A cadaveric study of morphology of portal vein with its clinical importance
Introduction: The morphological mensurative data of the portal venous system is important, as regards portal hypertension and surgical aspects related to portocaval anatomosis. Aims and Objectives: The study under consideration was under taken to determine the actual percentage incidence of major variations occurring in the disposition of the portal vein (PV) and its tributaries. The basis of major variations has different reasons such as genetic, metabolic, hemodynamic factors depending on the type of variations. Materials and Methods: This study was conducted on 30 adult human cadavers by dissection method. The abdomen was opened by standard cruciform incision, and the region of PV was cleaned and observations were made and recorded. Result: The PV had normal standard pattern in 30% cases and the rest 70% showed variations in one or the other form. The splenic vein was variant in 76.6% cases. The superior mesenteric vein showed variation in 73.3% cases. The mode of termination of the inferior mesenteric vein was variable in 60% of cases. Conclusion: The incidence of variations in the portal venous system can be of prime surgical and radiological importance for portocaval anastomosis and to study hemodynamic flow radiologically
Estimation of Total Length of the Femur from its Distal Segmental Measurements in Adult Population: A Morphometric Study
Aims and Objectives:
The aim of the study was to estimate the entire length of the femur from the parameters of its lower end after determining their association.
Methods and Materials:
The study used 200 mature, dry, completely ossified femora from an unknown sex. Twelve parametric variables related to the lower end were discovered. The measurements collected were statistically examined.
Results:
The measurements differed on both sides; however, the differences were not statistically significant except for the distal segment’s total length, lateral condyle height, and circumference. All of the parametric factors at the lower end of the femur show a positive relationship with the overall length of the distal segment, and a regression equation has been established.
Conclusions:
The study provides a thorough analysis of the morphometry of the lower end of the femur. These criteria can be utilized to develop knee prostheses and plates appropriate for the adult population
Bilateral communication between the mylohyoid and lingual nerves: Clinical implications
Although variations in the course of the mylohyoid nerve (MHN) in relation to the mandible are frequently found on dissection but these anatomical variations have not been conclusively described in the anatomical or surgical literature. However, it is a well-known fact that variations in the branching pattern of the mandibular nerve frequently account for the failure to obtain adequate anesthetic effect in routine oral and dental procedures and also for the unexpected injury to the branches of the nerves during surgery. Furthermore, anatomical variations might be responsible for unexpected and unexplained symptoms after a certain surgical procedures and in some cases of refractory neuralgias. We describe the presence of a rare bilateral communicating branch between the MHN and lingual nerves (LN) in an adult male cadaver, with a brief discussion of its anatomical, clinical and surgical implications as well as its possible role in the sensory innervations of the tongue. The present case reinforces the idea of a communicating branch between the MHN and LN, indicating that some of the sensory components of the MHN, instead of innervating the teeth or chin skin, might also innervate the tongue. This anatomical variation mandates to update the knowledge and awareness among surgeons and anesthesiologists who are frequently involved in oro-surgical procedures and nerve blocks of the face for various neuralgias so as to avoid any un-anticipated nerve injury
Cephalothoracoomphalopagus: A noninvasive study
Conjoined twins are rare variants of monozygotic twins, which result from an incomplete late division of the embryonic disc. Here, we report a rare case of conjoined twins of cephalothoracoomphalopagus variety where the line of fusion extended from the cranium up to the level of the umbilicus. The crown-rump length of each twin was 23 cm, which corresponded to the prenatal age of week 24 in a normal gravidity. The anatomical features of these conjoined twins were observed and external anomalies such as indefinite single nostril and male hypoplastic genitalia were described and documented. The old formalized specimen was studied to reveal anomalies by using current imaging techniques, thus keeping it intact for posterity. Computed tomography (CT) findings suggest cephalothoracopagus twinning, with the gestational age being 24 weeks (approximately). An attempt was made to correlate the findings on embryological basis
Isolated sporadic bilateral split hand malformation: a case report and review of the literature
Typical cleft hand is a rare disorder usually inherited in an autosomal-dominant manner. We report a case of bilateral typical cleft hand in a 6-year-old male. There was no positive family history of such presentation, and no systemic manifestations were associated with this entity. Preanesthetic evaluation revealed no systemic or airway challenges and parents were counseled for routine surgical and anesthetic risks. The hands underwent successful surgical reconstruction with no peri-operative complications. To the best of our knowledge, this is the first report of an operated case of an isolated, bilateral, and sporadic typical split hand malformation
