticelulose contendo 109 unidades formadoras de colônias (UFCs) de Bifidobacterium animalis subsp. lactis HN019TM, Lactobacillus rhamnosus HN001TM e Lactobacillus paracasei Lpc-37®. Os indivíduos receberam, então, cápsulas contendo 109 UFCs dos 3 microrganismos probióticos e foram instruídos a dissolverem o conteúdo das cápsulas em 20 mL de água para realizarem bochechos de 30 a 60 segundos, seguidos por deglutição, duas vezes ao dia, durante 12 semanas. O grupo Controle recebeu os mesmos procedimentos de debridamento mecânico e foi tratado com gel e pastilhas placebo. Parâmetros clínicos periodontais (Índice de placa modificado - IPM; Índice de Sangramento Sulcular Modificado - ISSm; Profundidade de Sondagem - PS e Sangramento à Sondagem - SS) foram avaliados no baseline (período pré-intervenção) e em 12 e 24 semanas após o debridamento mecânico inicial. Os dados obtidos foram estatisticamente analisados (p < 0,05). Não houve diferença, ao longo do tempo, para o percentual de IPM no grupo Teste. No grupo Controle, houve aumento significativo no percentual de IPM - escore 0, tanto nas comparações entre o baseline e 12 semanas como entre o baseline e 24 semanas (p < 0,05). Não foram observadas diferenças significativas nas comparações entre os grupos para os valores de IPM. No grupo Teste, o percentual de ISSm - escore 0 aumentou significativamente nas comparações entre o baseline e 12 semanas e entre o baseline e 24 semanas (p < 0,05). O percentual de ISSm - escore 1 diminuiu de forma significativa nas comparações entre o baseline e 24 semanas no grupo Teste. Nenhuma outra mudança significativa ao longo do tempo (comparações intragrupos) foi observada nos grupos Teste e Controle. Nas comparações entre os grupos, observa-se que a há maior percentual de ISSm - escore 0 no grupo Teste do que no grupo Controle em 24 semanas (p < 0,05). Ainda, há maior percentual de ISSm - escore 1 (24 semanas) e ISSm - escore 2 (12 semanas) no grupo Controle do que no grupo Teste (p < 0,05). Nos grupos Teste Controle, comparando os valores obtidos no baseline com aqueles de 24 semanas, a PS diminuiu significativamente, sendo que no grupo Controle esta diferença também foi observada nas comparações entre os valores de 12 semanas e àqueles do baseline (p < 0,05). Não foram observadas diferenças significativas nas comparações entre os grupos para os valores de PS em qualquer tempo experimental. Os valores de SS apresentaram diminuição significativa nas avaliações intragrupos, em ambos os tempos avaliados quando comparados ao baseline, tanto no grupo Teste como no grupo Controle (p < 0,05). Em 12 e 24 semanas, os valores de SS foram menores no grupo Teste quando comparado ao grupo Controle (p < 0,05). Em 24 semanas, 13 de 18 indivíduos do grupo Teste e apenas 6 de 18 indivíduos do grupo Controle apresentaram menos de 10% de SS (p < 0,05). Pode-se concluir que a utilização da terapia probiótica como recurso adjuvante ao debridamento mecânico promove benefícios clínicos adicionais no tratamento de indivíduos com mucosite peri-implantar.The aim of this study was to assess the adjuvant effect of probiotic therapy on the treatment of patients with peri-implant mucositis. In a double-blind, placebo-controlled, randomized clinical trial, 36 patients with peri-implant mucositis were treated with multispecies probiotic in association with mechanical debridement (test group) or with mechanical debridement only (control group). In the test group, the treatment session was concluded with a topical application of carboxymethyl cellulose gel containing 109 colony-forming units (CFUs) of Bifidobacterium animalis subsp. lactis HN019TM, Lactobacillus rhamnosus HN001TM, and Lactobacillus paracasei Lpc-37®. The patients received capsules containing 109 CFUs of the three probiotic microorganisms and were instructed to dissolve the capsule contents in 20 mL of water for mouth rinses during 30 to 60 seconds, followed by swallowing, twice a day, for 12 weeks. The control group was subjected to the same mechanical debridement procedures and treated with placebo gel and lozenges. Periodontal clinical parameters (modified plaque index - mPlI; modified sulcus bleeding index - mSBI; probing depth - PD, and bleeding on probing - BOP) were assessed at baseline (pre-intervention period) and at 12 and 24 weeks after the initial mechanical debridement. The data were statistically analyzed (p < 0.05). There was no difference over time in mPlI in the test group. There was a significant increase in mPlI in the control group - score 0, both in the comparisons between baseline and 12 weeks and between baseline and 24 weeks (p < 0.05). No significant differences in mPlI were observed in the comparisons between the groups. In the test group, mSBI - score 0 increased significantly in the comparisons between baseline and 12 weeks and between baseline and 24 weeks (p < 0.05). Also in the test group, mSBI - score 1 decreased significantly in the comparisons between baseline and 24 weeks. No other significant change was observed over time (within-group comparisons) in the test and control groups. In the between-group comparisons, there was a higher mSBI - score 0 in the test group than in the control group at 24 weeks (p < 0.05). Also, there was a higher mSBI - score 1 (at 24 weeks) and mSBI - score 2 (at 12 weeks) in the control group than in the test group (p < 0.05). In the test and control groups, when comparing the baseline values with those obtained at 24 weeks, PD decreased significantly, and this difference was also observed in the control group between baseline values and those at 12 weeks (p < 0.05). No significant differences were noted in the comparisons between the groups regarding PD in any time period. BOP decreased significantly in the within-group comparisons in both time periods assessed when compared with baseline, both in the test and control groups (p < 0.05). At 12 and 24 weeks, BOP was lower in the test group than in the control group (p < 0.05). At 24 weeks, 13 out of 18 patients from the test group and only 6 out of 18 patients from the control group had less than 10% of BOP (p < 0.05). It may be concluded that the use of probiotic therapy as an adjuvant in mechanical debridement provides additional clinical benefits in the treatment of patients with peri-implant mucositis
