Gender Medicine in Italy: The Point of View of Maria Grazia Modena: The Story, Limits, and Plans: My Personal Story
Gender medicine (GM) in Italy was born with the association I have been part of since the 90s Tailored Health for Women created by Dr. Elvira Reale and eight women doctors. We started research on risk factors for illness in women’s daily lives with Ministries of Pair Opportunity and Health, summarizing important documents in the history of GM in Italy. We had little success initially since (1) both ministers were changed and the successors were not interested in GM; (2) we were considered feminists and our congresses attended by few, all women! Each of us continued research in their own field, meeting every year with growing enthusiasm. The biggest limits for GM development in Italy are the excessive fragmentation of often self-referential associations/working groups, absence of GM in the core curriculum of medical school and of a gender-based law for health care. Finally I report of my personal story, an example of “gender related storm.”
Inferenze archeoambientali dalle sequenze polliniche della Terramara di Baggiovara - Modena (XVII - XVI sec. a.C.)
Il lavoro presenta i primi dati ottenuti da analisi palinologicheeffettuate nel sito della Terramara di Baggiovara, Modena. L’analisipalinologica è stata finalizzata alla ricostruzione del paesaggio vegetaleprecedente, coevo e successivo alle fasi di vita dell’abitato e ha permesso diottenere prime indicazioni sull’attività produttiva al suo interno. Inparticolare, emerge un paesaggio decisamente aperto con copertura forestaleconfinata lontano dall’abitato. Gli spettri pollinici sono caratterizzati dauna forte presenza di indicatori di pascolo, cereali e altre speciesinantropiche. Ne emerge, dunque, un paesaggio culturale con caratteri diantropizzazione che erano già evidenti in Pianura Padana durante il bronzomedio. Abstract The study reports dataobtained by the palynological analyses of samples taken from the Terramara ofBaggiovara, located in Modena, Emilia Romagna - northern Italy. Pollen allows to reconstructthe plant landscape that existed ‘before, during and after’ the terramara. Datasuggest that the economy of the site was mainly based on animal breeding ratherthan on crop cultivation. In particular, the landscape was open and the forest coverwas low.Pollen spectra show astrong presence of pastoral indicators, cerealia and other sinantropic indicators.They are important characters ofanthropization of these lands that testified how a cultural landscape hasalready developed in the Middle Bronze age in the Po Plain

Epidemiology and social impact of early onset dementia in the province of Modena, Northern Italy
Aims of the study. Early onset dementia (EOD), defined as onset of dementia <65 years, has a much higher social impact compared to the more common late onset dementia. EOD epidemiologic data in Italy are extremely scarce, and international estimates of prevalence are considerably variable, ranging between 15.1 and 153/100.000 in the age 45-65. We present data from an ongoing study aimed at establishing EOD epidemiology in a Northern Italy community (Modena province, around 700.000 inhabitants).
Materials. We retrospectively recruited all patients residing in Modena province referred to the Modena Baggiovara Hospital Neurologic Clinic CDCD (Centro disturbi cognitivi e demenze), by family practitioners and geriatricians of the province. Eligible patients were those affected by EOD from 2012 to January 2019. EOD was diagnosed in patients <65 years presenting with cognitive/behavioral symptoms, following a comprehensive neurological examination by a cognitive neurologist, as well as brain MRI, FDG PET scanning and CSF analyses when appropriate.
Methods. We collected clinical data such as age at onset, disease severity at time of diagnosis, time delay from onset to diagnosis, and epidemiologic data including residence and occupational status.
Results. We identified 223 patients with an EOD diagnosis from 2012 to January 2019. Of these, 103 are males (46,2%). 97 patients have AD (21 lvPPA, 13 PCA, 2 frontal variant), 47 have FTD (33 bvFTD, 14 svPPA), 6 have leucoencephalopathy. Mean onset age was 58,6 years (SD 5,1). Median time from symptoms onset to diagnosis was 3.1 years (SD 29,83). Mean MMSE score at diagnosis was 22,63/30 (SD 5,2). 24 patients were working at time of diagnosis (10,76%) and had to quit working due to the cognitive impairment. Two patients (0,89%) had young children (<18) at time of diagnosis.
Discussion. We provide the first epidemiological data on EOD in Italy. These are consistent with the estimates calculated by transposing European data to the population of Modena province (estimated prevalence=200 patients, detected prevalence=223 patients)1. Our data show a higher percentage of atypical AD phenotypes and FTD in the EOD group compared to late onset disease2. Also, CAA and leucoencephalopaties were over-represented compared to LOD. From a social perspective, EOD has significant impacts on patients and their families: patients are generally forced to leave work, and their children are expected to need psychological support, currently not routinely available.
Conclusions. Our population study provides the first data regarding EOD epidemiology and social impact in Italy
Possible influence of hemodynamic and neurohormonal factors on the heart's response to arterial hypertension
Hypertrophy represents a frequent but inconstant response of the heart to hypertension and probably other nonhemodynamic factors are involved. We have performed 2D and Doppler echocardiography and assessed of neurohumoral pattern in 85 untreated patients with hypertension (diastolic blood pressure: 101 +/- 12.8 mmHg). Left ventricular hypertrophy was defined as a mass index greater than 134 g/m2 in males and 110 g/m2 in females. Doppler evaluation of left ventricular filling pattern was performed to detect the possible association of hypertrophy and diastolic dysfunction. In all patients the following neurohumoral substances were sampled and tested: plasmatic renin activity (PRA), aldosterone and norepinephrine. At Doppler echocardiography, 27 patients had hypertrophy and diastolic dysfunction, 15 only hypertrophy and 43 only diastolic dysfunction. The presence or absence of morpho-functional anomalies were independent of age, duration of hypertension and blood pressure levels. The mean value of neurohumoral substances were: norepinephrine 323.3 +/- 245 pg/ml, PRA 2.5 +/- 4 ng/ml/h, aldosterone 153.58 +/- 102 pg/ml. A significant correlation was found between PRA and blood pressure, and between aldosterone and all the Doppler-derived parameters of diastolic dysfunction. In conclusion, left ventricular hypertrophy seems to be related to alteration in ventricular geometry rather than to hemodynamic factors.
