Disfiguring Hidradenitis Suppurativa of the Face
Introduction: Hidradenitis suppurativa (HS) is a chronic, disfiguring, and debilitating inflammatory skin condition accompanied by pain, malodorous discharge, scarring, and a scarce quality of life. Skin folds are the most frequently affected areas, while the face is rarely involved. Case Presentation: We herein report a case of a man in his 40s affected by a severe form of HS for 15 years. Physical examination showed massive, bridged scarring located on the cheeks, temporal area, chin, nuchae, and scalp. Discussion: Massive face scarring due to HS is an infrequent event and represents a further burden for patients affected by HS and a therapeutic challenge for clinicians due to the lack of codified guidelines. Novel therapies with lasers and combination therapies are emerging and may be beneficial for hypertrophic scarring in HS

The melanin pigmentation: cellular and biomolecular mechanisms
Several factors are determinant for the color of human skin: the thickness of the stratum corneum, the speed of blood flow, the degree of oxygenation of hemoglobin but the main role is played by the presence of pigments such as carotenoids and, above all, melanin. The amount and the type of melanin is genetically determined, but they are strongly influenced by the other conditions such as hormonal and environmental: age, presence of metal ions, and inflammatory processes, and specifically exposure to sunlight. In vertebrates, melanocytes cells, responsible for the production of melanin, have the main function, but not exclusively, to protect the skin from the genotoxic stress produced by ultraviolet rays (UV); melanin absorbs the UV and neutralizers reactive oxygen species (ROS) they produce. Some studies clarified, at least in part, enzymatic and non enzymatic factors involved in the biosynthesis of melanin and the molecular mechanisms underlying the different responses of skin pigmentation to external stimuli, particularly to solar radiation. A case of erythrodermic psoriasis successfully treated with guselkumab
Erythrodermic psoriasis (EP) is the most severe form of psoriasis, resulting in significant morbidity and mortality. EP treatment with biologics is not well ruled by international guidelines, so most biological drugs are used basing on case reports or small case series. Guselkumab, a fully human anti-interleukin (IL)-23 monoclonal antibody, is approved for moderate to severe plaque psoriasis while its use in EP is off label. To date, no case reports on Caucasian patients have been described in literature. We report the case of 38-year-old Caucasian male with EP successfully treated with guselkumab, reaching PASI 100 after 20 weeks of therapy and still maintaining this response at week 48. Our case report suggests guselkumab as an efficacious and well tolerated treatment in EP, presenting a long term efficacy in the prevention of recurrences. Further studies are warrant to confirm our data, with controlled trials specifically dedicated to EP being strictly needed in order to verify the role and efficacy of anti-IL23 in EP. A Case of Pemphigus Vulgaris and Hidradenitis Suppurativa: May Systemic Steroids Be Considered in the Standard Management of Hidradenitis Suppurativa?
Introduction: Recent studies have demonstrated a correlation between pemphigus and hidradenitis suppurativa (HS). Case Presentation: Here, we report the case of a 58-year-old man affected by HS for about 30 years who was found to have lesions attributable to pemphigus during a checkup. Discussion/Conclusion: There are few data in the literature on the use of systemic corticosteroids for the two diseases, and our article wants to be a food for thought on the management and treatment of these diseases
