    On a blustery fall morning, RTB welcomed Christine Walley, anthropologist and author of Exit Zero: Family and Class in Postindustrial Chicago. In the early 1980s Chris's father, along with thousands of other steel workers, lost his job when the mills in Southeastern Chicago closed. The book is part of a multimodal project, including the documentary film, Exit Zero: An Industrial Family Story, (with director Chris Boebel) and an NEH-funded digitization project of the Southeastern Chicago Historical Museum, a community-based archive of materials related to the neighborhood. How can academics begin conversations about class and deindustrialization with those most negatively affected by the precarious economic present? What is the secret to unpacking the great diversity hidden behind the phrase "white working class"? This episode's signature RTB move (fleeing the present, only to discover echoes of its misery back in the past) takes us to Elizabeth Gaskell's novel North and South, published in 1854 just as industrialization in the North of England was taking off. In Recallable Books, Elizabeth lingers in England's North to recommend George Orwell's The Road to Wigan Pier. Chris points out how Jane Addams's Twenty Years at Hull House (though perhaps patronizing in some ways) shows us 19th century projects for combating the dislocation and suffering of deindustrialization.     Aging and the intramyocardial inflammatory response

    The sepsis-induced intramyocardial inflammatory response results in decreased ventricular function and myocardial damage. Chemokines such as monocyte chemoattractant protein-1 causally contribute to retention of intramyocardial mononuclear leukocytes and subsequent ventricular dysfunction during endotoxemic shock in mice and, importantly, this effect is age dependent. It is therefore useful to consider where monocyte chemoattractant protein-1 fits in the complex pathway leading to ventricular dysfunction during sepsis, why this might be an age-dependent effect, and what this implies for care of older sepsis patients.Other UBCReviewedFacult

    Left ventricular function: time-varying elastance and left ventricular aortic coupling

    Many aspects of left ventricular function are explained by considering ventricular pressure–volume characteristics. Contractility is best measured by the slope, Emax, of the end-systolic pressure–volume relationship. Ventricular systole is usefully characterized by a time-varying elastance (ΔP/ΔV). An extended area, the pressure–volume area, subtended by the ventricular pressure–volume loop (useful mechanical work) and the ESPVR (energy expended without mechanical work), is linearly related to myocardial oxygen consumption per beat. For energetically efficient systolic ejection ventricular elastance should be, and is, matched to aortic elastance. Without matching, the fraction of energy expended without mechanical work increases and energy is lost during ejection across the aortic valve. Ventricular function curves, derived from ventricular pressure–volume characteristics, interact with venous return curves to regulate cardiac output. Thus, consideration of ventricular pressure–volume relationships highlight features that allow the heart to efficiently respond to any demand for cardiac output and oxygen delivery.Other UBCReviewedFacult

    Deeper understanding of mechanisms contributing to sepsis-induced myocardial dysfunction

    The inflammatory response of sepsis results in organ dysfunction, including myocardial dysfunction. Myocardial dysfunction is particularly important in patients with severe septic shock who progress to a hypodynamic pre-terminal phase. Multiple aspects of this septic inflammatory response contribute to the pathogenesis of decreased ventricular contractility. Inflammatory cytokines released by inflammatory cells contribute as does nitric oxide released by vascular endothelium and by cardiomyocytes. Endotoxins and other pathogen molecules induce an intramyocardial inflammatory response by binding Toll-like receptors on cardiomyocytes that then signal via NF-κB. These processes alter cardiomyocyte depolarization and, therefore, contractility. The particular role of the cardiomyocyte sodium current has not been characterized. Now new information suggests that the septic inflammatory response impairs normal depolarization by altering the cardiomyocyte sodium current. This results in decreased ventricular contractility. This is important because new targets for therapeutic intervention can be considered and new approaches to evaluation of this problem can be contemplated.Other UBCReviewedFacult

    For Bayesians, rational modesty requires imprecision

    Gordon Belot (2013) has recently developed a novel argument against Bayesianism. He shows that there is an interesting class of problems that, intuitively, no rational belief forming method is likely to get right. But a Bayesian agent’s credence, before the problem starts, that she will get the problem right has to be 1. This is an implausible kind of immodesty on the part of Bayesians. My aim is to show that while this is a good argument against traditional, precise Bayesians, the argument doesn’t neatly extend to imprecise Bayesians. As such, Belot’s argument is a reason to prefer imprecise Bayesianism to precise Bayesianism.Peer reviewe
