Management of Neurological Emergencies in Children: An Updated Overview
Neurological emergencies account for about one-third of the highest severity codes attributed in emergency pediatric departments. About 75% of children with acute neurological symptoms presents with seizures, headache, or other paroxysmal events. Life-threatening conditions involve a minor proportion of patients (e.g., less than 15% of children with headache and less than 5% of children with febrile seizures). This review highlights updated insights about clinical features, diagnostic workup, and therapeutic management of pediatric neurological emergencies. Particularly, details will be provided about the most recent insights about headache, febrile seizures, status epilepticus, altered levels of consciousness, acute motor impairment, acute movement disorders, and functional disorders, as well as the role of diagnostic tools (e.g., neuroimaging, lumbar puncture, and electroencephalography), in the emergency setting. Moreover, the impact of the current novel coronavirus disease2019 (COVID-19) pandemic on the evaluation of pediatric neurologic emergencies will also be analyzed

The Spectrum of Early Movement Disorders in Congenital Defects of Biogenic Amine Metabolism
This article analyzes the pattern of movement disorders in children with primary defects of biogenic amine metabolism emerging during the first 3 year of life. A PubMed search was performed using the keywords monoamine neurotransmitter disorders, congenital defects of biogenic amine metabolism, early onset movement disorders, autosomal dominant guanosine-triphosphate-cyclohydrolase deficiency, autosomal recessive guanosine-tryphosphate-cyclohydrolase deficiency, 6-pyruvoyl-tethrahydropterin synthase deficiency, sepiapterin reductase deficiency, dihydropteridine reductase deficiency, tyrosine hydroxilase deficiency, aromatic l-amino acid decarboxylase deficiency, dopamine transporter deficiency, and vesicular monoamine transporter 2. A personal series of 27 patients affected by these disorders was presented and discussed. Primary defects of biogenic amine metabolism result in a spectrum of movement disorders associated with derangement of postural reaction and global development delay. Movement disorders virtually represent the presentation symptoms in most of these diseases. The prominent early clinical patterns of movement disorders are akinesia (with rigidity or hypotonia) and hyperkinesia (dystonia, myoclonic jerks, and tremor). While some diseases present with a rather stereotyped clinical pattern (i.e.; Aromatic l-amino acid decarboxylase, sepiapterin reductase, and Dopamine transporter deficiencies), others (i.e.; Guanosine-triphosphate-cyclohydrolase, 6-pyruvoyl tetrahydropterin synthase, dihydropteridine reductase, and tyrosine hydroxylase deficiencies) may present with more pleomorphic features. As a general rule, earlier the onset of the disease is, more severe and generalized is the derangement of motor functions. Inherited disorders of monoamine neurotransmitter metabolism include the few idiopathic movement disorders with the possibility of etiological treatments. For these reasons, they should always be considered in the diagnostic work-up of children with early onset movement disorders

Until the 1960s, the urban expansion of Tokyo was not overseen by any actual urban planning system but rather by an independent development surrounding the railway network and the train stations. Recently, and in view of what were supposed to be the 2020 Olympic Games, which have been postponed until 2021 as a consequence of the Covid–19 pandemic, numerous urban requalification interventions have been planned that go beyond those strictly linked to the performance of the games. These projects include building infrastructure within the transportation system in the metropolitan area. Today, talking of mega sports events means dealing with the changes that have put the area in the forefront over the past decades and analysing its urban changes. We will reflect here on the forms of urban requalification that are driven by constructing sports installations and redefining public transport lines
A hybrid method combining analytical and simulation models for performance evaluation of reconfigurable manufacturing systems
Today's dynamic manufacturing context, characterized by frequent product variations and consistently rising production volumes, forces companies to continuously adapt their systems with frequent reconfigurations. To support effective decision-making in this regard, it is necessary to have performance evaluation methods that can be modified conveniently to represent configuration alternatives while accounting for the intertwined dynamics of different production areas. The objective of this paper is to propose a modular architecture for performance evaluation of manufacturing systems, able to integrate models of different parts of the same system that are built independently from each other with different approaches, as analytical or simulation. The proposed method is based on the decomposition approach and evaluates the performance of manufacturing systems at the steady-state. The method has been validated through comparison with discrete event simulation considering different system layouts and parameters. Results demonstrate the accuracy of the method and the robustness of the underlying evaluation algorithm. The applicability of the method in industry has been proven in a case study involving the reconfiguration analysis of a manufacturing system producing electrical distribution equipment in scenarios with strongly increasing demand of products
