    Cytodiagnosis of primary thyroid lymphoma coincident with unnoticed papillary thyroid carcinoma: A case report and review of the literature

    Papillary thyroid carcinoma (PTC) is the most common type of thyroid cancer, whereas primary thyroid lymphoma is very rare. Here, we report a case in which a right-sided nodule measuring 4.3 × 2.2 cm was examined using fine-needle aspiration biopsy. This revealed abundant monomorphic non-cohesive large lymphoid cells without thyroid follicular cells, on which basis acytodiagnosis of lymphoma coincident with lymphocytic thyroiditis was made. Subsequent histologic examination revealed CD45−, CD20+, and Bcl-6 + and cytokeratin-, CD3-, CD5-, and CD30-negative tumor cells arranged diffusely in the whole thyroid coexisting with a separate PTC nodule sized 1.3 × 1.0 cm in the right lobe. The key point exemplified by this case is that a cytodiagnosis of this extremely rare coexistence of PTC and lymphoma can be made by adequate sampling of both nodules preoperatively. In our case, only one nodule formation was sampled, and therefore the coexisting PTC was not detected with cytology preoperatively

    Breast cancer and immune thrombocytopenic purpura: Is there any association between these 2 distinct diseases?

    Background: An association between breast cancer and immune thrombocytopenic purpura (ITP) is very rare; there are only 18 cases reported in the English literature so far. Patients and Methods: A 65-year-old woman who had been diagnosed with ITP 5 years ago presented with 2 palpable masses in her left breast. Following establishment of the histopathologic diagnosis of invasive ductal carcinoma by fine needle aspiration biopsy, the patient underwent left modified radical mastectomy. Results: Histopathologic examination confirmed invasive ductal carcinoma in the bigger mass, and in situ ductal carcinoma in the smaller. The case was T2NoMo, therefore exclusive anti-estrogen therapy was started for the next 5 years. However, thrombocytopenia recurred twice in the first postoperative year. Fluorodeoxyglucose positron emission tomography (FDG-PET) scanning showed no breast cancer metastases anywhere in the body. Following immunoglobulin infusion, the platelet count increased enough for surgery, and the patient underwent splenectomy for ITP in the 10th month after the previous breast cancer surgery. After an uneventful postoperative period, the patient has been symptom-free during the 1-year of follow-up with normal platelet levels. Conclusion: Although the relevant current data are conflicting, in the present case no direct relationship between ITP and breast cancer could be observed clinically

    Clinical significance of benign endometrial cells found in papanicolaou tests of Turkish women aged 40 years and older

    Background: Spontaneously exfoliated benign-appearing endometrial cells (BEC) on a Papanicolaou smear might indicate endometrial pathology in postmenopausal women, necessitating further investigation. A cut-off age of 40 years was included in the Bethesda System 2001 based on studies of clinical significance of endometrial cells in Pap smears in Western countries. Aims: The purpose of this study was to determine the significance of age subgroup for women with a cytological diagnosis of BEC, regardless of menopausal status, in a retrospective cohort of Turkish women. Materials and Methods: Between October 2006 and November 2011, 41 patients with a BEC diagnosis and 64 patients with a cytological diagnosis of normal smear (NS) were enrolled; regardless of menopausal status, these women were 40 years and older and for whom follow-up endometrial biopsies had been performed. Results: On subsequent histopathologic evaluation, no malignant lesion was detected in women aged 40-50 years compared to three endometrioid-type adenocarcinomas in women older than 50 years with cytological diagnosis of BEC. There was a significant difference between women older than 50 years with cytologic diagnosis of BEC and NS in relation to premalignant lesions on histopathologic evaluation; however, this was not the case for women aged 40-50 years. Conclusions: According to our study, reporting BEC for women aged between 40 and 50 years has minor clinical significance but is significant for women older than 50 years, regardless of menopausal status. Larger sample size would be appropriate to confirm the results of the current study

    Clinicopathologic and immunohistochemical profile of ovarian metastases from colorectal carcinoma

    Metastasis of colorectal adenocarcinoma of the ovary is not an uncommon occurrence and ovarian metastases from colorectal carcinoma frequently mimic endometrioid and mucinous primary ovarian carcinoma. The clinical and pathologic features of metastatic colorectal adenocarcinoma involving the ovary is reviewed with particular focus on the diagnostic challenge of distinguishing these secondary ovarian tumors from primary ovarian neoplasm. Immunohistochemical stains that may be useful in the differential diagnosis of metastatic colorectal tumors to the ovary and primary ovarian tumors are detailed. (C) 2010 Baishideng.     Isolated metachronous splenic metastasis from synchronous colon cancer

    Abstract Background Isolated splenic metastases from colorectal cancer are very rare and there are only 13 cases reported in the English literature so far. Most cases are asymptomatic and the diagnosis is usually made by imaging studies during the evaluation of rising CEA level postoperatively. Case presentation A 76-year-old man underwent an extended left hemicolectomy for synchronous colon cancers located at the left flexure and the sigmoid colon. The tumors were staged as IIIC (T3N2M0) clinically and the patient received adjuvant chemotherapy. During the first year follow-up period, the patient remained asymptomatic with normal levels of laboratory tests including CEA measurement. However, a gradually rising CEA level after the 14th postoperative month necessitated further imaging studies including computed tomography of the abdomen which revealed a mass in the spleen that was subsequently confirmed by 18FDG- PET scanning to be an isolated metastasis. The patient underwent splenectomy 17 months after his previous cancer surgery. Histological diagnosis confirmed a metastatic adenocarcinoma with no capsule invasion. After an uneventful postoperative period, the patient has been symptom-free during the one-year of follow-up with normal blood CEA levels, although he did not accept to receive any further adjuvant therapy. To the best of our knowledge, this 14th case of isolated splenic metastasis from colorectal carcinoma is also the first reported case of splenic metastasis demonstrated preoperatively by 18FDG PET-CT fusion scanning which revealed its solitary nature as well. Conclusion Isolated splenic metastasis is a rare finding in the follow-up of colorectal cancer patients and long-term survival can be achieved with splenectomy.</p

    Integrated Production and Transportation Scheduling Problem With Multiple Plants, Multiple Vehicles and Perishable Products

    The integrated production and transportation scheduling problem is one of the interesting topics in the literature. This paper addresses Integrated Production and Transportation Scheduling Problems with Multiple Plants, Multiple Vehicles, and Perishable Products (PTSP-MP-MV). To the best of our knowledge, this problem has not been studied in the literature. In this problem, each plant has a different capacity and a different production rate. There is a single machine and a limited number of homogeneous vehicles in each plant. Each vehicle can be used more than once because of the vehicle capacity and planning horizon restrictions. Only one type of product is produced, and this product has a limited lifespan. The objective of the problem is to minimize the total cost including production, distribution, and vehicle costs. We propose a mixed integer programming (MIP) formulation for the problem and strengthen it with several valid inequalities to improve lower bounds. The performance of the formulation is calculated using a newly generated set of test instances. Computational results show that small-sized instances with 5 and 10 customers can be solved optimally in less than 90 s. Satisfactory results are obtained for the test instances up to 20 customers whose optimality gap values are calculated as 0.54% nearly 1 h. It is also shown that the valid inequalities are improved lower bounds about 3%. © 2022, The Author(s), under exclusive license to Springer Nature Switzerland AG.Acknowledgements.
