Rehabilitation of a Patient with Completely Edentulous Maxillary Arch using “All on 4” Concept of Implantation
ABSTRACT
Treatment options for insufficient ridge morphology includes the use of short implants, vertical ridge augmentation procedures, or cantilever prostheses in completely edentulous patients. Due to the less predictable long-term prognosis associated with the above-mentioned procedures, the “All on 4” technique was proposed for the rehabilitation in edentulous jaws. The All on 4 treatment concept meets the patient requirements with an immediately loaded fixed prosthesis supported by four implants. This article depicts a clinical report in which an “All on 4” implant treatment is done with delayed loading protocol due to inadequate primary stability obtained.

PROSTHETIC REHABILITATION OF EDENTULOUS HEMI MANDIBULECTOMY PATIENT USING AN UNCONVENTIONAL DENTURE
The restoration of normal function and aesthetics is often challenging for patients with hemi mandibulectomy due to mandibular rotation, deviation, and unstable occlusion. The successful rehabilitation of a patient with hemi mandibulectomy depends upon the extend of surgical defect, degree of tongue impaired, presence of remaining natural teeth, treatment plan, type of prosthesis, patient co-operation, and duration for initiation of treatment. Prosthodontic treatment along with an exercise program helps to reduce mandibular deviation and thereby improves efficiency of masticatory function. CORRELATION OF THE DISTANCE BETWEEN PTERYGOMAXILLARY NOTCHES AND THE COMBINED WIDTH OF MAXILLARY ANTERIOR TEETH IN DENTATE SUBJECTS - AN OBSERVATIONAL ANALYTICAL STUDY
Purpose: The objective of this study was to find the correlation of the distance between pterygomaxillary notches and the combined width of maxillary anterior teeth.Material &Methods:One hundred and twenty-six maxillary dentulous impressions were made (18-50 years old) using metallic stock tray and irreversible hydrocolloid impression material, then poured in dental stone. The distance between Pterygomaxillary notches and the mesiodistal width of six maxillary anterior teeth were measured by digital vernier calliper on a dental stone cast. The mean of the measurements was statistically analysed using Pearsons correlation co-efficient.
Results: The mean distance between pterygomaxillary notches was 39.5 ± 3.7mm and the mean of combined width of six maxillary anterior teeth was 45.5 ± 2.A moderately significant correlation was found for the distance between pterygomaxillary notches and the combined width of maxillary anterior teeth (p<0.01, r=0.335).Only 11.2 percent of the samples showed a correlation between the combined width of maxillary anterior teeth and the distance between the pterygomaxillary notches.
Conclusions: The relation between the combined width of maxillary anterior teeth and distance between the pterygomaxillary notches was significant (p<0.01). The distance between pterygomaxillary notches can influence the size of maxillary anterior teeth along with other factors.It was concluded that for every 1mm increase in the distance between the pterygomaxillary notches, there will be a 0.18mm increase in the dimension for the combined width of maxillary anterior teeth.Distance between pterygomaxillary notches is a reliable method for the selection of maxillary anterior teeth.
Clinical Relevance To Interdisciplinary Dentistry:
1. This can be used for teeth selection in patients with nasal, eye defects where inter-alar/ inter-pupillary width is not reliable.
2. This correlation can be applied to any restorative procedures involving rehabilitation of maxillary anterior teeth
Chemotherapy-induced adverse drug reaction of antineoplastic drugs among oncology patients in a tertiary care teaching hospital
Background: The main objectives of the study were to assess the pattern of adverse drug reactions (ADRs) associated with the use of antineoplastic drugs along with their causality and severity. The study also aims to analyse the different stages of cancer, cycles of chemotherapy and the quality of life of the patients.
Methods: This prospective, cross-sectional and descriptive study was conducted among 100 patients of either sex for 6 months from April 2023 to October 2023. This study included patients above the age of 18 years who were clinically diagnosed with cancer and prescribed at least one antineoplastic drug.
Results: Most of the cancer patients belonged to stage 2 and were undergoing the third cycle of chemotherapy. Most common ADRs were alopecia, anemia and anorexia. The study revealed that 79% of ADRs were possible and nearly 21% were probable in nature. After the severity assessment, it was found that all of the ADRs were mild (level 1). It was also identified that quality of life (QoL) decreases with advancing stages of cancer.
Conclusions: Cancer, along with therapeutic agents, can impair a patient's quality of life, prompting the development of effective management strategies. Assessing the severity and causation of ADRs can help determine the right supportive care agent to improve patients' general well-being
Prosthetic rehabilitation in neurosurgical cranioplasty
The defects of the skull cause mechanical vulnerability of the brain, esthetic disfigurement, and transmission of vibrations and pulsation of the brain. Subsequent cranioplasty may be required to compensate for the defect and to alleviate various signs and symptoms. When long-term outcome of biomaterial use in pediatric cases is limited, alloplastic cranioplasty in adults are supported by several large case series. A comparative evaluation of the marginal adaptation of a thermoplastic resin, a light cured wax and an inlay casting wax on stone dies: An in vitro study
Background: Different pattern materials do not produce copings with satisfactory, marginal accuracy when used on stone dies at varying time intervals.
Purpose: The purpose of this study was to evaluate and compare the vertical marginal accuracy of patterns formed from three materials, namely, thermoplastic resin, light cured wax and inlay casting wax at three-time intervals of 1, 12, and 24 h.
Methodology: A master die (zirconia abutment mimicking a prepared permanent maxillary central incisor) and metal sleeve (direct metal laser sintering crown #11) were fabricated. A total of 30 stone dies were obtained from the master die. Ten patterns were made each from the three materials and stored off the die at room temperature. The vertical marginal gaps were measured using digital microscope at 1, 12, and 24 h after reseating with gentle finger pressure.
Results: The results revealed a significant statistical difference in the marginal adaptation of three materials at all the three-time intervals. Light cured wax was found to be most accurate at all time intervals, followed by thermoplastic resin and inlay casting wax. Furthermore, there was a significant difference between all pairs of materials. The change in vertical marginal gap from 1 to 24 h between thermoplastic resin and light cured wax was not statistically significant.
Conclusion: The marginal adaptation of all the three materials used, was well within the acceptable range of 25–70 μm. The resin pattern materials studied revealed significantly less dimensional change than inlay casting wax on storage at 1, 12, and 24 h time intervals. They may be employed in situations where high precision and delayed investing is expected
