Arterial wall thickness and blood pressure in children who were born small for gestational age: Correlation with umbilical cord high-sensitivity C-reactive protein
Background: Small for gestational age (SGA) infants have an increased risk of later cardiovascular disease. At birth, high sensitivity-C reactive protein (hs-CRP), a prognostic marker of cardiovascular disease, is significantly higher in SGA than in appropriate for gestational age (AGA) infants. Aim: To measure aortic and carotid intima-media thickness (aIMT, cIMT) and blood pressure (BP) in children (aged 3-5 years) who were born SGA and AGA, and to assess the correlation between hs-CRP concentrations obtained at birth and these haemodynamic variables. Methods: Umbilical cord hs-CRP concentrations were obtained in 38 neonates. In the same subjects aged 3-5 years, aIMT and cIMT were measured by high-resolution ultrasound scan, in the dorsal arterial wall. Anthropometric variables and BPs were obtained for each child. Results: Maximum (median 0.700 mm, range 0.500-1.080 vs 0.650 mm, 0.400-0.860; p=0.32) aIMTs were similar between children who were born SGA (n=17) and AGA (n=21), respectively. Concentrations of hs-CRP were not correlated with IMTs. In children who were born SGA, systolic BP was significantly correlated with umbilical cord hs-CRP concentrations (r=0.60; p=0.009). Conclusions: Children who were born SGA have a higher, although not significant, aortic thickening than those who were born AGA. Umbilical cord hs-CRP concentrations do not seem to be involved in this process. Instead, hs-CRP concentrations were significantly related to systolic BP values in children who were born SGA, suggesting that hs-CRP at birth could be associated with sympathetic system hyperactivity and with the stress response during childhood
Statistical models of CO concentration in a large urban area
Indications for the intervention strategies to be included in the context of air quality remediations plans for Milan metropolitan area obtained by evaluating 5 years of hourly CO concentration data. The work reports on the analysis of standard excesses and concentration time trends and on the development of empirical statistical models for describing relationships between annual concentration averages and air quality related parameters (number of standard exceedances, extreme percentiles of hourly concentration distributions). The models are applied, in combination with simple roll back equations, for the evaluation of emission reduction required for standards attainment. Similar statistical approaches for describing the dependence of the duration of excess episodes over fixed concentration levels with simple time series parameters (i.e. annual mean concentration) are also reported
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Aesthetic outcomes of non-functional immediately restored single post-extraction implants with and without connective tissue graft : A multicentre randomized controlled trial
Objectives: To compare the 1-year aesthetic results of flapless single implants, placed in fresh extraction sockets with bone replacement and immediate provisional restoration with or without a connective tissue graft (CTG). Material and Methods: The present study proposes a partially blinded multicentre parallel randomized controlled trial, where computer-generated tables were used for central randomization to allocate treatments. A total of 59 of the 60 patients screened by eight private practices in Northern Italy fulfilled the inclusion criteria. Immediate implants were placed in a fresh extraction socket with a non-functional immediate provisional restoration with (Test group) or without (Control group) a CTG. The primary outcome variable was the implant Crown Aesthetic Index (ICAI) at the 1-year follow-up. Results: ICAI for the 59 randomized patients (Test group = 31, Control group = 28) at the 1-year follow-up was 4.69 (95% CI = 3.16–6.22) for the Test group and 3.45 (95% CI = 1.83–5.08) for the Control group, without statistically significant difference between the two groups (p =.086). One implant failure was recorded in each group, resulting in an implant survival rate of 96.8% [95% CI = 83.3–99.9] for the Test group and 96.4% [95% CI = 81.7–99.9] for the Control group. Other secondary outcome variables and complication rates were comparable across the two groups. Conclusions: Within the limitations of the present clinical trial, the results suggested that the adjunct use of CTG is not mandatory to achieve successful aesthetic outcomes for a well-planned immediate implant placement with immediate non-functional provisional restoration in a fresh extraction socket
