Trends in the prescription of antidepressants in urban and rural general practices
Analyzed the relationship between antidepressant drug (AD) prescription by general practitioners and population density in Italy between 1983 and 1988. The association between AD prescription and major sociodemographic variables was examined, and the seasonal pattern of prescribing was investigated. Data were calculated as defined daily dose (DDD), and the area was divided into 25 districts, classified on the urban-rural continuum according to population density. An increase in the levels of prescription of AD was observed over the 6 yrs. No correlation was found between DDD/1000 inhabitants/day rate and population density. Only the proportion of females in the population was significantly associated with AD prescription. Analysis of the seasonal variation in prescription showed a moderate degree of seasonality in all districts; however, seasonality was correlated only with the number of pharmacies available in each district. The demand for extramural psychiatric intervention in a community-based service
Using a psychiatric case register, 549 patients who had extramural contacts with the South Verona, Italy, community psychiatric service in 1983 were selected and followed up for 3 yrs. Out of 12,429 extramural contacts, 16% were unplanned (drop-in) contacts. A linear-logistic model was used to investigate the joint effects of sociodemographic and clinical variables on the frequency of unplanned extramural contacts. The model to fit the data was complex, confirming that many interactions among the variables affect the probability of using the community services on an urgent or unplanned basis. Long-stay and long-term psychiatric patients in an area with a community-based system of care: A register follow-up study
Analyzed case-register data on 25 long-stay psychiatric inpatients and 29 psychiatric outpatients in community-based treatment. 88% of the long-stay Ss were still long-stay after 2 yrs, whereas only 45% of long-term Ss in the community remained in long-term treatment over the same period. Ss with psychoses were most likely to remain in treatment, and Ss with neuroses were most likely to suspend treatment. Service utilization by schizophrenic patients in Groningen and South-Verona: An event-history analysis
The question addressed to in this paper is whether severely mentally ill patients are treated differently in a community mental health service without the back-up of a mental hospital (South-Verona, Italy) compared with an institution-based system in which mental hospitals, although highly modernized, are still predominant (Groningen, The Netherlands). Using the psychiatric case-registers in both areas, the patterns of care in 2 years of follow-up of schizophrenic patients were constructed. Survival analysis was used to analyse in-, day- and out-patient episodes of care. Three-quarters of the Groningen and half of the South-Verona patients experienced at least one episode of hospitalization; 20% of the Groningen and 5% of the South-Verona patients were long-stay patients at the end of the observation period. The South-Verona patients had more episodes of in-patient and especially of day-patient and out-patient care. Cox's regression showed that the duration of episodes controlled for the history of events and sociodemographic characteristics, was significantly shorter in South-Verona. One of the main conclusion was that hospitalizations for the severely mental ill are also needed in a community-based system of care, supporting the assumption of a 'bed-rock' of mental illness. However, the South-Verona community mental health service seems to be able to reduce the duration of hospitalizations considerably
