Gender- und subgruppenspezifische Aspekte zur Optimierung systemtherapeutischer und operativer Behandlungsansätze in der Therapie des frühen Mammakarzinoms
Die Therapie des Mammakarzinoms wird immer komplexer und zielt zunehmend auf die Identifikation von genetischen Subgruppen ab, welche gezielt behandelt werden können.
Wenig Rücksicht wurde dabei auf die Behandlung von klinischen Subgruppen wie die ältere PatientInnen, männlichen Patienten oder solche mit einem beidseitigen Karzinom genommen. Die vorliegende Habilitationsschrift fasst die neusten Erkenntnisse dieser Subgruppen mit der Diagnose eines frühen Mammakarzinoms zusammen und stellt dieses in den Kontext mit der einschlägigen internationalen Literatur. Ein Schwerpunkt dabei ist die Male-Studie, die weltweit erste prospektive Studie zum männlichen Mammakarzinom, die die Effektivität drei verschiedener antihormoneller Therapieansätze untersucht und die daraus veränderte Lebensqualität beschreibt. Die Senta-Studie untersucht die Machbarkeit der zielgerichteten axillären Dissektion (TAD= targeted axillary Dissection) bei Frauen mit initial positiven axillären Lymphknotenstatus nach neoadjuvanter Chemotherapie mit gutem Ansprechen. Zur Behandlung des frühen Mammakarzinoms gehört somit auch die Kenntnis der optimalen Therapieansätze für Patientengruppen, die nicht klassischerweise in den großen prospektiven randomisierten Studien abgebildet sind aber ebenso einen Anspruch auf optimale onkologische Versorgung haben

Prognostic and predictive impact of gene expression in node-positive early breast cancer patients receiving dose-dense versus standard-dose adjuvant chemotherapy

We conducted exploratory transcriptomic analysis of 758 genes (Breast Cancer 360 panel, nCounter® platform; NanoString) in primary tumor samples collected during a phase 3 trial comparing adjuvant taxane-containing dose-dense chemotherapy (ddCTX) versus standard-dosed chemotherapy (stCTX) in resected EBC with ≥ 4 positive lymph nodes. Prognostic and predictive associations with disease-free survival (DFS) and overall survival (OS) were evaluated by Cox regression with false discovery rate (FDR) adjustment. Data were available from tumor samples of 141/226 patients (median follow-up: 14 years). Several genes/signatures, including immune markers, showed prognostic relevance in unadjusted analyses. Of these, two remained significant after multiplicity adjustment: a positive effect on DFS of programmed cell death 1 ligand-2 (PD-L2) in the ddCTX arm (univariate HR: 0.53, FDR-adjusted P = 0.036) and a negative effect on OS of HER2-enriched (HER2-E) signature in the stCTX arm (univariate HR: 5.40, FDR-adjusted P = 0.036). Predictive analyses showed greater DFS benefit of ddCTX in tumors with high antigen processing machinery (APM) expression (multivariate interaction P = 0.024). Multigene expression assays have a prognostic and predictive potential in advanced EBC, and further investigation is warranted in order to identify candidates for de-escalated treatment. from DNA damage. To study invertebrate p53 during early development, we identified three differentially
expressed p53 family members (p53, p97, p120) in the surf clam, Spisula solidissima.
In these mollusks, p53 and p97 occur in both embryonic and adult tissue, whereas p120 is exclusively
embryonic. We sequenced, cloned, and characterized p120 cDNA. The predicted protein,
p120, resembles p53 across all evolutionarily conserved regions and contains a C-terminal extension
with a sterile alpha motif (SAM) as in p63 and p73. These vertebrate forms of p53 are
required for normal inflammatory, epithelial, and neuronal development. Unlike clam p53 and
p97, p120 mRNA and protein levels are temporally expressed in embryos, with mRNA levels
decreasing with increasing p120 protein (R2 = 0.97). Highest surf clam p120 mRNA levels coincide
with the onset of neuronal growth. In earlier work we have shown that neuronal development
is altered by exposure to polychlorinated biphenyls (PCBs), a neurotoxic environmental
contaminant. In this study we show that PCBs differentially affect expression of the three surf
clam p53 family members. p120 mRNA and protein are reduced the most and earliest in development,
p97 protein shows a smaller and later reduction, and p53 protein levels do not change. For
the first time we report that unlike p53 and p97, p120 is specifically embryonic and expressed in
a time-dependent manner. Furthermore, p120 responds to PCBs by 48 hr when PCB-induced
suppression of the serotonergic nervous system occurs.This research was supported by NCI grant 44307

Questo lavoro è dedicato principalmente a studi linguistici e mostra che il somalo appartiene al ramo Cuscitico della lingua Camitica dell’Africa Orientale, ed è più strettamente correlata alle lingue afar e saho, piuttosto che al galla e all’oromo.
Reinish presenta le forme grammaticali esattamente nel modo in cui le ha trovate nei testi originali, oppure come li ha trascritti lui stesso, “in base alla propria percezione”. In the author’s words, this grammar owes a lot to “the practical grammar to the Somali language by Rev. Fr. Evangeliste de Larajasse and Ven. Fr. Cyprien de Sampont, London 1897.” The present work is dedicated to a primarily linguistic issue and shows that Somali belongs to the Lower-Cushitic branch of the Chamitic languages of East Africa and is most closely related to Afar and Saho and more remotely so to Gallo or Oromo.
Reinish presents grammatical forms in exactly the same way he found them in the original texts or as he has “transcribed them according to his own perception.” The author underlines that linguistic descriptions should not display forms “as they are expected according to certain grammatical theories”, but as they occur in reality. Grammatical rules must be deduced from actually existing forms, but the latter should not be changed according to a particular grammatical theory
Learning behind bars : how IRA prisoners shaped the peace process in Ireland
This book is an oral history of former Irish republican prisoners in the Republic of Ireland and Northern Ireland between 1971, the year of the introduction of internment in Northern Ireland, and 2000, the year of the closure of the high-security prison HMP Maze. It focuses on the lives of Irish republican prisoners inside Irish and British internment camps and prisons during the height of the Northern Irish Troubles. The book discusses the relationship between three themes: political subjectivity, informal education, and collective resistance. Based on extensive life-story interviews with 34 ex-prisoners, the book examines the evolution of their subjective understandings of self and identity at the intersection of informal education in the prisons and the collective resistance resulting from this subjectification. Using the recent conflict on the island of Ireland as a case study, the book provides insight into political prisoners' role in ending armed conflicts, and into the personal and political development of radical activists during their imprisonment. Of the many groups supporting the Northern Irish peace process in the 1990s, one of the most remarkable are former inmates of internment camps and prisons. This group is noteworthy because it was formed of collectives of political prisoners who were almost entirely self-educated. The book's central focus is as follows: due to their informal self-education, the republican internees and prisoners could influence political developments outside the prisons from within their organizations. The author argues that the key to the process of (political) subjectivity, the becoming of a subject inside and outside the prisons, is political education. It was, namely, the self-organized lectures and debates that formed the subject politically and strengthened the inmates' identity as 'Prisoners of War'. This subjectivity enabled them to stage acts of resistance in defence of their developed identity.
