    Obesity and prevalence of chronic diseases in the 1999–2000 Italian National Health Survey

    Abstract Background There is consistent evidence that obesity is a correlate of mortality. Less information is available about the relation between body weight and the prevalence of diseases. We investigated the prevalence of overweight and obesity and their relationship with 14 groups of chronic diseases in a Mediterranean population using data from the Italian National Survey collected in 1999–2000. Methods A sample of 52,300 families was randomly selected using a complex stratified multistage design, within strata of geographical areas, municipalities, and household sizes, to produce estimates representative of the whole Italian population. Data were collected by civil servants both with an interview and a self-reported questionnaire. Results The present study documents an increase in the prevalence of overweight among Italian adults in the last decades and an increased prevalence of several chronic conditions in obese or overweight individuals. A general pattern of a positive association between excess weight and chronic disease was observed for both sexes. The ratio of the prevalences of cardiovascular diseases, diabetes and chronic respiratory diseases was higher in obese versus normal-weight individuals in the age group under 45 years. Conclusion To reduce the prevalence of chronic diseases a policy promoting a healthier individual lifestyle is becoming more and more desirable.</p

    Risky behaviors, substance use, and other lifestyle correlates of energy drink consumption in children and adolescents: a systematic review

    Increasing concerns have been raised on the health-related risks connected with energy drink (ED) consumption in children and adolescents, with high acute or chronic consumers exceeding 10% in either age group in Europe in 2011. Preliminary evidence has suggested a common pattern of ED and substance use, especially alcohol. Additional evidence has been accumulating very fast; in addition, other lifestyle and risky behaviors may contribute to shed light on the complex interplay of factors involved in ED consumption. We have undertaken a comprehensive systematic review of the evidence on psychosocial correlates of ED consumption in 0-18 years subjects, as published up to April 1, 2021, in MEDLINE/PubMed, PsycINFO, and Cochrane Reviews and Central Register of Controlled Trials, which allowed to select 104 original articles. Only ~ 10% of the papers provided results based on longitudinal analyses. A common pattern of ED consumption and polysubstance use, including alcohol, tobacco, and soft and hard drugs, was still confirmed in adolescents; violent and risky behaviors were also related to a higher ED consumption. In addition, frequent ED consumers are more likely to have bad dietary habits, including consumption of sugar-sweetened beverages and junk foods. A generally inconclusive evidence was found for sport/physical activities, although sedentary behaviors were generally related to ED consumption.Conclusions: Frequent ED consumption might be a screening indicator to identify students at risk of substance use or other risky/problem behaviors; enquiring about an adolescent's recent ED consumption could create opportunities for early intervention/prevention by informed pediatricians.     Dietary Patterns and Cancer Risk: An Overview with Focus on Methods

    Traditionally, research in nutritional epidemiology has focused on specific foods/food groups or single nutrients in their relation with disease outcomes, including cancer. Dietary pattern analysis have been introduced to examine potential cumulative and interactive effects of individual dietary components of the overall diet, in which foods are consumed in combination. Dietary patterns can be identified by using evidence-based investigator-defined approaches or by using datadriven approaches, which rely on either response independent (also named “a posteriori” dietary patterns) or response dependent (also named “mixed-type” dietary patterns) multivariate statistical methods. Within the open methodological challenges related to study design, dietary assessment, identification of dietary patterns, confounding phenomena, and cancer risk assessment, the current paper provides an updated landscape review of novel methodological developments in the statistical analysis of a posteriori/mixed-type dietary patterns and cancer risk. The review starts from standard a posteriori dietary patterns from principal component, factor, and cluster analyses, including mixture models, and examines mixed-type dietary patterns from reduced rank regression, partial least squares, classification and regression tree analysis, and least absolute shrinkage and selection operator. Novel statistical approaches reviewed include Bayesian factor analysis with modeling of sparsity through shrinkage and sparse priors and frequentist focused principal component analysis.     A new Italian instrument for the assessment of irritability in patients with epilepsy

    The purpose of the work described in this article was to analyze the psychometric properties of a new Italian instrument for the assessment of irritability in adult patients with epilepsy (I-Epi). Five hundred four patients from nine secondary and tertiary Italian centers for the care of epilepsy were recruited and interviewed. Each patient was evaluated on a series of demographic and clinical variables recorded before administration of the I-Epi and the AQ (Aggression Questionnaire), used for external validity. The final results supported the reliability and validity of the I-Epi as a measure of irritability in the adult epilepsy population. The psychometric characteristics of the I-Epi seemed fairly good. We believe that adoption of this new instrument could be very useful in both clinical and research management of patients with epilepsy. © 2011

    Are Reproducible Dietary Patterns Consistently Associated With Disease Outcomes or Their Drivers in Italy? A Systematic Review

    The strength, direction, and trend of associations between specific diseases and reproducible a posteriori dietary patterns (DPs) based on principal component analysis (PCA) or exploratory factor analysis (EFA) have rarely been investigated across populations. We conducted a systematic review of PCA/EFA-based DPs identified in Italy to explore 2 methodological issues: 1) cross-study reproducibility of Italian DPs; 2) consistency of associations between reproducible DPs and the same/similar disease outcomes/DP drivers/correlates. The systematic review process and findings on DP cross-study reproducibility were published separately. This paper focuses on associations, summarizing the data in figures and tables, with post-hoc criteria for similarity among target variables, statistical methods, and adjustment for confounding. Predefined rules of inference were used to evaluate selected Hill's causal criteria (consistency, strength, and dose–response effects) and draw valid scientific conclusions on the association between PCA/EFA-based DPs and similar/the same target variables. Fifty-two articles, primarily on EFA-based DPs derived from food frequency questionnaires, were included. Regression models were used to explore the relationships between DPs and disease outcomes/DP drivers, aligning with original research questions, study designs, and literature on confounding. When considering similar target variables, 9 groups of reproducible DPs showed &gt;50% statistically significant associations in the same direction across 1–3 groups of target variables, such as socioeconomic characteristics, incidence of chronic diseases, overall/cause-specific mortality, cardiovascular disease risk factors, pregnancy/breastfeeding-related and elderly-related outcomes. Groups targeting dairies/sweets and vegetable sources of fats showed &gt;50% nonsignificant findings across all similar target variables. Overall, 54% of findings were nonsignificant. When considering the same target variable, the median number of DPs per group was equal to 2 (interquartile range: 2–2.5). Together with population comparability issues, this prevented us from reliably performing any meta-analyses. At this stage, valid scientific conclusions cannot be drawn to inform Italian nutritional recommendations. This study was registered at PROSPERO as registration number CRD42022341037

    Smoking in Italy, 1990-1991

    Smoking prevalence and patterns in Italy were analyzed using data from the 1990-1991 Italian National Health Survey, based on a sample of 27, 135 males and 28,854 females aged 15 years or over, randomly selected within strata of geographic area and size of the place of residence and of the household, in order to be representative of the general Italian population. Overall, 26.9% of the Italians aged 15 years or over described themselves as current smokers (37.2% males, 17.4% females), and 14.0% as ex-smokers (22.2% males, 6.4% females). The difference in smoking prevalence between males and females was 65% below age 45, but increased substantially with increasing age up to 5-fold above age 65. Moderate smokers (&lt; 15 cigarettes per day) were 12.6% of males and 10.4% of females, intermediate smokers (15 to 24 cigarettes per day) 17.7% of males and 5.5% of females, and heavy smokers (&gt; 25 cigarettes per day) 6.3% of males and 1.5% of females. Pipe or cigar smokers were 0.6% of males. The averange number of cigarettes per smoker per day was 16.6 (17.9 for males, 14.0 for females). The overall smoking prevalence of 26.9% was the lowest registered since 1949, thus confirming the long-term steady decline of smoking, particularly among males. Smoking prevalence, however, has remained constant over the last 15 years among females, after substantial rises in previous calendar years. These falls in overall self-reported smoking prevalence were reflected in declines of legal sale figures (-15% between 1986 and 1991), although it is difficult to quantify the impact of smuggling on total tobacco consumption. Thus, at least part of the falls in self-reported tobacco consumption is attributable to increased underreporting. In males, but not in females, smoking was less frequent in northern (and wealthier) areas of the country, and in more educated individuals. The opposite pattern was observed in females, indicating that even more educated Italian women have not yet recognized the accumulated evidence on the health consequences of smoking. These patterns in smoking are reflected by recent trends in lung cancer, which show some decline in males but persistent upward trends in females, although still on much lower absolute values. </jats:p

    Processed meat and selected hormone-related cancers

    Objective: To assess and quantify the association between processed meat consumption and cancers of the breast, endometrium, ovary, and prostate. Methods: Data were derived from an integrated network of hospital-based case-control studies conducted between 1982 and 2006 in various Italian areas. These studies included 5981 cases of cancer of the breast, 992 of the endometrium, 2002 of the ovary, 1582 of the prostate, and a total of 16 394 controls with data on processed meat. Odds ratios (ORs) and 95% confidence intervals (CIs) were calculated by unconditional multiple logistic regression models, adjusted for major recognized confounders for each cancer site. Results: The median consumption of processed meat in this population was 2 portions per wk in each cancer-specific cases, controls, and overall, corresponding to 100 g/wk. The OR for the highest (≥20 g/d) compared with lowest (<10 g/d) category of processed meat consumption was 1.16 (95% CI 1.06–1.28) for breast, 1.31 (95% CI 1.07–1.60) for endometrial, 1.49 (95% CI 1.30–1.71) for ovarian, and 0.89 (95% CI 0.74–1.07) for prostate cancer. Conclusion: In this case-control study, we found some excess risks of high processed meat consumption with female hormone-related cancers. Conversely, no association with prostate cancer was found in men

    Olive Oil and Nuts in Rheumatoid Arthritis Disease Activity

    Few observational studies investigated the relationship between single food groups and disease activity in rheumatoid arthritis (RA). Within a recent Italian cross-sectional study (365 patients, median age: 58.46 years, 78.63% females), we focused on two food groups, olive oil and nuts, representing vegetable sources of fatty acids. Disease activity was measured with Disease Activity Score on 28 joints based on C-reactive protein (DAS28-CRP) and the Simplified Disease Activity Index (SDAI). Robust linear and logistic regression models included tertile-based consumption categories of each food group and several confounders. Stratified analyses were performed by disease severity or duration. Higher consumption of both food groups exerted a favorable effect on disease activity, significant only for olive oil (Beta: &minus;0.33, p-value: 0.03) in the linear regression on the overall sample. This favorable effect was stronger in the more severe or long-standing forms of RA (p-value for heterogeneity &lt;0.05, especially for disease severity). Significant ORs were as low as ~0.30 for both food groups, strata (i.e., more severe and long-standing RA), and disease activity measures. Mean DAS28-CRP significantly decreased by ~0.70 for olive oil and ~0.55 for nuts in the two strata; mean SDAI significantly decreased by 3.30 or more for olive oil in the two strata. Globally, the beta coefficients doubled, and the ORs halved (in absolute values) for both food groups, reaching significance in 12 of the 16 available models fitted to the more severe or long-standing RA strata. More compromised forms of RA may benefit from increasing consumption of olive oil, olives, and nuts

    Validation of the Italian version of the Tool to Measure Parenting Self‐Efficacy questionnaire using data from an intervention study

    Background: Parent self-efficacy (PSE), parents' confidence in their ability to successfully raise their children, has proved to be a powerful direct predictor of specific positive parenting practices. The aim of this study was to validate the Italian version of the Tool to Measure Parenting Self-Efficacy (TOPSE) using data from the questionnaires previously completed in a controlled before–after study conducted in 2015 to evaluate a newsletter programme to help improve parenting. Mothers and fathers of newborns were asked to complete the TOPSE at the child's birth (t0), at 6 months (t1) and at 12 months (t2): 265 TOPSE questionnaires were collected at t0 (43%), 158 at t1 (26%) and 188 at t2 (31%). Methods: We measured internal reliability using Cronbach's alpha for each of the eight domains of the TOPSE. The intracluster correlation coefficient (ICC) was used to evaluate the external reliability only for parents with more than one child. Responsiveness was measured by testing the ability of the questionnaire to detect differences between groups and times that we expected to be measurable, based on consolidated findings in the literature. Mean scores of PSE improved from t0 to t2 (Hypothesis 1), PSE was lower at baseline for first-time parents than for those with multiple children (Hypothesis 2) and the improvement from t0 to t2 was stronger for first-time parents than for parents with multiple children (Hypothesis 3). Results and Conclusion: Based on our sample of questionnaires, the Italian version of the TOPSE was reliable for almost all of the domains except for Emotion, Self-acceptance and Learning, which could be refined by re-framing or dropping one item. External reliability was moderate, bearing in mind that the questionnaire was repeated at different times over 12 months, during which parents normally change. Responsiveness was good, especially for the Emotion and Empathy domains
