Tacrolimus ointment may improve the effectiveness of adalimumab in patients with hidradenitis suppurativa: a novel promising treatment

A Case of Ofuji Disease Successfully Treated with the Combination of Low-Dose Indomethacin and Topical Tacrolimus
Eosinophilic pustular folliculitis (EPF) (Ofuji disease) is a chronic, noninfectious pruritic cutaneous disorder of unknown etiology. No official guidelines are available for its treatment. Herein we present the case of a 59-year-old Caucasian man admitted to our outpatient clinic due to a generalized itchy skin rash characterized by papulo-pustules involving the face, trunk, and limbs. Histological examination supported the clinical diagnosis of EPF (Ofuji disease). The combination of low-dose oral indomethacin and topical tacrolimus ointment once a day led to a complete resolution of the lesions as well as associated symptoms in 8 weeks
Acne Smart Club: An Educational Program for Patients with Acne
Objective: To evaluate adherence to therapy in acne patients using mobile phones and Short Message Service (SMS) to communicate. Methods: 160 patients were randomly assigned to two groups: the SMS group received 2 text messages twice a day for a period of 12 weeks; the control group did not receive any messages. Before and after 12 weeks, the following evaluations were performed in all patients: digital photographs, the Global Acne Grading System, the Dermatology Life Quality Index, the Cardiff Acne Disability Index, the doctor-patient relationship evaluated through the Patient-Doctor Depth-of-Relationship Scale, and the adherence to treatment evaluated by asking patients how many days a week they had followed the therapy. For statistical analysis we used Student's t test. Results: The SMS group had a better improvement of all parameters compared to the control group. Conclusion: Adherence and compliance are higher for patients who are included in a strategy of control. © 2014 S. Karger AG, Basel
Valutazione dell'efficacia di un coadiuvante topico a base di argento micronizzato, zinco acetato e acido laurico nel trattamento dell'acne lieve-moderata
L'acne è una dermatosi ad eziopatogenesi multifattoriale in cui differenti fattori contribuiscono al mantenimento del processo infiammatorio. Il ruolo del P. acnes è stato, infatti, rideterminato in quanto trigger principale della risposta infiammatoria, essendo in grado di attivare la liberazione di numerose citochine proinfiammatorie. Non a caso, gli antibiotici sono tra i farmaci cardine della terapia topica dell'acne. Il loro uso è, tuttavia, limitato dallo sviluppo di resistenze batteriche. Ne deriva la necessità di utilizzare nuove molecole, le cui proprietà antibatteriche non siano suscettibili di fenomeni di resistenza. Tra le molecole che hanno destato maggiore interesse, per l'azione antibatterica non antibiotico-dipendente, e, pertanto, non soggetta allo sviluppo di resistenze, annoveriamo l'acido laurico, l'argento micronizzato e lo zinco acetato. Il nostro studio si propone di valutare l'efficacia nel trattamenti dell'acne lieve-moderata di una terapia topica a base di argento micronizzato, acido laurico e zinco acetato attraverso una valutazione sia oggettiva, mediante GAGS e Sebutape, che soggettiva, mediante l'ausilio di un nuovo strumento di valutazione della componente psicologica del paziente acneico: l'acne radar
Update on pathogenesis of hidradenitis suppurativa
The exact pathogenesis of hidradenitis suppurativa (HS) is still unproven. In the last few years numerous studies hypothesized that the disease is triggered by genetic and environmental factors. Follicular occlusion seems to be central to its development followed by follicular rupture, and immune responses finally leading to the development of clinical HS lesions. Data to date suggest that there is immune dysregulation in both the cutaneous and systemic immune system in patients with HS, but relative contributions from the innate and adaptive immune systems have not been fully elucidated. Furthermore, it can be further assumed that endocrine disorders related to follicular diseases can be implicated in the pathophysiologic pathways of HS. Ultraviolet radiation, vitamin D and COVID-19
Severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2), the causative agent of coronavirus disease 2019 (COVID-19), has become pandemic on March 11, 2020. COVID-19 has a range of symptoms that includes fever, fatigue, dry cough, aches, and labored breathing to acute respiratory distress and possibly death. Health systems and hospitals have been completely rearranged since March 2020 in order to limit the high rate of virus spreading. Hence, a great debate on postponable visits and treatments including phototherapy for skin diseases is developing. In particular, as regards phototherapy very few data are currently available regarding the chance to continue it, even if it may be an useful resource for treating numerous dermatological patients. However, phototherapy has an immunosuppressive action possibly facilitating virus infection. In the context of COVID-19 infection risk it is important to pointed out whether sunlight, phototherapy and in particular ultraviolet radiation (UV-R) constitute or not a risk for patients.
