Surgical closure of apical ventricular septal defects through a right ventricular apical infundibulotomy
Background: We present a new understanding of the anatomic position of apical ventricular septal defects and its surgical relevance. These defects occur between the left ventricular apex and the infundibular apex, rather than between the left and right ventricular apices. Often a sizable apical recess, the infundibular apex lies anteriorly and inferiorly to the moderator band and is the most leftward part of the right ventricle. Methods: Four patients (2 boys and 2 girls) with a mean age of 109 days (range, 48 to 217 days) underwent patch closure through an apical infundibulotomy, which allowed complete visualization of the muscular apical ventricular septal defect. Results: There were no early or late deaths at operation. No significant residual shunt at ventricular level was detected by postoperative two-dimensional and Doppler echocardiography. Intraoperative comparison of right atrial and pulmonary arterial blood samples showed a difference of less than 5%. At a mean follow-up of 18 months, all the patients are asymptomatic and growing well. Conclusions: The successful outcome of these 4 patients indicates that surgical closure of apical ventricular septal defects can be achieved safely and completely in early infancy through a limited right ventricular apical infundibulotomy. Long-term follow-up of these and similar patients is needed to provide further evaluation of this approach
Thailand's struggle for democracy the life and times of M. R. Seni Pramoj
Former prime minister of Thailand, great-grandson of a Siamese king, and twentieth-century Renaissance man, Seni Pramoj was also the Thai envoy in Washington, D.C. in December 1941 - just after the Japanese bombed Pearl Harbor - who refused, in direct defiance of orders from the Bangkok regime, to declare war on the United States. This political biography portrays one of the world's most fascinating statesmen and depicts Thailand as a nation at war with itself as David Van Praagh traces the history of the country's often thwarted attempts to become increasingly democratic - from the 1930s until after the shocking clashes of May 1992 in the streets of Bangkok between the military and the middle class. Van Praagh also incisively compares other Southeast Asian countries' experiences with democratization and analyzes the implications for Western foreign policyIn addition to presenting the portrait of an extraordinary aristocrat who pointed the way to a rare democratic triumph in Asia, this informed account involves royal figures, corrupt generals, avowed democrats unable to agree on what democracy means, foreign intrigues and Westerners such as Franklin Roosevelt, Lord Louis Mountbatten, and Harry Truman. Never having been colonized by a Western power and long neglected by Western journalists and historians Thailand offers a unique, often surprising histor
Aluminium recovery vs. hydrogen production as resource recovery options for fine MSWI bottom ash fraction
Waste incineration bottom ash fine fraction contains a significant amount of aluminium, but previous works have shown that current recovery options based on standard on-step Eddy Current Separation
(ECS) have limited efficiency. In this paper, we evaluated the improvement in the efficiency of ECS by using an additional step of crushing and sieving. The efficiency of metallic Al recovery was quantified by measuring hydrogen gas production. The ash samples were also tested for total aluminium content with X-ray fluorescence spectroscopy (XRF). As an alternative to material recovery, we also investigated the possibility to convert residual metallic Al into useful energy, promoting H2 gas production by reacting metallic Al with water at high pH. The results show that the total aluminium concentration in the <4 mm bottom ash fraction is on average 8% of the weight of the dry ash, with less than 15% of it being present in the metallic form. Of this latter, only 21% can be potentially recovered with ECS combined with crushing and sieving stages and subsequently recycled. For hydrogen production, using 10 M NaOH at 1 L/S ratio results in the release of 6–11 l of H2 gas for each kilogram of fine dry ash, equivalent to an energy potential of 118 kJ

Modeling longitudinal changes in maximal-intensity exercise performance in young male rowing athletes.
The purpose of the current study was to examine the effect of age and body size upon maximal-intensity exercise performance in young rowing athletes. Male participants (n = 171) aged 12-18 years were assessed using an "all-out" 30-s rowing ergometer test, and reassessed after 12 months. The highest rate of performance development, which amounts to [mean(SD)] +34%(23%) and +32%(23%) for mean and maximal power output, respectively, is observed between the ages of 12 and 13, while this rate of development gradually declines as the athletes mature through adolescence. Performance increases with body size, and mass, stature and chronological age all proved to be significant (all p < .05) explanatory variables of mean power output, with respective exponents [mean(SE)] of 0.56(0.08), 1.84(0.30) and 0.07(0.01), and of maximal power output, with respective exponents of 0.54(0.09), 1.76(0.32) and 0.06(0.01). These findings may help coaches better understand the progression of rowing performance during adolescence
