    Measuring the impact of managing incontinence on people with dementia and informal caregivers: Cognitive debriefing interviews of ‘ICIQ-Cog’ (English translation)

    Aim:The ICIQ-Cog questionnaire was developed in German to measure the impact of incontinence and/or toilet use problems on people living with dementia and their paid carers and unpaid caregivers; it has been translated into English. This study aimed to (1) examine the linguistic validity of the ICIQ-Cog English translation and (2) determine whether it addresses what is important to unpaid caregivers.Methods:Cognitive debriefing interviews were conducted with 13 unpaid caregivers of people with dementia and problems with incontinence or toilet use recruited via Joint Dementia Research. Interviews were conducted across 3 rounds and transcribed then thematically analysed based on the questionnaire framework.Results:All participants were unpaid caregivers; they highlighted the need for the questionnaire. Changes were made to the title and response options were added to 10 of 12 questions; wording changes were made to 5 questions. One of the questions was deemed difficult to answer as an unpaid caregiver due to the need for more general caring experience; another used phraseology potentially more understandable by professional carers. Several participants emphasised missing questions (such as cost of buying continence aids and extra washing). The interviews were halted because the questionnaire did not fully meet unpaid caregivers needs.Conclusion:This linguistic validation of the ICIQ-Cog questionnaire identified that it is not appropriate for use among unpaid caregivers. A questionnaire to assess impact of managing incontinence designed purposefully for unpaid caregivers of a person with dementia is needed. Further work is required to finalise the English translation for paid carers

    Auswertung von Miktionssymptomen bei Prostatakarzinompatienten drei Jahre nach Therapie mit verschiedenen Behandlungsmethoden : eine prospektive Studie

    Micturition symptoms are common complications that may occur in the course of a therapy for localised carcinoma of the prostate. Consequently these are sometimes hard to treat and impair affected patients physically, but these symptoms have an impact on health-related quality of life, as well. In this prospective thesis research was done on the influence of diverse epidemiological, socio-demographic and oncological parameters, as well as the pre-therapeutic evaluation of individual “Health related Quality of Life” (HRQOL) on micturition symptoms three years after a therapy. Longitudinal process observations were included and additionally categorized based on diverse therapy modalities. This approach is innovative, since as of today mostly merely retrospective studies exist. Furthermore, selected patients still suffering from incontinence symptoms after three years were interviewed on the phone. A total of 183 files from the “Aachen (Aix-la-Chapelle) Prostate Carcinoma Register“ (APCAR), were analyzed. From 1996 onwards, this register listed all patients suffering from prostate carcinoma treated at the Aachen university hospital. Questionnaires that included internationally acknowledged scores, such as the ICSmale and the EORTC, were distributed to interview these men at different points of time about parameters of urine incontinence and their quality of life. These data were correlated with outcome variables previously defined by us. Subsequently, process observations of improvements and degradations were compiled. After correlating the given data, it could be shown that post-therapeutic micturition symptoms three years after the therapy of local carcinoma of the prostate were mainly determined by the choice of the medical treatment (LUTS p=0.0491, Urgency p=0.0006) and, to a lesser degree, by pre-existing problems of incontinence (LUTS p=0.0001, Urgency p=0.0002). Pre-therapeutic data, tumor-descriptive and oncological parameters did not have any influence (all p>0.05). Likewise, the post-therapeutic micturition symptoms essentially influence the health-related quality of life from an opposing perspective. Moreover, the kind of treatment is responsible for the structure of longitudinal processes. With the help of the phone interviews it could be verified that for stressed patients with an ongoing post-therapeutic defiance of micturition symptoms, the implantations of an artificial sphincter should be employed as a valid therapy option for improving the status of incontinence after exhausting the conservative measures, especially when a patient still prefers treatment and shows his willingness to be operated on

    The aim of this survey is to measure the statistic influence of different epidemiological factors including status of health insurance and therapeutic side effects on the Health Related Quality of Life (HRQoL) on patients with localized prostate cancer received either prostatectomy or one form of brachytherapy in the urological clinic of the university clinic of Aachen. Data of patients with a recently diagnosed prostate cancer in the TNM-stadium cT1-cT3 N0 M0 were collected between 1999 and end of 2006. All patients received a self-administered 12-page questionnaire including a HRQoL questionnaire (EORTC QLQ C-30 with the prostate cancer module), questions regarding sexuality and the ICSmale questionnaire for urinary symptoms prior to therapy and 36 months after. Completely filled pre- and post-therapeutic questionnaires of 496 patients could be collected. Patients with a recurrence according to the guidelines of the EAU were excluded from this study. Ultimately a total of 326 data sets were further analyzed. All data are included in a special data base (Aachener Prostatakarzinom Register - APCAR). In a prior study of the same working group the status of health insurance, albeit different between treatment groups could not be defined as an independent variable associated with the treatment method in the urological clinic of the university clinic of Aachen. In addition we find no influence on post therapeutic HRQoL of status of health insurance in this study. In a multivariate analysis we find "sexual functioning" and urgency without leakage as independent significant variables influencing post therapeutic HRQoL. The negative influence of sexual dysfunction on HRQoL is well known and broadly reviewed. Whereas nervesparing techniques have been enhanced urgency without leakage is often clinically neglected and underestimated in literature. Sexual dysfunction and urgency without leakage are therapy associated side effects of the currently available therapy methods and negatively influence post-therapeutic HRQoL. These findings show the importance of a precise informing of patients and their partners of possible therapy associated side effects prior to treatment of localized prostate cancer

    Anwendungsbeobachtung zur Wirksamkeit und Verträglichkeit von ANGOCIN<sup>®</sup> Anti-Infekt N bei Patienten mit Katheter-assoziierter Harnwegsinfektion

    Einleitung In den vergangenen Jahren wurde vielfach über die Zunahme von nosokomialen Infektionen, darunter auch Harnwegsinfektionen (HWI) berichtet. Die Mehrzahl dieser Infektionen ist Katheter-assoziiert, parallel hierzu wird zunehmend von einer Resistenzentwicklung der Erreger gegenüber den gängigen Antibiotika – getriggert durch deren vermehrten Einsatz – berichtet. Vor diesem Hintergrund gewinnen die phytotherapeutische Ansätze zunehmend an Bedeutung. ANGOCIN&#174; Anti-Infekt N ist ein Kombinationspräparat, bestehend aus Kapuzinerkressenkrautpulver und Meerrettichwurzelpulver, zugelassen u.a. bei akuten entzündlichen Erkrankungen der ableitenden Harnwege. Ziel dieser Beobachtungsstudie war die prospektive Dokumentation der Wirksamkeit, Verträglichkeit und Sicherheit von ANGOCIN&#174; Anti-Infekt N bei Katheter-assoziierter HWI. Methode Es wurden 100 Patienten, überwiegend querschnittsgelähmt mit Katheter-assoziierter HWI über einen Zeitraum von 5 Wochen beobachtet. Die Patienten wurden in folgenden Therapiearme eingeteilt: ANGOCIN&#174; Anti-Infekt N als Monotherapie (42%), ANGOCIN&#174; Anti-Infekt N in Kombination mit einem Antibiotikum (29%) und ausschließlich Monotherapie mit einem Antibiotikum (29%). Untersuchte Parameter waren: Anzahl von Leukozyten, Erregerspektrum und Resistenzspektrum im Urin, Entzündungsparameter im Blut (CRP, Leukozyten), Antibiotikaverbrauch, Unerwünschte Ereignisse (UE), Unerwünschte Arzneimittelwirkung (UAW) sowie Wirksamkeit und Verträglichkeit. Ergebnisse Unter reiner Therapie mit einem Antibiotikum und unter Therapie mit ANGOCIN&#174; Anti-Infekt N in Kombination mit einem Antibiotikum waren 100% der HWI im Mittel nach jeweils 9 und 10,5 Tagen ausgeheilt. Unter der Monotherapie mit ANGOCIN&#174; Anti-Infekt N waren 46,2% der HWI im Mittel nach 28 Tagen ausgeheilt. Ein Rezidiv trat bei 79,3% der Patienten aus der Antibiotika-Gruppe auf. In der Gruppe ANGOCIN&#174; Anti-Infekt N in Kombination mit einem Antibiotikum sowie unter ANGOCIN&#174; Anti-Infekt N Monotherapie trat bei 50% der Patienten nach Ausheilung der akuten HWI ein Rezidiv auf. Alle UAW waren von leichter Intensität. Diskussion Eine Therapie mit ANGOCIN&#174; Anti-Infekt N in Kombination mit einem Antibiotikum reduziert im Vergleich zur reinen Antibiotika-Therapie signifikant die Rezidivrate von HWI. Dies deutet auf eine prophylaktische Wirkungsweise von ANGOCIN&#174; Anti-Infekt N hin. Somit stellt ANGOCIN&#174; Anti-Infekt N bei Patienten mit Katheter-assoziierter HWI eine vielversprechende Alternative bzw. Ergänzung zur konventionellen Antibiotikatherapie in der Rezidivprophylaxe dar. Die Erfassung weiterer klinischer Daten ist notwendig, um diese Ergebnisse zu stützen

    Urodynamische Untersuchungen und statistische Erhebungen bei einem Patientenkollektiv mit Morbus Pompe

    Einleitung: In der Literatur sind bei Patienten mit der Glykogenspeichererkrankung Typ II (Morbus Pompe) Beschwerden des unteren Harntraktes (LUTS) beschrieben. Das Ziel dieser Studie ist es, die Blasenfunktion von Patienten mit Late-onset Morbus Pompe (LOPD) erstmals objektiv mit Hilfe von urodynamischen Untersuchungen darzustellen und die Krankheitsverläufe von LOPD-Patienten mit und ohne LUTS zu vergleichen. Hierbei wurde insbesondere ein Schwerpunkt auf das Ansprechen auf die Enzymersatztherapie (ERT) mit rekombinanter humaner alpha-Glukosidase, der einzigen zugelassenen kausalen Therapie bei Morbus Pompe, gelegt. Methode: Von insgesamt 18 LOPD-Patienten gaben 7 LUTS an. Diese 7 Patienten wurden einer ausführlichen Anamnese zugeführt und 6 von ihnen klinisch und mittels Videourodynamik untersucht. Die klinischen Daten dieser 7 LOPD-Patienten mit LUTS wurden mit den Daten von 11 LOPD-Patienten ohne LUTS in einer retrospektiven Langzeituntersuchung verglichen. Es wurden insbesondere Daten zur motorischen- und Lungenfunktion mit einbezogen. Zur statistischen Analyse wurden neben Mittelwerten der t-Test sowie zur Analyse des Langzeitverlaufes ein gemischtes lineares Modell verwendet. Ein p&lt;0,05 wurde als statistisch signifikant angesehen. Ergebnisse: In der Anamnese gaben die Patienten mit LUTS besonders Drang und Dranginkontinenz an. Klinisch zeichneten sich die LOPD-Patienten mit LUTS durch eine stärkere körperliche Einschränkung durch die Grundkrankheit aus, so erzielten sie in motorischen Tests teils signifikant schlechtere Ergebnisse, während die Lebensqualität nicht signifikant schlechter war. Auch zeichneten sie sich durch eine signifikant längere Therapielatenz aus (p=0.017) als die Patienten ohne LUTS. In der Videourodynamik deuten die Ergebnisse insgesamt auf eine neurogene Blasenstörung in diesem Patientenkollektiv hin. So zeigten sich gestörte Wahrnehmungen im Bezug auf die Blasenfüllung im Sinne eines hypo- bzw. hyperaktiven Detrusors und in einem Fall eine neurogene Reflexblase mit Detrusor-Sphinkter-Dyssynergie. Hier lässt sich als Ursache sowohl eine Glykogenablagerung im Blasenurothel als auch im ZNS vermuten. In der Langzeituntersuchung zeichneten sich sehr heterogene Verläufe in beiden Patientenkohorten ab mit Verbesserungen insbesondere in den ersten zwei Jahren, wie es auch in ähnlichen Studien gezeigt werden konnte. Jedoch scheinen die Patienten mit LUTS auch hier einen tendenziell schlechteren und rapideren Verlauf in einigen motorischen Tests und der gemessenen Vitalkapazität zu haben. Schlussfolgerung: Um LUTS und schweren Verläufen bei Morbus Pompe-Patienten vorzubeugen bzw. zu verzögern sollte die Enzymersatztherapie möglichst frühzeitig begonnen werden. Es hat jedoch den Anschein, dass LUTS mit ERT allein nicht ausreichend zu therapieren ist. LUTS müssen bei den Patienten regelmäßig erfragt und ausgeschlossen werden, um möglichen Folgeerkrankungen des oberen Harntraktes vorzubeugen.Urodynamic and clinical studies in patients with late-onset Pompe disease and lower urinary tract symptoms Aims: In late-onset Pompe disease (LOPD), a lysosomal storage disorder with glycogen accumulation in several tissues, patients suffer from progressive skeletal muscle weakness. Lower urinary tract symptoms (LUTS) have rarely been reported. The aim of this study is to objectively assess LUTS in patients with LOPD for the first time using urodynamic studies and to determine differences between LOPD patients with and without LUTS. Methods: Eighteen patients with LOPD were recruited, of whom seven patients (38.9%) reported LUTS (both voiding and storage symptoms). Six of these patients underwent urodynamic studies. Medical histories and motor function tests were compared between the 7 patients with LUTS and the 11 patients without LUTS. The Student t test was used to determine an association between the two cohorts. Results: In the seven LOPD patients with LUTS urodynamics revealed neurogenic dysfunction, underactive detrusor, and bladder outlet obstruction. These patients had suffered from clinical symptoms for a longer period of time before starting enzyme replacement therapy (P=.017) than patients without LUTS. They also scored more poorly on muscle function tests. Urodynamic results point to neurogenic causes for LUTS in LOPD, that is, neurogenic reflex bladder or impaired filling sensation. This could be due to glycogen accumulation in the urothelium and central nervous system. Patients with LUTS also seem to be more severely affected by LOPD than patients without LUTS. Conclusion: LUTS in LOPD requires early and specific treatment to limit the development of severe health problems. Urodynamic studies should be considered in assessing LUTS

    Gibt es klinische Prädiktoren der neurogenen Dysfunktion des unteren Harntraktes bei Patienten mit Multipler Sklerose?

    Hintergrund: MS-Patienten entwickeln häufig neurogene Blasenfunktionsstörungen mit einem erhöhten Risiko für Schäden des oberen Harntraktes. Zur Diagnostik werden Miktionstagebücher, die Uroflowmetrie, der Restharn und die Urodynamik genutzt. Einheitliche Leitlinien gibt es aber nicht. Ziel: Parameter zu identifizieren, die es ermöglichen Patienten mit neurogenen Blasenfunktionsstörungen frühzeitig zu identifizieren. Methode: In einer prospektiven Studie wurden 207 MS-Patienten unabhängig von der urologischen Symptomatik mithilfe des EDSS, der Uroflowmetrie, Restharnvolumen, Anzahl an Harnwegsinfekten innerhalb 6 Monaten, der standardisierten Miktionsfrequenz und dem Miktionsvolumen und der Urodynamik untersucht. Ergebnisse: Eine signifikante Korrelation wurde gefunden zwischen urodynamischen Ergebnissen einhergehend mit Zeichen der neurogenen Blasenfunktionsstörung und dem Restharn (Odds Ratio (OR) 4.17, Konfidenzintervall (CI) 1.20–22.46), Rate an HWI (OR 3.91, CI 1.13–21.0), Miktionsvolumen (OR 4.53, CI 1.85–11.99), erhöhter standardisierter Miktionsfrequenz (OR 7.40, CI 2.15–39.66). Der EDSS zeigte keine Korrelation. Ergebnis: Ein Miktionstagebuch und die Erfassung der Anzahl an Harnwegsinfekten sollte bei allen MS-Patienten regelmäßig erhoben werden

    Comparison of air-charged and water-filled catheters for use in cystometric assessment

    The aim of this study was to determine whether the air-charged and water-filled pressure readings were equivalent. A single catheter system, the commercially available 7-Fr T-Doc air-charged catheter, was tested in different scenarios (like during Valsalva and cough manoeuvres). With this catheter model, simultaneous readings of water and air pressures within the bladder could be achieved. Then the filling and voiding data recordings were compared to each other. The paired-data points of these events were evaluated using the paired t-test, Bland-Altman plots and linear correlation methods, respectively. 25 patients were recruited and examined, 21 patients were included for analysis in the end. Both sets of the system showed a good correlation at Valsalva manoeuvres (R2=0.988, 0.972 for vesical and abdominal pressure respectively) and coughs (R2=0.968, 0.943 for vesical and abdominal pressure respectively). Paired t-test of detrusor pressure (Pdet) showed that there was no statistically significant difference between the two measurements at Valsalva manoeuvres (p=0.85), at initial resting pressure (p=0.13), at 50±10ml resting pressure (p=0.16) and at voiding pressure at maximum flow (Qmax; p=0.51).However, Bland-Altman plots indicated values of a given patient between the two methods could reach up to 5.2 cm H2O and 8.1 cm H2O in vesical and abdominal pressure measurement respectively at Valsalva manoeuvres, to 20 cm H2O and 19.5 cm H2O in vesical and abdominal pressure measurement respectively at coughs. Paired data, in other urodynamic events, also showed similar discrepancies between the two systems. ACC and WFC might be interchangeable for some urodynamic parameters like Pves and Pabd at Valsalva manoeuvres, but not for fast changing pressure signals like coughs. Studies focusing ACCs are still lacking. Missing data from such studies is the main reason why ACCs currently cannot replace WFCs in conventional UDS. However, based on data reported in this thesis, it appears that the catheter systems are equivalent when it comes to diagnosis based on patterns, like for diagnosis of urinary stress incontinence and DO. In case accurate pressure values are needed - like for diagnosis of BOO and calculation of bladder compliance - ACCs appear to be inferior compared to WFCs based on current guidelines
