Energy Minimization in Piperazine Promoted MDEA-Based CO2 Capture Process
A piperazine (PZ)-promoted methyldiethanolamine (MDEA) solution for a carbon dioxide (CO2) removal process from the flue gas of a large-scale coal power plant has been simulated. An Aspen Plus® was used to perform the simulation process. Initially, the effects of MDEA/PZ concentration ratio and stripper pressure on the regeneration energy of CO2 capture process were investigated. The MDEA/PZ concentration ratio of 35/15 wt.% (35 wt. MDEA and 15 wt.% PZ) was selected as an appropriate concentration. The reboiler duty of 3.235 MJ/kg CO2 was obtained at 35/15 wt.% concentration ratio of MDEA/PZ. It was considered a reference or base case, and process modifications including rich vapor compression (RVC) process, cold solvent split (CSS), and the combination of both processes were investigated to check its effect on the energy requirement. A total equivalent work of 0.7 MJe/kg CO2 in the RVC and a reboiler duty of 2.78 MJ/kg CO2 was achieved in the CSS process. Similarly, the total equivalent work, reboiler duty, and condenser duty of 0.627 MJe/kg CO2, 2.44 MJ/kg CO2, and 0.33 MJ/kg CO2, respectively, were obtained in the combined process. The reboiler duty and the total equivalent work were reduced by about 24.6 and 16.2%, respectively, as compared to the reference case. The total energy cost saving was 1.79 M per year

Clinical and cost-effectiveness analysis of an open label, single-centre, randomised trial of spinal cord stimulation (SCS) versus percutaneous myocardial laser revascularisation (PMR) in patients with refractory angina pectoris: The SPiRiT trial
Background: Patients with refractory angina have significant morbidity. This study aimed to
compare two of the treatment options, Spinal Cord Stimulation (SCS) and Percutaneous
Myocardial Laser Revascularisation (PMR) in terms of clinical outcomes and cost-effectiveness.
Methods: Eligible patients were randomised to PMR or SCS and followed up for exercise
tolerance time (ETT), Canadian Cardiovascular Society (CCS) classification and the quality of life
measures SF-36, Seattle Angina Questionnaire and the EuroQoL at 3, 12 and 24 months. Utilities
were calculated using the EQ-5D and these and costs were compared between groups. The
incremental cost-effectiveness ratio (ICER) per QALY for SCS compared to PMR was also
calculated.
Results: At 24 months post-randomisation, patients that had SCS and PMR had similar ETT (mean
difference 0.05, 95% CI -2.08, 2.18, p = 0.96) and there was no difference in CCS classification or
quality of life outcomes. The difference in overall mean costs when comparing SCS to PMR was
GBP5,520 (95% CI GBP1,966 to GBP8,613; p < 0.01) and the ICER of using SCS was GBP46,000
per QALY.
Conclusion: Outcomes after SCS did not differ appreciably from those after PMR, with the former
procedure being less cost-effective as currently applied. Larger studies could clarify which patients
would most benefit from SCS, potentially increasing cost-effectiveness.

Prevalence and correlates of smoking among urban adult men in Bangladesh: slum versus non-slum comparison

BMC Public Health. 2009;9(1):149.Background: Smoking is one of the leading causes of premature death particularly in developing countries. The prevalence of smoking is high among the general male population in Bangladesh. Unfortunately smoking information including correlates of smoking in the cities especially in the urban slums is very scarce, although urbanization is rapid in Bangladesh and slums are growing quickly in its major cities. Therefore this study reported prevalences of cigarette and bidi smoking and their correlates separately by urban slums and non-slums in Bangladesh. Methods: We used secondary data which was collected by the 2006 Urban Health Survey. The data were representative for the urban areas in Bangladesh. Both slums and non-slums located in the six City Corporations were considered. Slums in the cities were identified by two steps, first by using the satellite images and secondly by ground truthing. At the next stage, several clusters of households were selected by using proportional sampling. Then from each of the selected clusters, about 25 households were randomly selected. Information of a total of 12,155 adult men, aged 15 59 years, was analyzed by stratifying them into slum (= 6,488) and non-slum (= 5,667) groups. Simple frequency, bivariable and multivariable logistic regression analyses were performed using SPSS. Results: Overall smoking prevalence for the total sample was 53.6% with significantly higher prevalences among men in slums (59.8%) than non-slums (46.4%). Respondents living in slums reported a significantly (P < 0.001) higher prevalence of smoking cigarettes (53.3%) as compared to those living in non-slums (44.6%). A similar pattern was found for bidis (slums = 11.4% and non-slums = 3.2%, P < 0.001). Multivariable logistic regression revealed significantly higher odds ratio (OR) of smoking cigarettes (OR = 1.12, 95% CI = 1.03-1.22), bidis (OR = 1.90, 95% CI = 1.58-2.29) and any of the two (OR = 1.23, 95% CI = 1.13-1.34) among men living in slums as compared to those living in non-slums when controlled for age, division, education, marital status, religion, birth place and types of work. Division, education and types of work were the common significant correlates for both cigarette and bidi smoking in slums and non-slums by multivariable logistic regressions. Other significant correlates of smoking cigarettes were marital status (both areas), birth place (slums), and religion (non-slums). Similarly significant factors for smoking bidis were age (both areas), marital status (slums), religion (non-slums), and birth place (both areas). Conclusion: The men living in the urban slums reported higher rates of smoking cigarettes and bidis as compared to men living in the urban non-slums. Some of the significant correlates of smoking e. g. education and division should be considered for prevention activities. Our findings clearly underscore the necessity of interventions and preventions by policy makers, public health experts and other stakeholders in slums because smoking was more prevalent in the slum communities with detrimental health sequelae
Factors associated with wife beating in Egypt: Analysis of two surveys (1995 and 2005)

We assessed the rates of wife beating and examined factors associated with wife beating in 1995 and 2005 in Egypt. METHODS: We used data from two Demographic and Health Surveys (DHS) conducted in Egypt in 1995 and 2005 using multistage household sampling. Data related to wife beating included information from 7122 women in 1995 and 5612 women in 2005. Logistic regression was used to analyze factors independently associated with wife beating. Special weights were used to obtain nationally representative estimates. RESULTS: In 1995 17.5% of married women in Egypt experienced wife beating in the last 12 months, in 2005--18.9% or 16.0%, using different measures. The association between socio-demographic differentials and wife beating was weaker in the newer survey. The 12-month prevalence of wife beating was lower only when both partners were educated, but the differences across education levels were less pronounced in 2005. Based on the information available in the 2005 survey, more educated women experienced less severe forms of wife beating than less educated women. CONCLUSION: Different measures used in both surveys make a direct comparison difficult. The observed patterns indicate that the changes in prevalence may be masked by two opposite processes occurring in the society: a decrease in (severe forms of) wife beating and an increase in reporting of wife beating. Improving the access to education for women and raising education levels in the whole society may help reducing wife beating
