Pulmonary Vein Isolation as a Treatment of Choice for Atrial Fibrillation in General Hospital Zadar for Period from 2018 to 2022
Hemokultura ostaje najbolji pristup za identifikaciju inkriminirajućih mikroorganizama kada se sumnja na infekciju krvotoka i za jamstvo da je antimikrobno liječenje odgovarajuće. Posljednjih godina napravljena su velika poboljšanja kako bi se povećala osjetljivost i specifičnost i smanjilo vrijeme za identifikaciju mikroorganizama izdvojenih iz hemokultura. Kod nekih infekcija, uzročnici se samo povremeno nalaze u krvi, pa se može provesti serija od tri ili više hemokultura, kao i vađenja krvi iz različitih vena, kako bi se povećala mogućnost pronalaska infekcije. Hemokulture se inkubiraju nekoliko dana prije nego što se prijave kao negativne. Neke vrste bakterija i gljivica rastu sporije od drugih i/ili im treba više vremena da se otkriju ako su u početku prisutne u malom broju. Pozitivan rezultat hemokulture ukazuje na to da su u krvi pronađene bakterije (bakterijemija). Druge vrste patogena, poput gljivica ili virusa, također se mogu naći u hemokulturi. Kada je hemokultura pozitivna, identificira se specifični mikrob koji uzrokuje infekciju i provodi se testiranje osjetljivosti kako bi se obavijestio zdravstveni djelatnik koji će antibiotici ili drugi lijekovi najvjerojatnije biti učinkoviti za liječenje. Važno je da medicinske sestre pamte da kada se prime nove narudžbe i za antibiotike i za hemokulturu, da se antibiotici ne smiju davati prije nego se uzmu uzorci za hemokulture. Primjena antibiotika kao empirijske terapije prije uzimanja uzorka hemokulture može utjecat na rezultate pretrage, kao i na primjenu neodgovarajuće antimikrobne terapije koja može utjecati na ishod liječenja (produljeni boravak ili smrtni ishod). Cilj ovog rada je prikazati ulogu medicinske sestra na osiguranju kvalitete rezultata pretrage hemokultura, te prikazati rezultate Službe mikrobiologije i parazitologije vezane uz praćenje vrijednosti indikatora kvalitete u predanalitičkoj fazi za uzorke hemokultura uzorkovanih u Općoj bolnici Zadar (OB Zadar) u razdoblju od 01.01.2022. do 31.12.2022.Blood culture remains the best approach to identify the incriminating microorganisms when a bloodstream infection is suspected, and to guarantee that the antimicrobial treatment is adequate. Major improvements have been made in the last years to increase the sensitivity and specificity and to reduce the time to identification of microorganisms recovered from blood cultures.In some infections, the causative agents are only occasionally found in the blood, so a series of three or more blood cultures, as well as blood draws from different veins, can be performed to increase the chance of finding the infection. Blood cultures are incubated for several days before being reported as negative. Some types of bacteria and fungi grow more slowly than others and/or take longer to be detected if initially present in small numbers. A positive result indicates that bacteria were found in the blood (bacteremia). Other types of pathogens, such as fungi or viruses, may also be found in the blood culture. When a blood culture is positive, the specific microbe causing the infection is identified and susceptibility testing is performed to inform the healthcare professional which antibiotics or other drugs are most likely to be effective for treatment. It is important for nurses to remember that when new orders for both antibiotics and blood cultures are received, antibiotics should not be given before blood cultures are taken. The use of antibiotics as empiric therapy before taking a blood culture sample can affect test results, as well as the use of inappropriate antimicrobial therapy that can affect the outcome of treatment (prolonged stay or death). Administering antibiotics prior to taking a blood culture will affect the results and adversely affect the treatment plan. The aim of this paper is to show how the nurse is responsible for the pre-analytical phase of blood culture testing and how her knowledge and skills can influence the quality of blood culture test results. The method of preparing the space, accessories and the patient himself when taking the test, as well as proper storage and transportation, will be explained

Grlobolja– implementacija hrvatskih nacionalnih smjernica
Grlobolja je jedan od najčešćih simptoma zbog kojih se pacijenti javljaju liječniku obiteljske medicine. Pri tom se najčešće radi o virusnim infekcijama kod kojih je dovoljna simptomatska terapija. Sama dijagnoza grlobolje ne podrazumijeva automatsku primjenu antibiotika. Jedini bakterijski uzročnik za koji je indicirana antimikrobna terapija je beta hemolitički streptokok grupe A (piogeni streptokok).
Interdisciplinarna sekcija za kontrolu rezistencije na antibiotike (ISKRA) Ministarstva zdravstva i socijalne skrbi Republike Hrvatske potakla je razvoj i primjenu nacionalnih smjernica o postupku s bolesnikom pri kliničkim stanjima u kojima antimikrobna terapija ima značajnu ulogu u liječenju pa tako i razvoj smjernica za dijagnostički i terapijski pristup u slučaju grlobolje.
Svrha je ovih smjernica da u svakodnevnoj praksi pomažu u razlučivanju kliničkih slika težih streptokoknih infekcija gdje je antimikrobna terapija opravdana od onih brojnih drugih slučajeva grlobolje gdje ta terapija nije opravdana, već naprotiv, može doprinijeti razvoju rezistencije bakterija na antibiotike
Implementation of the Domestic Violence Law in the Republika Srpska
In this paper author analyzes criminal offence "Violence in family or extended family". This criminal offence was introduced for the first time in the new Criminal Code of the Republika Srpska in 2000 (article 198). In the first part of the paper author exposes characteristics of legal provisions regarding criminal offence "Violence in family or extended family". In the second part practical implementation of this criminal offence is analyzed. For doing that analyzes, the author uses the interviews carried out with representatives of the police, prosecutors judges and representatives of non-governmental organizations. At the end author suggests introduction of certain measures for victim protection such as mandatory arrest of a perpetrator, the increased efficiency of the criminal procedure, restraining orders (regarding communication and contact between perpetrator and victim) and other measures, which should provide adequate implementation of this incrimination and optimal protection of victims of this crime
Inhabitants and Social Organization in the Villages near Zagreb
Within the ethnographic research of folk culture in the villages near Zagreb: Resnik, Čučerje, Ščitarjevo, Markuševec and Šestine, two themes were investigated: inhabitants and social organization. While researching and recording of the given data the author had in mind the time between the two World Wars. During the research, there was a need for registration of contemporary cultural phenomena. Although the main task was finding out the changes in folk life in these suburban villages near Zagreb between the two World Wars, it was necessary to establish transformation found after World War II up to the date of investigation. Competences of the general meeting of a limited liability company
and keywords This diploma thesis deals on the competence of general assembly in a limited liability company. In the introduction the author made a brief historical summary presenting the previous legislation of the Czech republic. The thesis is divided into four parts. The first part concerns with the general findings on the competence of general assembly including its classification. The second part is special to the first one, since it concerns with particular cases of competencies. This part is the most voluminous one because of its goal to interpret various conditions to take a decision depending on case of competency. In the third part the author made description of the French legislation and compared it to the Czech one. From the previous findings the author made a critical evaluation of contemporary legislation in the fourth part as well as specific suggestion for law-making changes
Core sample characterisation using 3D terrestrial laser scanning | TU Delft
The main purpose of this thesis is to conclude whether terrestrial laser scanning (TLS) can be used as an automated method to improve the classification of soils. It was in collaboration with Fugro N.V., a large Dutch consultancy and engineering firm that is active in onshore and offshore services in the field of geological and geotechnical investigations for a wide field of clients in the petroleum and gas industry but also for other infrastructure. Current methods for the classification of soils comprise of laboratory and in-situ measurements. Fugro is searching for a more accurate and time efficient method to classify soil and provided five soil samples on which a classification had to be performed. The Leica C10 laser scanner, provided by the department of Geoscience and Remote Sensing (TU Delft), was used to scan the soil samples. TLS is based on LIDAR (light detection and ranging) which is known for emitting pulses in a very narrow beam of monochromatic light (one wavelength) in the ultraviolet, visible or near-infrared range of the electromagnetic spectrum. After scanning the soil samples, the following features extracted from the 3D point cloud data were used to perform classification on each soil sample: intensity (backscattered energy), colour (an RGB-image), surface height variations (roughness). The chosen method was iso cluster unsupervised classification. After classification, interpretations were made based on the descriptions of the soil sample and knowledge of bare soil reflectance. The main factors that influenced the soil reflectance were related to characteristics of the samples: glauconite content (an iron-bearing mineral) and surface roughness. Both factors were responsible for a decrease in soil reflectance. Unsupervised classification was applicable on three out of five samples. However, only one sample provided good results. The other two samples were less promising, although some features could clearly be identified after comparing the classified image with ground truth data.Applied Earth Science
The governing bodies of a cooperative
This diploma thesis deals with cooperatives and closely focuses on their bodies. Czech legal order gives only superficial legislation which causes many questions and imperfections to arise. The aim of this thesis is to find solutions to these problems and create an ideal model of cooperative. Following that, the author bases his thoughts on the legal doctrine and Czech court jurisdiction. The introductory part focuses on general questions, i.e. history of cooperatives, legislation and legal characteristics of cooperatives. The author refers to the negative impact of the Communistic Party of Czechoslovakia on cooperativeness between years 1948 - 1989. Such impacts are still present in nowadays legislation. Another imperfections are seen in the starkness and fragmentation of the legislation itself. All these imperfections may be removed by the future law about business cooperatives. The main body of the thesis follows as this describes the cooperative body legislation. Current legislation brings three obligatory bodies - assembly, board and control committee. Next to these, every cooperative may establish optional bodies. The chapter on assembly deals with its status and scope.
