    Fading correlation of co-located transmitters

    Fading or attenuation of a signal due to environment is a phenomenon often encountered in wireless communications. It is expected that co-located transmitters i.e. transmitters placed very close to each other show a high signal fading correlation due to the presence of similar fading environment. In this thesis, we present an experimental study of this phenomenon. Correlation of received signal strengths obtained from co-located transmitters in dynamically varying environments indicate that the large scale signal variations (shadow fading) are highly correlated while the small scale variations (multipath or fast fading) show a low correlation. Highly correlated large scale variations suggest a presence of same large shadowing elements in the transmit-receive path while a low correlation among the multipath variations is due to mutual coupling between the antennas at very close distances. This has two implications: it suggests that shadow fading variations can serve as an indicator of the co-location of closely spaced transmitters while the multipath variations cannot. However, low multipath signal correlations suggest that antenna diversity could be investigated for implementation in mobile handsets.M.S.Includes bibliographical referencesby Prashant Jadha

    Suture fixation of migrated septal occluder device to prevent further migration: a simple surgical technique

    Abstract As the use of percutaneous intervention is increasing for the closure of the atrial septal defect, the procedure related complications are also on rise, migration of the device being most common. The migrated devices with failed percutaneous retrieval must be removed surgically under cardiopulmonary bypass. During establishment of cardiopulmonary bypass, the handling of heart may cause further migration of the device into other chambers of heart which leads to difficulty in finding and retrieval of the device. The authors propose a simple and unique technique to prevent further migration of the septal occluder device.</p

    Anesthesia for combined cesarean section and pheochromocytoma resection

    Pheochromocytoma (PCC) is a rare cause of hypertension during pregnancy [1:54000 pregnancies]. Fetomaternal morbidity and mortality is about 58% if the diagnosis is missed. Administration of anesthesia to patients with PCC is challenging. Associated pregnancy adds to the problems. This is a case report of a patient having PCC diagnosed at 26 weeks of gestation. With medical management pregnancy was continued till 34 weeks. She was posted for cesarean section and resection of PCC. Patient underwent surgery lasting for 7 h due to inferior vena cava tear and had stormy intra as well as postoperative course. Mother and baby had uneventful recovery due to continuous invasive monitoring and a good teamwork, despite limited anesthetic resources

    Parents' caring approach for their children suffering from pneumonia—A study among Bhil Tribes of Maharashtra

    Context: In India, pneumonia deaths in the past decade show a decreasing trend in the child mortality rate from 74.6 to 45.4. However, NFHS-5 records an increase in prevalence to 2.8% from 2.7% in NFHS-4. The childhood pneumonia control strategies focus on strengthening the health system, skill-building health workers, counseling, and creating awareness about promoting healthy behaviors regarding the management of sick children. Aims: The study attempts to understand “caregivers” care-seeking behavior and the management of childhood pneumonia. Setting and Design: The study was conducted in the Akkalkuwa block of Nandurbar district, Maharashtra, India. We used episodic interviews, asking caregivers to recollect specific events linked to the need for treatment. Methods and Material: A total of 11 in-depth interviews of mothers were conducted whose under-five children had pneumonia in the past year. These interviews used vignettes from real pneumonia cases to discuss community priorities for health care and actions taken to improve child's health. In addition, the qualitative data from the in-depth interviews were thematically analyzed. Result: Cough, breathlessness, and disturbance in the routine schedule of the child were the major symptoms to identify pneumonia (vavlya) among children. Branding on the stomach, oil massage, and jadi-buti were commonly observed phenomena to seek help. Low priority, the influence of traditional healers and herbal medicines, and the inaccessibility of quality healthcare services were the main factors that led to the child's treatment. Conclusion: Culturally appropriate activities are to be imparted on recognition of symptoms and appropriate care seeking, and community health workers need capacity building

    A clinical study of puberty menorrhagia in the modern era at a tertiary health-care center

    Background: Puberty Menorrhagia is the most common gynecological complication. Puberty menorrhagia when accompanied by serious systemic complications such as anemia and high blood protein (i.e., hypoproteinemia) poses substantial difficulties among gynecologists. The foundation of managing puberty menorrhagia is its early diagnosis and individualized treatment customized to each and every patient. The exclusion of pregnancy is mandatory in all cases, regardless of the history, and following up regularly along with a balanced diet and iron therapy goes a long way towards successful treatment. Objectives: To evaluate the etiology, clinical presentation, and therapeutic efficacy in patients suffering from puberty menorrhagia in our program. Materials and Methods: The prospective observation study method was opted for this study. Study participants were forty women who presented to JMF's ACPM Medical College, Dhule between November 1, 2019 and October 31, 2020 with excessive vaginal bleeding between the onset of menarche and 19 years of age. All cases were evaluated using history, physical assessment, and pertinent laboratory investigations. Results: In our study, half of the patients (50%) were between the age group of 13 and 16 years. Nine out of every ten patients had hemoglobin levels of 10 g% or less, and 55% of patients were suffering from puberty menorrhagia for 6 months to a year. Our study suggests that polycystic ovary disease (25%) and anovulatory dysfunctional uterine bleeding (DUB) (47.5%) were the basic etiological factors, resulting in the puberty menorrhagia. Genitourinary tuberculosis (7.5%), idiopathic thrombocytopenic purpura (5%), ectocervical polyps (5%), hypothyroidism (7.5%), and Von Willebrand's disease (2.5%) were found to be a common cause of excessive bleeding. The fundamental medical therapy for managing puberty menorrhagia comprises nonsteroidal anti-inflammatory drugs, tranexamic acids, and hormones. Conclusion: Menorrhagia during puberty is a distressing ailment for both the sufferer and her parents. The majority of instances are caused by self-limiting anovulatory DUB. For effective diagnosis and therapy, a thorough history, clinical examination, and investigation are required. Patient counseling is an integral aspect of management. Surgical approaches are preferred in a few cases, while long-term medical assistance is largely effective

While traditional chemical routes to chiral amines often lack stereoselectivity and require harsh conditions, biocatalytic methods using engineered enzymes can offer high efficiency and selectivity under sustainable conditions. This review discusses recent advances in protein engineering of transaminases, oxidases, and other enzymes to improve catalytic performance. Strategies such as directed evolution, immobilization, and computational redesign have expanded substrate scope and enhanced efficiency. Furthermore, process optimization guided by techno-economic assessments has been crucial for establishing viable biomanufacturing routes.
