Vineberg Procedure by "Vineberg technique modified by Lobo Filho": morbidity and mortality in the imediate post operative period, angiography results and flow analyze of the left internal thoracic artery implanted Procedimento de Vineberg pela técnica de "Vineberg modificada por Lobo Filho": morbi-mortalidade imediata, resultados angiográficos e análise do fluxo na artéria torácica interna esquerda implantada
BACKGROUND: The Vineberg procedure consists in inserting the internal thoracic artery in the ischemic left ventricle muscle. The main question is the ability of this artery to supply adequate flow for the isquemic miocardium. OBJECTIVES: To evaluate angiographic results of Vineberg procedure by the Vineberg technique modified by Lobo Filho, the morbidity and mortality in the imediate post operative period (POP) and analysis of the flow of the implanted left internal thoracic artery (LITA) at rest and under stress. METHOD: Between September 1999 and April 2002, in our service, eight patients were operated by the above describe technique, in which the implant of the internal thoracic artery in the intimacy of the left ventricle muscle is achieved using a kit used for the introduction of the endocardial leads of pacemakers through the subclavian vein. After six months, they went to angiographic and doppler evaluation. For the LITA study with the doppler we divided the sample into two groups: "Vineberg group", formed by the eigth patients mentioned above; and "control group" formed by 20 patients in whom the LITA recascularize directly the anterior interventricular artery. The angiographic study demonstrade patency of all the grafts in both groups. With the doppler, it was measured the output and flow velocity in the grafts. The analysis of data obtained was taken with T-test for paired and unpaired samples. RESULTS: There were no deaths or complications in imediate POP. The angiographic study showed 100% patency. The total output of Vineberg group was 55% of the ones in the Control group. In both groups, the total output increased with the stress. CONCLUSIONS: The Vineberg technique modified by Lobo Filho can be used with low rates of morbidity and mortality, high index of patency providing a significant blood flow at rest and under stress.INTRODUÇÃO: O procedimento de Vineberg consiste no implante da artéria torácica interna na musculatura isquêmica do ventrículo esquerdo. O principal questionamento acerca desse procedimento é a capacidade de fornecimento de fluxo sanguíneo adequado para o miocárdio isquêmico. OBJETIVOS: Avaliar os resultados angiográficos do procedimento de Vineberg pela técnica de Vineberg modificada por Lobo Filho e a morbi-mortalidade no pós-operatório imediato (POI), bem como analisar o fluxo na artéria torácica interna esquerda (ATIE) implantada, em repouso e estresse. MÉTODO: De setembro de 1999 a abril 2002, em nosso serviço, oito pacientes foram operados pela técnica supracitada, na qual a introdução da artéria torácica interna na intimidade da musculatura do ventrículo esquerdo foi realizada com auxílio de um "kit" destinado à introdução de marcapassos cardíacos endocárdicos definitivos por punção da veia subclávia. Após seis meses, os pacientes foram estudados angiograficamente e submetidos ao ecodoppler da ATIE implantada. Para estudo do fluxo da ATIE pelo ecodoppler, constituímos dois grupos: "Grupo Vineberg", composto pelos oito pacientes citados; e o "Grupo Controle", composto por vinte pacientes, nos quais a ATIE foi utilizada para revascularizar diretamente a artéria interventricular anterior. O estudo angiográfico demonstrou perviabilidade de todos os enxertos em ambos os grupos. No estudo pelo ecodoppler, foram realizadas medidas de débito e velocidade de fluxo no enxerto. A análise dos dados obtidos foi feita pelo teste-T para amostras pareadas e não pareadas. RESULTADOS: Não houve óbitos ou complicações no POI. O estudo angiográfico demonstrou perviabilidade em 100% dos casos. O débito total observado foi cerca de 55% daquele encontrado no Grupo Controle. Em ambos os grupos, o débito total aumentou do repouso para o estresse. CONCLUSÃO: A técnica de Vineberg modificada por Lobo Filho pode ser utilizada com baixas taxas de morbi-mortalidade, alto índice de perviabilidade, oferecendo fluxo sanguíneo significativo em repouso e sob estresse
Origin and assessment of deep groundwater inflow in the Ca' Lita landslide using hydrochemistry and in situ monitoring
Changes in soil water content, groundwater flow and a rise in pore water pressure are well-known causal or triggering factors for hillslope instability. Rainfall and snowmelt are generally assumed as the main sources of groundwater recharge. This assumption neglects the role of deep water inflow in highly tectonized areas, a factor that can influence long-term pore-pressure regimes and play a role on local slope instability. This paper aims to assess the origin of groundwater in the Ca' Lita landslide (northern Italian Apennines) and to qualify and quantify the aliquot attributable to deep water inflow. The research is essentially based on in situ monitoring and hydrochemical analyses. It involved 5 yr of continuous monitoring of groundwater levels, electrical conductivity and temperature and with groundwater sampling followed by determination of major ions (Na+, K+, Mg2+, Ca2+, Cl?, HCO3?, SO42?), tracers (such as Btot and Sr2+), and isotopes (?18O, ?2H and 3H). Leaching experiments on soil samples, hydrochemical modelling and water recharge estimation were also carried out. Results show that the groundwater balance in the Ca' Lita landslide must take into account an inflow of deep and highly mineralised Na-SO4 water (more than 9500 ?S cm?1) with non-negligible amounts of Cl? (up to 800 mg l?1). The chemical and isotopic fingerprint of this water points to oilfield water hosted at large depths in the Apennine chain and that uprises through a regional fault line crossing the landslide area. It recharges the aquifer hosted in the bedrock underlying the sliding surface (at a rate of about 49 000–85 700 m3 yr?1) and it also partly recharges the landslide body. In both the aquifers, the hydrochemical imprint of deep water mixed with rainfall and snowmelt water was observed. This indicates a probable influence of deep water inflow on the mobility of the Ca' Lita landslide, a finding that could be applicable to other large landslides occurring in highly tectonized areas in the northern Apennines or in other mountain chains. The paper demonstrates that hydrochemistry should, therefore, be considered as a valuable investigation method to define hydrogeological limits and the groundwater sources in hillslope and to assess groundwater flow patterns in deep-seated landslides.Water ManagementCivil Engineering and Geoscience

Dzinteres, I. Stangaines, D. Augstkalnes, I. Hadaņonokas, D. Markus, V. Romenkovas un citu izcilu psihologu un pedagogu atziņas par bērnu valodas bagātināšanu rotaļās pirmsskolā. Darbā skatīts trīsgadīgu bērnu psiholoģiskais raksturojums un bērnu valodas bagātināšana rotaļās. Analizēta pedagoģiskā darbība un rotaļu veidi. Pētījuma praktiskajā daļā izmantota vecāku anketēšana, bērnu novērošana izmantojot rotaļas un MK noteikumu Nr. 533 vadlīniju kritērijus. Veikta, pētījuma darba rezultātā, iegūto datu matemātiskā apstrāde. Kā rezultātā tiek atbildēts uz pētījuma darbā izvirzīto jautājumu: Kā rotaļās pirmsskolā bagātināma trīsgadīgu bērnu valoda? Bērna valoda veiksmīgi tiks bagātināta, ja rotaļas pirmsskolā tiks veidotas atbilstoši vecumposma īpatnībām, interesantas, aizraujošas un valodu bagātinošas. Pētījuma bāze ir Siguldas pirmsskolas iestāde no pavasara līdz rudenim, piedaloties 14 bērniem, viņu vecākiem un 1 skolotājai. Kvalifikācijas darbu veido 44 lpp.,18 attēli, 2 tabulas, 5 pielikumi. The theoretical part of the paper contains the analysis of the findings of B. Brice, D. Dzintere, I. Stangaine, D. Augstkalne, I. Hadaņonoka, D. Marku, V. Romenkova and other oustanding educators and psychologists inferences about enriching the language of children in the preschool with games. Work explored psychological characteristics of three-year-old children and children language enrichment with games. Analyzed the pedagogical activity and types of games. In the practical part of the study was used the questionnaire of parents, child observation using games and MK regulation No. 533 guidelines. Used mathematical analysis of the acquired data. As a result answers the question raised in the research paper: How games in preschool enrich language of three-year-olds? The language of the child will be successfully enriched if the games in preschool will be designed according to age peculiarities, and will be interesting, exciting and language enriching. The research base is Sigulda's preschool institution from spring to autumn, with participation of 14 children, their parents and 1 teacher. Integration of Montesori methods for the development of self-care skills in junior Pre-school age 1,5-3 years
Litas Blāzes izstrādātajā kvalifikācijas darbā "Montesori metožu integrēšana pašapkalpošanās prasmju pilnveidošanai jaunākajā pirmskolas vecumā 1,5 - 3 gadi" ir divas daļas. Teorētiskajā autore analizē latviešu un ārzemju avotus, raksturo 1,5-3 gadus vecu bērnu vecumposma īpatnības pētījuma kontekstā, kā arī izzina M. Montesori metodi un principus, salīdzinot tos ar jaunajām vadlīnijām. Empīriskajā daļā, izmantojot anketēšanu, ekspertu intervijas un pedagoģisko novērojumu, tiek pamatoti apstiprināts pētāmais jautājums - vai, papildinot tradicionālo pirmsskolas izglītības programmu ar M. Montesori materiāla elementiem, tiek sekmētas 1,5 - 3 gadus vecu bērnu pašapkalpošanās prasmes. Pētījuma beigās, balstoties uz iegūtajām zināšanām, autore izstrādā principus un modeli grupas vides papildināšanai ar pašapkalpošanās prasmju attīstošu centru. Pētījumu noslēdz secinājumi un priekšlikumi. Pēc veiktā pētījuma autore secina, ka M. Montesori principi un vingrinājumi ir piemēroti, lai tos integrētu vispārizglītojošo pirmsskolas iestāžu mācību saturā jaunākajā vecuma grupā pašapkalpošanās prasmju attīstīšanai un nostiprināšanai. Darbs sastāv no 54 lapām, 3 tabulām, 12 attēliem, 34 izmantotajiem literatūras avotiem un 9 pielikumiem. In the theoretical part, the author analyses Latvian and foreign sources, describes the special characteristics of children aged 1.5-3 in the context of the study, as well as explores Montessori methods and principles by comparing them to the current guidelines. In the empirical part, questionnaire surveys, expert interviews, and pedagogical assessment justifiably provide a confirmation answer to the subject matter – would adding Montessori elements to the traditional pre-school educational program promote the self-care skills of children aged 1.5-3. At the end of the study, by using acquired knowledge the author develops principles and a model for complementing the group’s environment with the self-care skill–developing centre. The conclusions and proposals are indicated at the end of the paper. The author concludes that Montessori principles and exercises are suitable for them to be integrated into the youngest age group’s general education programs for developing and strengthening self-care skills. Darba - Pētījuma mērķis – Teorētiski un praktiski analizēt, un pētīt sociālās prasmes adaptācijas procesā 1.posma izglītojamiem. Autors - Lita Savicka. Darba vadītājs – Lekt. P.i. Jana Buboviča . Teorētiskā daļa autores darbā balstās uz vairāku autoru, zinātnieku un pedagogu (gan latviešu, krievu, ārzemju) atziņām. Autore aprakstīja savā darbā, ko paredz pirmsskolas izglītības iestādes programma bērnu sociālo prasmju bērnu personības attīstībā 1. vecuma posmā. Kāda ir sociālās prasmes būtība un veidi. Galvenais mērķis ir kā atvieglot bērniem no 1,5 – 3 gadu veciem bērniem, kuri nekad nav bijuši pirmsskolas iestādē adaptācijas procesu. Pētījums sastāvēs no teorētiskās daļas, praktiskās daļas un pārējais būs pielikumi. Ieskaitot (attēlus no personīgā arhīva, literatūru, tabulas, dažādiem pielikumiem u.c.). Teorētiskajā daļā autore analizēja 1. posma bērnu attīstības īpatnības, pašu adaptācijas procesu un pedagoģisko darbu caur sociālajām prasmēm un jau zināmajām iemaņām ar kurām bērns jau ir atnācis uz pirmsskolas izglītības iestādi. Pētījuma laikā autore novēroja bērnu sociālās prasmes apguvi un kā pedagogs pielietoja metodes, lai bērniem atvieglotu ātrāku adaptēšanos svešā vidē. Praktiskajā daļā aprakstīja sociālo prasmju metodes no 1,5 – 3 gadu veciem izglītojamiem bērniem pirmsskolas iestādē. Veicot pētījumu analizēja attīstības rezultātus. Tika izveidota tabulas, kurā adaptācijas pakāpē katrs bērns atrodas. Pēc tām varēja noteikt pie kā vairāk jāpiestrādā un kādas metodes jāizmanto, iesaistot arī ģimeni un pārējo ģimenes locekļus. Praktiskās daļas pētījumu rezultātā autore novēroja, ka pašas iejūtīgā komunikācija, smaids un ikdienas ierastais rituāls un neliela samīļošana, palīdzēja ātrāk adaptācijas procesā, arī tiem bērniem kuriem vecāki ātrāk aizgāja savās darba gaitās, bērni grupiņā iejutās labāk un autores klātbūtnē pārāk ilga pārdzīvošana un raudāšana netika novērota. Arī dziesmas ar kustībām un rotaļām ir viena no galvenajām adaptācijas perioda atvieglošanas sastāvdaļām pedagoģiskajā procesā. Beidzot pētījumu darba autore secināja, ka ir jāizmanto vairāk sociālo prasmju metodes, lai pēc iespējas ātrāk iedrošinātu pirmsskolas izglītības 1. posma izglītojamos bērnus. Pedagogam jau savā būtībā jābūt tādam, lai ierodoties svešā vidē nerastos ne mazākās baiļu sajūtas, tad arī adaptācijas periods noritēs daudz vieglāk un bez raizēm.Title of the work topic – Promotion of social skills in the adaptation process, stage 1. After them, it was possible to determine what needs to be worked on more and what methods to use, involving the family and other family members as well. As a result of the research of the practical part, the author observed that her own sensitive communication, a smile and the usual daily ritual and a little caressing helped in the adaptation process faster, also for those children whose parents left for their work sooner, the children felt better in the group and in the presence of the author, they did not experience too much suffering and crying. observed. Songs with movements and games are also one of the main components of easing the adaptation period in the pedagogical process. Finally, the author of the research paper concluded that it is necessary to use more social skills methods to encourage children of the 1st stage of preschool education as soon as possible. The teacher should be such that he does not have the slightest feelings of fear when he arrives in a foreign environment, then the adaptation period will go much easier and without worries
Anticoagulant selection depending on glomerular filtration rate in acute pulmonary thrombembolism
MedicīnaVeselības aprūpeMedicineHealth CareStudējošā pētnieciskā darba autore Lita Saulīte izstrādāja darbu, kura temats ir “Antikoagulantu izvēle atkarībā no glomerulu filtrācijas ātruma pie akūtas plaušu trombembolijas”.
Plaušu artērijas trombembolija ir trešā visbiežāk sastopamā sirds un asinsvadu slimība un paredzamā saslimstība Eiropā un ASV ir vismaz 250-300 tūkstoši gadā[1]. ASV plaušu artērijas trombebolija var izraisīt 300 000 nāves gadījumu gadā, ieņemot augstu vietu strap kardiovaskulārās mirstības cēloņiem[6]. Antikoagulantu lietošana progresējošas nieru mazspējas gadījumā vai pacientiem ar HNS 4.-5. stadiju patlaban ir ierobežota un antikoagulantu terapija hroniskas nieru slimības pacientiem ir izaicinoša, ieguvumu un risku dēļ, jo šai pacientu grupai jau tā ir izmainīta koagulācijas kaskāde[20].
Darba mērķis ir noskaidrot kā glomerulu filtrācijas ātrums ietekmē antikoagulantu izvēli un to devu pielāgošanu akūtas PATE gadījumā.
Studējošā pētnieciskā darba uzdevums ir apkopot pieejamo literatūru, prospektīvi ievākt datus, apjautājot pacientus, analizēt iegūtos datus un veikt secinājumus.
Darba pirmajā daļā apskatīti literatūras avoti un veikta to analizēšana. Apkopota informācija par antikoagulantiem un to dozēšanu pacientiem ar progresējošu hronisku nieru slimību.
Darba otrajā daļā izklāstītas pētījumā izmantotās metodes un veikta pētījumā iegūto datu analīze.

Pulmonary artery thromboembolism is the third most common cardiovascular disease and the estimated incidence in Europe and the USA is at least 250-300 thousand per year[1]. Pulmonary artery thromboembolism can cause 300 000 deaths in the USA per year, ranking high among the leading causes of cardiovascular mortality[6]. Use of anticoagulants in case of advanced renal failure or in patients with CKD 4-5. stages are currently limited and anticoagulant therapy in CKD patients is challenging, due to benefits and risks, as the patient group already has an altered coagulation cascade[20].
The aim of study was to find out how the glomerular filtration rate affects the choice of anticoagulants and the adjustment of their doses in acute PATE.
The tasks of the research are to collect the available literature, collect data prospectively by interviewing patients, analyze the obtained data and draw conclusions.
In the first part of the research work several sources of literature have been examined and analyzed. Collected information on anticoagulants and their dosing in patients with advanced chronic kidney disease.
The second part of the study work sets out the methods used in the study and analyzes the results of the study.
