Approccio videotoracoscopico nel trattamento della sequestrazione polmonare intralobare : a proposito di un caso
The Authors report on the use of a videothorascopic approach in the treatment of a patient with intralobar bronchopulmonary sequestration. The latter is a rare malformation which more often than not manifests itself in young patients with hemophthisis, recurrent infection and cough. The condition is difficult to diagnose and is often diagnosed intraoperatively. The treatment is surgical resection. The videothorascopic approach was used in a young patient presenting a lesion of the left inferior lobe with polycyclic contours suspected of being an intralobar bronchopulmonary sequestration, along with medical history findings of cough and hyperpyrexia. The approach permitted identification of the lesion in the context of the left inferior lobe, safe identification and section of the abnormal systemic vessel supplying the lesion and subsequent inferior lobectomy. The postoperative course was uneventful and the patient was discharged on postoperative day 5 with an excellent esthetic and functional outcome. The videothorascopic approach should be taken into consideration for the diagnosis and treatment of intralobar bronchopulmonary sequestrations
Diagnosi laparoscopica di agenesia della colecisti : caso clinico
Congenital absence of the gallbladder is a rare anomaly. We report a case of agenesis of the gallbladder in a 45-year-old woman incorrectly diagnosed as cholelithiasis on preoperative ultrasound. The diagnosis of agenesis was confirmed laparoscopically with laparoscopic exploration, intraoperative ultrasound and cholangiography. Formal laparotomy was avoided. Difficulties in management during laparoscopic surgery are discussed. A review of the literature is presented
Palm oil : health risks and benefits
Recently, there has been growing discussion on the use of palm oil in the preparation of food products. In this review, we first examine the criticism of palm oil, in particular the supposed contribution to increase cardiovascular disease risk factors because of the high content in saturated fatty acid and the content of potential toxic compound formed during the refining processes, such as glycidols and chloropropanediols. The results of the main publications on these topics demonstrate that the massive attack against palm oil is largely unjustified if palm oil is assumed within the limits of a balanced diet. Finally, we discuss the possible benefits on health of the minor components of palm oil, such as tocopherols, tocotrienols and carotenoids
A new anoscope for transanal surgery
BACKGROUND:
Although stapled mucosectomy has several advantages over hemorrhoidectomy for hemorrhoidal prolapse, complications such as hemorrhage, pain, and life-threatening pelvic sepsis may occur, often due to poorly executed purse-string suture. We describe a simple new anoscope that makes it easy to correctly perform and position the purse-string suture that is an integral part of stapled mucosectomy.
METHODS:
The apex of the middle part of the new anoscope consists of digitiform projections separated by spaces. After insertion of the instrument into the anus, the inner part is removed, allowing strips of rectal mucosa to protrude through the spaces between the digitiform projections. The purse-string suture is made through these protrusions. The suture catches the mucosa and submucosa but not the deeper muscle layer, which does not protrude through the spaces.
CONCLUSION:
Preliminary histologic studies in the pig suggest that the design of the anoscope prevents inclusion of the muscular layer in the pursestring

Local anaesthesia and TEP : is it feasible?
Traumatic diaphragmatic hernias, when diagnosed many years after the traumatic event, are observed in about 10% of diaphragmatic injuries. Due to coexisting injuries and the silent nature of diaphragmatic injuries, the diagnosis is easily missed or difficult. The medical records of 26 patients, who were treated for diaphragmatic hernias during the last 20 years, were analysed retrospectively. The patients were divided into acute phase and late-presenting groups, in whom emergency surgery and elective intervention were performed respectively. Chest radiography was diagnostic in 34.6% (n = 9) of patients. 92.3% of the hernias were on the left side, while the most common herniated organs were the stomach (31.8%) and the colon (27.2%). Coexisting injuries were recorded in 38.4% (n = 10) of the patients. Primary repair was predominantly used (92.3%). The hospitalisation period was longer in the late-presenting group (24.1 ± 18.8 vs. 143 ± 7.7 days). Two deaths occurred in the late-presenting group. Diaphragmatic hernia should be suspected in all blunt abdominal trauma patients. Prompt surgical repair is the treatment of choice in all traumatic diaphragmatic hernias
