Video-assisted mitral surgery through a micro-access: A safe and reliable reality in the current era
Background and aim of the study: Minimally invasive mitral valve surgery was introduced into clinical practice during the mid 1990s. The clinical benefits of the technique, namely a reduction of surgical trauma, increased patient comfort and shorter hospital stay, are achieved by using a video-assisted, mini-thoracotomy approach rather than a standard median sternotomy. Herein is described the authors' experience with video-assisted mitral surgery through a micro-access. Methods: Between September 2003 and September 2006, 100 patients (mean age 65.7 years; range: 16-84 years; 29 aged >75 years) underwent video-assisted port-access mitral valve surgery through a 4- to 6-cm anterior mini-thoracotomy. Mitral valve repair was carried out in 36 patients (36%) and mitral valve replacement (MVR) in 64 (64%) for degenerative (n = 54), rheumatic (n = 44), functional (n = 1) or infective disease (n = 1). Redo procedures were performed in 14 patients. Results: Peripheral extra-thoracic cardiopulmonary bypass (CPB) was used in all cases, and Endoclamp occlusion of the ascending aorta in 94%. The median intensive care unit and hospital stays were 20.0 +/- 30.8 h and 7.0 +/- 5.9 days, respectively. Hospital mortality was 4% (n = 4). No patient required conversion to sternotomy. Five patients (5%) underwent minimally invasive surgical revision for bleeding, and one patient (1%) had an early reoperation for MVR during the immediate postoperative course due to failure of a mitral valve repair. There were no perioperative myocardial infarctions, permanent strokes, major vascular complications, or peripheral ischemic events. Among the patients, 63% had no complications at all during the postoperative course, and no wound infections were observed. Conclusion: Video-assisted mitral surgery through a micro-access may be performed safely, at low risk of morbidity and mortality, and with results and quality standards similar to those reported for a sternotomy approach. Of note, older patients may be successfully treated using this technique
The ISCHEMIA trial revisited: setting the record straight on the benefits of coronary bypass surgery and the misinterpretation of a landmark trial
OBJECTIVES: The ISCHEMIA trial is a landmark study that has been the subject of heated debate within the cardiovascular community. In this analysis of the ISCHEMIA trial, we aim to set the record straight on the benefits of coronary artery bypass grafting (CABG) and the misinterpretation of this landmark trial. We sought to clarify and reorient this misinterpretation. METHODS: We herein analyse the ISCHEMIA trial in detail and describe how its misinterpretation has led to an erroneous guideline recommendation downgrading for prognosis-altering surgical therapy in these at-risk patients. RESULTS: The interim ISCHEMIA trial findings align with previous evidence where CABG reduces the long-term risks of myocardial infarction and mortality in advanced coronary artery disease. The trial outcomes of a significantly lower rate of cardiovascular mortality and a higher rate of non-cardiovascular mortality with the invasive strategy are explained according to landmark evidence. CONCLUSIONS: The ISCHEMIA trial findings are aligned with previous evidence and should not be used to downgrade recommendations in recent guidelines for the indisputable benefits of CABG
Mangroves and aquaculture in Southeast Asia.
The history of brackishwater pond culture in the Philippines and elsewhere in Southeast Asia has been associated with mangrove loss because in earlier times, the larger society has viewed mangroves and other wetlands as wastelands to be developed. In a proactive move to address environmental concerns over shrimp aquaculture, the SEAFDEC Council in 1999 mandated the development of environment-friendly shrimp culture and the build-up of expertise in mangroves.
This paper reviews the status of mangroves and their conversion to culture ponds in Southeast Asia, and the environmental impacts of aquaculture, aside from habitat loss. To improve the sustainability of brackishwater aquaculture, recommendations include an inventory of mangrove and pond culture areas in the region, zonation of mangrove areas within the context of integrated coastal zone management, aqua-silviculture and environmentfriendly aquaculture, rehabilitation of degraded mangroves, and enforcement of existing laws including the buffer zone/mangrove greenbelt requirement
Why consumers have contamination concerns in refurbished personal care products and how to reduce them via design
Refurbishment is a strategy to extend products’ lifetimes. However, refurbished products that are used intimately, such as personal care products, feel uncomfortable to use for consumers because they are perceived to be contaminated. In fifteen in-depth interviews, we explored why consumers have contamination concerns regarding a refurbished Intense-Pulsed-Light device and how to decrease them. Participants expected refurbished personal care products with wear-and-tear to malfunction, to have a shorter product lifetime and to be contaminated. Participants’ inferences differed depending on the location and amount of wear-and-tear. To keep refurbished personal care products at their highest value, we suggest five design strategies to minimize contamination concerns by designing a product that smells and looks hygienic after multiple lifecycles: 1. Using color to evoking associations with hygiene, 2. making wear-and-tear less visible, 3. using smooth materials, 4. minimizing the number of split lines, and 5. a clean product smell.Marketing and Consumer Researc
"Is this alive?": Towards a vocabulary for understanding and communicating living material experiences
Living materials are a nascent material class where living organisms are embedded and kept alive in the design outcome to achieve novel functionalities, expressions, and interactions. Experiential characterisation studies with potential endusers will provide insights for developing these novel materials for meaningful materialapplications. Nevertheless, the current literature lacks a vocabulary to communicate and discuss living materials in user studies. To bridge this gap, our paper presents the development of a “Living Materials Vocabulary” consisting of 45 descriptive items. Through a term frequency analysis of relevant literature and in-depth interviews witheight biodesigners, we identified a set of descriptions which we clustered under fivethemes: origin, making, agency and autonomy, temporality, and impact of living materials. We selected representative items from these themes to compile our final vocabulary. Assessment on the effects of an oil spill in the mangroves of Cordova, Cebu, Philippines
This study was conducted to determine the effects of an oil
spill in mangrove communities in Cordova, Cebu, Philippines in terms of
1) overall community structure2) mapping of degree of oiling3)
sapling, wildlings and tree percentage mortality4) height of oiling on
trees, saplings and wildlings, 5) lenticels conditionand 6)
pneumatophore condition. Results of the assessment documented a total
of ten true mangrove species belonging to 6 families for the
overall species composition. It is only about 28% of the Philippine true
mangrove species. Avicennia marina and Rhizophora stylosa dominated
the areas of the vegetation. Eight months after the spill, only traces of oil
bands remained in the areas where the average height ranged 0-1.08 m in
all plant categories. Total area with dead mangroves was only 0.43
hectares representing 0.29% only of the total mangroves in the
municipality. Overall mortality by species was 5.14% but percentage
mortality differed among plant forms. The observed mortality is one of
the direct acute impacts of the oil spill on a short-term. However, higher
wildling recruitments of Avicennia marina were evident in areas with
dead trees, a possible indication of an early recovery but needs further
monitoring
