The analysis of criminal cases involving crimes committed by individuals with epilepsy had lead to the singling-out of several different issues. First of all, Law tends to consider epilepsy as a psychiatric illness rather than a neurological one and a person with epilepsy as socially dangerous. Further, the set of rules on the basis of which the judge must operate constitute a complex and unharmonious system which is badly adapted to the evaluation of single differentiated cases. Finally, the ease with which precautionary measures and psychiatric evaluations are requested when a subject with epilepsy find himself in front of the Criminal Court, is a clear symptom of the deep need for co-operation between the legal and scientific fields. The latter should make available to all legal operators engaged in issues concerning epileptic persons the wealth of neurological studies carried out in the past 10 years

Antiepileptic therapies can give rise to toxic problems. AED toxicity can be classified into idiosyncratic reactions, dose-dependent side effects, dose-independent side-effects. Furthermore adverse effects during pregnancy have to be taken into account. The AA reviewed the indications of AED and special situations where risks linked with an antiepileptic therapy might be more troublesome than rare epileptic seizures

We investigated the prognostic value of any structural brain abnormality and of the location of the epileptogenic zone. A total of 228 pts with symptomatic localization related epilepsies, from 2 different centers have been included. All the pts included were studied with neuroimaging and EEG monitoring. Seizure control was achieved in 25% of pts. Brain abnormalities, especially Mesial Temporal Sclerosis, are a major prognostic factor. The location of the epileptogenic zone does not seem to be a determining factor. Seizure free periods during the evolution of epilepsy might be the cause of an unfavorable drug reduction with frequent relapses. In drug resistant cases the surgical options should be taken into account early
