Haloperidol for severe anorexia nervosa restricting type with delusional body image disturbance: a nine-case chart review
Here we report on a case series chart review conducted on nine severe and treatment-resistant patients with anorexia nervosa, body mass index <13 kg/m(2), and a delusional body image disturbance. Patients received low doses of haloperidol during hospitalization. Haloperidol was well tolerated. The delusional body image disturbance and the drive for thinness were subjectively perceived as less intense. BMI increased from the initial 12.2 ± 0.5 to 16.0 ± 1.5 kg/m(2). Mean pulse rate and blood pressure did not change significantly from admission to discharge (66 ± 11.6 bpm; 91/56 mmHg vs 77 ± 12.0 bpm; 102/66 mmHg). Mean of QTc, available from electrocardiograms performed during hospitalization, was 413 ± 38.5 ms (range 342-469 ms). Further investigations are warranted to elucidate clinical usefulness and safety of low doses of haloperidol for patients with treatment-resistant anorexia and delusional body image disturbance

ADULT ATTACHMENT STYLE AND SUICIDALITY
Background: There is evidence in the literature that adverse early attachment experiences and subsequent attachment
insecurities during adulthood would lead to pessimism, low self-esteem, hopelessness and, ultimately, to suicide risk.
Subjects and methods: This paper aims to review finding on the link between attachment style and suicidality. We searched the
literature using the database of the U.S. National Center for Biotechnology Information (NCBI)-MedLine/Pubmed system from
January 1992 until December 2016. We started with 1992 because, as far as we know, there are no published studies exploring the
relationship between suicide and insecure attachment before that year. We considered reports published on the relationship between
attachment style and suicidality. We applied several combinations of the following search terms: attachment, adult attachment style
and suicidality, suicide, suicidal ideation, suicidal behavior or suicidal thoughts, and suicide attempts. We selected only English
language studies.
Results: Research suggests that insecure attachment style, mostly anxious, and unresolved traumas are associated with an
increased suicide risk. Few studies prospectively examined clinical course, comorbid psychiatric disorders, familial suicidality or
other psychosocial factors.
Conclusions: Further research is needed to highlight the nature of the link between attachment and suicidality. The presence of
suicidal ideation and attempts might be a consequence of an underlying interaction between the emergence of psychiatrics symptoms,
and the long-lasting presence of inadequate patterns of attachment. Within this context, Separation Anxiety Disorder, categorized in
the DSM-5 as a condition not confined to childhood but as an anxiety disorder that may occur through the entire lifespan, might be
the a key for the comprehension of this link. From a neurobiological point of view, the role of oxytocin remains unclear

Psychological distress among healthcare professionals involved in the COVID-19 emergency: Vulnerability and resilience factors
The aim of this paper is to outline some considerations about the psychological distress in healthcare professional during the Covid-19 pandemic. We summarize available literature both on ‘protective’ and ‘predisposing’ factors potentially involved in the occurrence of psychological distress, including PTSD, in frontline healthcare operators. Valid social support, self-efficacy, internal locus of control (LOC) and sense of coherence (SOC) have been considered as resilience factors, in previous studies. Likewise, several observations pointed on the relevance of individual and environmental vulnerabilities. No real evidence is available about strategies to face the emotional burden for healthcare operators due to present COVID-19 scenario. However, we strongly believe that the containment of isolation anxiety with an appropriate emotional support should be the first instrument to minimise the psychological effect of pandemic on the more exposed healthcare professionals
The value of portable chest roentgenography in adult respiratory distress syndrome. Comparison with computed tomography
In 17 patients with adult respiratory distress syndrome, we used data derived from computed tomographic (CT) scan densitometric analysis to validate the value of portable chest roentgenograms in objectively estimating the amount of pulmonary edema. Chest roentgenograms and CT scans were taken in the same ventilatory conditions (apnea at 10 cm H2O of positive end-expiratory pressure [PEEP]); blood gas samples and hemodynamic parameters were collected at the same time. Roentgenographic analysis was undertaken by independent observers using two standardized scoring systems proposed in the literature. CT scan analysis was performed using the CT number frequency distribution and the gas lung volume (measured by helium dilution technique) to estimate quantitatively the lung density, the lung weight, and the percentage of normally aerated and nonaerated tissue. Knowing the mean CT number of the pulmonary parenchyma in a group of normal subjects, we also inferred the ideal lung weight expected in the study population and computed the excess tissue mass as the difference between actual and ideal lung weight. Both the roentgenographic scoring systems showed direct correlation with the pulmonary impairment as detected by CT scan densitometric analysis (CT number, percentage of nonaerated tissue, lung weight, and excess tissue mass; p less than 0.01) and inverse relation with the percentage of normally aerated tissue (p less than 0.01). We also found a relationship between roentgenographic scores and the impairment in gas exchange as detected by shunt fraction (p less than 0.05). We conclude that standardized reading of portable chest roentgenograms by means of scoring tables is a valuable tool in estimating the amount of pulmonary edema in a patient with adult respiratory distress syndrome
Can sleep disturbance be a cue of mood spectrum comorbidity? A preliminary study in panic disorder
Objective.To investigate if sleep disturbances may affect treatment outcomes of patients with panic disorder (PD).Methods.Eighty-five PD outpatients with no Axis I comorbidity for mood disorders completed a baseline assessment (T1) and were evaluated after 3 (T2), 6 (T3) and 12 months (T4), with the Panic Disorder Severity Scale (PDSS) total score as outcome measure during a 12-month naturalistic follow-up. Patients were assessed with the Mood Spectrum Self-Report (MOODS-SR, Lifetime Version), and the PDSS.Results.Forty-three patients (50.5%) met criteria for remission (PDSS<5) and 42 (49.5%) for no remission. In a logistic regression model with remission as the dependent variable, MOODS-SR sleep disturbances was the only determinant for a lower likelihood of PD remission. The items accounting for this result were the following: Repeated difficulty falling asleep (chi-square = 4.4; df = 1; p = 0.036), and Repeatedly waking up in the middle of the night (chi-square = 5.2; df = 1; p = 0.022).Conclusion.Lifetime sleep disturbances would represent a cue of mood spectrum (in absence of overt affective comorbidity) that may impair remission in PD
