Trends in Facility Caseloads of Adult Pneumonia in Nairobi
Pneumonia commonly causes hospitalizations and deaths among all age groups in Kenya, with major socio-economic burden. It is the main cause of childhood mortality worldwide. Adult pneumonia prevalence is also notably high but due to lack of data on its burden, disease outcomes and epidemiology, this work became necessary.This was a cross-sectional study of 56 health facilities selected through one-stage cluster sampling. Passive Surveillance data was obtained from official statistics. Facility outpatient and inpatient registers were reviewed for pneumonia cases. Data was collected using phones running Android operating system, equipped with Open Data Kit software, then subjected to statistical and descriptive trend analysis using STATA 13.Inpatient and outpatient adult pneumonia cases rose steadily from 2011 to 2014 though the prevalence in 2014 was 3.2% of all outpatients, and 6.9% of inpatients, down from 4.8% and 8.1%, respectively, in 2013. Women visited the facilities more than men, with men (95% Confidence Interval of mean: 41.5-45 years) older than women (CI: 38-41.8). Some 46.4% of the patients were inpatients, with average length of stay of 5 days; and 76.6% lived in slums. Of those who died, 72.3% were men while 59.1% were younger than 35 yrs.The prevalence of adult pneumonia is significant. Its burden needs to be determined so as to help policy makers identify required healthcare investments, preventable risk factors, and further research needs
